Clinical outcomes of perforator-based propeller flaps versus free flaps in soft tissue reconstruction for lower leg and foot trauma: a retrospective single-centre comparative study.

BACKGROUND: The efficacy and safety of perforator-based propeller flaps (PPF) versus free flaps (FF) in traumatic lower leg and foot reconstructions are debated. PPFs are perceived as simpler due to advantages like avoiding microsurgery, but concerns about complications, such as flap congestion and necrosis, persist. This study aimed to compare outcomes of PPF and FF in trauma-related distal lower extremity soft tissue reconstruction. METHODS: We retrospectively studied 38 flaps in 33 patients who underwent lower leg and foot soft tissue reconstruction due to trauma at our hospital from 2015

Prevalence and a correlation analysis of discoid meniscus and femoral trochlear dysplasia.

BACKGROUND: Discoid meniscus (DM) and femoral trochlear dysplasia (FTD) are common knee disorders. Both as congenital malformation, whether there is a connection between them is unclear and the research on their prevalence in the general population is inadequate. This study aimed to investigate the prevalence of FTD and DM in the general population through a large sample size, and to explore the relationship between them. STUDY DESIGN: Retrospective study. METHODS: Patients undergoing knee magnetic resonance imaging (MRI) examinations at our outpatient clinic were screened and 1003 patients

Effectiveness of orthopedic implant removal surgery in patients with no implant-related symptoms after fracture union of isolated lower extremity shaft fractures: patient-centered evaluation.

INTRODUCTION: Routine orthopedic implant removal surgery (OIRS) in patients with no implant-related symptoms after union is controversial. Various factors, such as various fracture sites, fracture types, and accompanying fractures at other sites, act as error variables in the evaluation of routine OIRS. The purpose of this study is to evaluate the benefits of orthopedic implant removal surgery (OIRS) in patients with no implant-related symptoms such as infection and soft tissue irritation after union of isolated lower extremity shaft fractures from a patient-centered evaluation MATERIALS AND M

Effect of guided growth intervention on static leg alignment and dynamic knee contact forces during gait.

BACKGROUND: Lower limb malalignment in the frontal plane is one of the major causes of developing knee osteoarthritis. Growing children can be treated by temporary hemiepiphysiodesis when diagnosed with lower limb malalignment. RESEARCH QUESTION: Is there a difference between medial or lateral knee contact force (KCF) before (PRE) and after (POST) hemiepiphysiodesis in patients with valgus malalignment and compared to a typically developed control group (TD)? Does a linear relationship exist between the static radiographic mechanical axis angle and dynamic medial/lateral KCF? METHODS: In thi

Elongation of the Long Bones in Humans by the Growth Plates.

The disk of hyaline cartilage that is interposed between the epiphysis and the metaphysis of each of the long bones is responsible for its elongation, and, thus, when the lower limbs are concerned, for increases in bodily height. This so-called growth plate is avascular, aneural, and alymphatic. It consists solely of chondrocytes and an extracellular matrix which the cells elaborate. The growth plate is architectonically striking in so far as the chondrocytes are aligned in strictly vertical columns, which represent the functional units of longitudinal bone growth. The growth process begins wi

Bone Stress Injuries in Runners.

Bone stress injuries (BSIs) are common running injuries and may occur at a rate of 20% annually. Both biological and biomechanical risk factors contribute to BSI. Evaluation of a runner with suspected BSI includes completing an appropriate history and physical examination. MRI grading classification for BSI has been proposed and may guide return to play. Management includes activity modification, optimizing nutrition, and addressing risk factors, including the female athlete triad. BSI prevention strategies include screening for risk factors during preparticipation evaluations, optimizing nutr

Easy interpretation of heterotopic ossification demonstrated on bone SPECT/CT.

A 29-year-old male patient suffered from paraplegia 6 months after brain injury. Heterotopic ossification was diagnosed based on clinical characteristics such as decreased range of motion at the bilateral hips without infective signs. A planar bone scintigraphy using Tc-MDP showed intensive radiotracer accumulation in the proximal portion of the bilateral femurs and the right acetabulum. The SPECT/CT images showed increased radiotracer accumulation in the adjacent soft tissues of the bilateral hips but not in the bone. The conservative treatment rather than surgical intervention was given acco

Bone fracture characterization under mixed-mode I+II loading using the single leg bending test.

Fracture under mixed-mode I+II was induced in bovine cortical bone tissue using a developed miniaturized version of the single leg bending test (SLB). Due to the difficulty in crack length monitoring in the course of the test, an equivalent crack method based on specimen compliance and beam theory was adopted as a data reduction scheme. The method was applied to the experimental results in order to obtain the Resistance curves in each loading mode. The determined fracture energy is well described by an energetic power law whose exponent is below one, which means that the linear energetic crite

Congenital posteromedial bowing of the tibia and fibula: treatment option by multilevel osteotomy.

Few papers have described patients treated surgically with single osteotomy for congenital posteromedial bowing of the tibia and fibula. Only one paper has described two-level osteotomy for deformity correction: the first for deformity correction and the second for bone lengthening. There are no publications describing the surgical correction of deformation only by the method of multilevel tibial and fibular osteotomy. Research material included four children aged between 3.1 and 5.1 years (average age: 3.7 years) who were operated upon for bowing of the tibia and fibula exceeding 35° in the c

Management strategies for osteochondritis dissecans of the knee in the skeletally immature athlete.

SYNOPSIS: Osteochondritis dissecans (OCD) is a disorder resulting in focal breakdown of the subchondral bone, with potential disruption of the overlying articular cartilage. The femoral condyles of the knee are the most common locations for OCD, and the incidence of OCD appears to be increasing among active children. Juvenile OCD (JOCD) can be distinguished from adult OCD by the presence of open growth plates. Due to a lack of evidence on its early diagnosis, optimal treatment, and long-term course of healing, JOCD presents a unique challenge for the health care team. Approximately 50% to 67%

Free fillet foot flap for salvage of below-knee amputation stump.

Management of extensive bone and soft tissue defects, which occur after severe trauma of lower extremities and always lead to an unacceptable amputation in some cases, continues to challenge reconstructive surgeons. When performing lower extremity amputation, preservation of the knee joint has been put into a higher priority. The benefit of below-knee amputation over above-knee ones concentrates on a more normal gait with less energy expenditure during ambulation when a functioning knee joint is present.

Gait analysis and muscle weight analysis after lower extremity fractures in a small animal model.

BACKGROUND: Besides adequate healing of bone and soft tissues, mobility represents a significant factor in functional outcome after lower extremity fractures. Although gait analysis is gaining clinical interest and importance in the rehabilitation of patients with fractures, it is rarely used in experimental fracture healing research. The aim of this study is to establish an accurate gait analysis method for fracture healing research in small animal models and to evaluate the influence of a lower extremity fracture on gait pattern and muscle atrophy in rats. RESEARCH QUESTION: How does an int

Reamer-irrigator-aspirator bone graft and bi Masquelet technique for segmental bone defect nonunions: a review of 25 cases.

INTRODUCTION: Segmental bone loss, either from trauma, tumor or infection is a challenging clinical entity. Amputation is a possible outcome and part of the decision making process. Surgical management is almost always needed and can require several interventions to obtain bone union. A staged protocol of obtaining a clean viable soft tissue bed, placement of a PMMA antibiotic impregnated spacer to induce a neovascular and bioactive membrane followed by autogenous bone graft has been reported with good outcomes. Our study attempts to expand on this data by evaluating the use of RIA bone graft

Orthogonal double plate fixation for long bone fracture nonunion.

PURPOSE OF THE STUDY: The treatment of long bone diaphyseal fracture-nonunion is challenging. While taking into account biological needs, a stable mechanical environment is pertinent for fracture healing. This work aims at evaluating the surgical management of stubborn ununited fractures using orthogonal double plating of diaphyseal fractures with limited periosteal stripping and soft tissue dissection. PATIENTS AND METHODS: Retrospective analysis in a level I trauma center. Between the years 2007-2009, 22 patients were treated with double plating due to nonunion of long bone fractures. Long
