Burkholderia (Pseudomonas) cepacia as a cause of brain abscesses secondary to chronic suppurative otitis media.

A patient with a cholesteatoma secondary to chronic suppurative otitis media developed infective complications including cerebellar and cerebello-pontine abscesses. Burkholderia cepacia was isolated from ear swabs, infected tissue and surgical pus on five occasions over a six-month period. Despite exhibiting three different antibiotic susceptibility profiles, all five isolates were confirmed as clonally indistinguishable by pulsed-field gel electrophoresis. To the best of the authors' knowledge there are no previous reports of Burkholderia cepacia causing infection of the adult central nervous

[A case of brain abscess due to Streptococcus sanguis in association with multiple pulmonary arteriovenous fistulas].

A case is reported here of brain abscess due to Streptococcus sanguis in association with multiple pulmonary arteriovenous fistulas. A 19-year-old male who had been diagnosed in 1988 as pulmonary arteriovenous fistulas was admitted to our hospital for repeated epistaxis, headache, and vomiting. A brain abscess which was diagnosed using various roentogenologic examinations, worsened despite conservative therapy. Additional surgical drainage was performed; S. sanguis was isolated from the drainage fluid. After drainage, clinical symptoms gradually improved with no subsequent neurological deficit…

[Computerized tomography, radionuclide gamma-topography and angiography in the diagnosis of brain abscesses of rhinosinusogenic origin].

X-ray and other methods were used to examine 21 patients with brain abscesses of rhinosinusogenic origin. The diagnostic methods including computer tomography, brain gamma-topography and cerebral angiography yielded objective information about the localization, size, distribution and multiplicity of brain abscesses. They were displayed as doughnuts or rings on computer tomograms and as pies on gamma-topograms. The noninvasive methods such as computer tomography or gamma-topography proved very helpful for assessing the efficacy of surgical intervention at an early postoperative stage.

The role of neuroendoscopy in treatment of pediatric brain abscesses: case series and systematic review of the literature.

Treatment of brain abscesses in children requires a multimodal approach, consisting of neurosurgical evacuation and intravenous antibiotics. Endoscopic aspiration has been used for extensive removal of the purulent collection, but few pediatric cases have been described in literature. We report the management of endoscopically treated brain abscesses in children in our department and provide a systematic review of the literature. We retrospectively collected data on a series of children with intracerebral abscesses treated endoscopically. The literature search was performed and studies between…

Enhanced microbiological evaluation of brain abscesses by proteomics-based techniques: a prospective cohort study.

OBJECTIVE: Microbiological analysis might not be able to identify the causative pathogen in one-third of brain abscess patients. Proteomics analysis of brain abscesses can aid in microbiological diagnosis. This could help tailor the antimicrobial therapy, give possible insights into the source of the abscess, and aid in developing targeted antimicrobial agents. METHODS: This was a single-center prospective study including patients with brain abscess recruited over a 1-year period. Proteome-based analysis was used to identify the causative pathogen, and the results were compared using conventi…

From Headache to Hydrocephalus: The Hidden Pulmonary Arteriovenous Malformation Behind Multiple Brain Abscesses.

A 24-year-old man experienced high-grade fever, chills, and headaches for 2 months. From August 18 to 28, 2021, he received intraperitoneal treatment, then remained at home. One month passed before he underwent external ventricular drainage for hydrocephalus. Two days later, he presented with vomiting, altered sensorium, and a foreign body sensation. Upon examination, the patient's vitals were stable. Clubbing was noted on all digits. He was afebrile but had an abnormal central nervous system exam with a Glasgow Coma Scale score of E4V3M5. He was disoriented and nonresponsive to commands. The

Clinical features and outcome of brain abscess after introduction of CT and MRI: A meta-analysis.

OBJECTIVE: To describe the clinical features and outcome of brain abscess since introduction of computerised tomography and magnetic resonance imaging. METHODS: MEDLINE and Embase were searched using "brain abscess" or "cerebral abscess" from 1970 through 2023 and analyses restricted to study populations hospitalised after 1980. Single-variable meta-analyses were done using a random-effects model. RESULTS: A total of 21,840 patients from 209 studies were included. The mean age was 34 years (95% confidence interval [CI] 30-38) and 11,817/17,406 (66%, 95% CI 64-67) were male. The aetiologies w…

Brain Abscess Secondary to Streptococcus Intermedius Following an EGD with Biopsy in a Patient with Eosinophilic Esophagitis.

Brain abscess is a rare complication of esophagogastro- duodenoscopy (EGD) with few reported cases in the literature. In this report, we discuss a patient presenting with altered mental status, headache, and dysarthria due to brain abscess caused by S. intermedius shortly after an EGD with an esophageal biopsy showing a new diagnosis of eosinophilic esophagitis. We highlight the rare association of EGD and brain abscess, and discuss the importance of prompt diagnosis and treatment.

Clinicopathologic characteristics of Nocardia brain abscesses: Necrotic and non-necrotic foci of various stages.

Nocardia brain abscesses are rare bacterial infections associated with a high mortality rate, and their preoperative diagnosis can be difficult for various reasons including a nonspecific clinical presentation. While late-stage nocardial brain abscesses may be radiologically characteristic, early-stage lesions are nonspecific and indistinguishable from another inflammatory/infectious process and other mimics. Despite the paucity of previous histopathological descriptions, histopathological examination is critical for the identification of the pathogen, lesion stage(s), and possible coexisting …

An epidural brain abscess and thrombus in the superior sagittal sinus in a 12 year-old with sinusitis.

BACKGROUND: Upper respiratory infections can be complicated by acute bacterial sinusitis in pediatric patients, and usually resolve with antibiotic therapy (DeMuri and Wald, 2011). However, intracranial complications such as: epidural abscess, meningitis and more rarely cerebral sinus venous thrombosis (CSVT) can occur (Germiller et al., 2006). We report an unusual case of sinusitis complicated by an epidural abscess and later a CSVT in a young previously healthy patient. CASE DESCRIPTION: A 12-year-old female presented to the emergency department with a 9-day history of headaches and a 3-day

Do brain abscesses have a higher incidence of odontogenic origin than previously thought?

OBJECTIVE: The aim of this study was to answer the following clinical question: "In patients admitted to a tertiary care hospital with a diagnosis of brain abscess, how common is odontogenic etiology?" STUDY DESIGN: We designed a retrospective study of patients with brain abscesses diagnosed at the Massachusetts General Hospital between 1980 and 2017. Inclusion criteria were complete medical records outlining clinical course, relevant dental history, and radiographic and microbiologic data. RESULTS: Of 167 intracranial abscesses, 88 (52.7%) originated from a head/neck source, and 12 (13.6%) …

Acrophialophora levis brain abscess in a kidney transplant patient: A case report and review of the literature.

We report the first case of Acrophialophora levis causing cerebral phaeohyphomycosis in a solid organ transplantation recipient. A. levis is a rare cause of invasive dematiaceous fungal infection among immunocompromised persons. We describe the clinical course of a kidney transplant patient who presented with acute hemiplegia due to a brain abscess from which A. levis was isolated. We review published clinical cases attributed to Acrophialophora species infection and discuss current limitations in its identification, diagnosis and management.

Immunocompetent patient with a brain abscess caused by Nocardia beijingensis in Latin America: A case report.

RATIONALE: Nocardia species are not commonly referred as primary infectious entities but rather as opportunistic pathogens. Infectious cases of Nocardia spp. in immunocompetent individuals are rare. PATIENT CONCERNS: An immunocompetent 58-year-old patient presented with recurrent headaches. DIAGNOSIS: A brain abscess was found and surgically drained. Matrix-assisted laser desorption ionization-time-of-flight mass spectrometry and heat shock protein 65/16S-23S rRNA gene intergenic spacer genotyping from the sample revealed the etiological agent as Nocardia beijingensis. INTERVENTIONS: Merope…

Multiple brain abscesses after professional tooth cleaning: Case report and literature review.

Brain abscess (BA) is an encapsulated infection and can be the consequence of head and cranio-maxillo-facial surgery or trauma, or may be secondary to cavernous thrombophlebitis, but is most frequently due to hematogenous septic dissemination from an adjacent site of infection, such as the paranasal sinuses, middle ear or oral cavity. We report a rare and unfortunate case of multiple BA caused by dental procedures in a young man with undiagnosed patent foramen ovale (PFO). Simple routine dental procedures, such as tooth brushing and professional oral hygiene, can predispose to life-threatening…
