OBJECTIVE: To study external skeletal fixation for long tubular bony defect by end to end compression with lengthening (EECL) and segmental bone transport by lengthening (STBL).
METHODS: About 20% - 30% of the original length was resected from the middle shaft of the tibia in 10 goats of 1 - 2 years of age. Percutaneous epiphysiodesis using transphyseal screws (PETS) has been developed for the treatment of lower limb discrepancies with the aim of replacing traditional open procedures. In a prospective study 20 patients were regularly monitored with sonograms in four planes and radiographs during distraction osteogenesis of the lower limb. Ultrasonography is more sensitive than radiography in identifying new bone formations during distraction, in measuring the length of the distraction gap, and in detecting early bone healing complications, and can therefore reduce the need for radiographs. Radiographs are necessary to assess the time of fixator removal and the axis of the leg.

Application of distraction osteogenesis to the human craniofacial skeleton in properly selected cases represents a major advance in the treatment of craniofacial deformities. We report our initial clinical experience with a system of miniature distraction devices that permitted maxillary, orbital, and mandibular distraction in a 4-month-old boy with unilateral craniofacial microsomia and anophthalmia. At 6 months of age, after maxillary repositioning and orbital expansion, a costochondral rib graft was used to construct the missing left mandibular ramus and condyle.

We report the outcome of ulnar lengthening with the Ilizarov technique in five adults with radial hemimelia and previous wrist centralization. The indications for ulnar lengthening in these patients were either a functional deficit due to the short ulna or poor appearance that caused the patient significant unhappiness. All patients had a successful lengthening of the ulna, with a gain in length from 4 to 13 cm; however, the procedures were prolonged (7 to 25 months), and all patients experienced complications. At the time of follow-up examination no patient had residual pain or paresthesias.

Limb lengthening takes careful planning and meticulous outpatient care and still is burdened by minor complications. Experience may lessen major complications, but pin tract problems, swelling, and pain will plague these patients. A large group of patients can benefit from lengthening procedures, and new techniques are improving the risk-benefit balance. A thoughtful approach is needed by the surgeon, especially taking time to fully inform the patient about the time, effort, and problems involved in substantial limb lengthening.
