BACKGROUND: Worldwide there are differences in the procedure of determining brain death. An irreversible loss of all brain functions, including cerebrum, cerebellum and brainstem is mandatory for the diagnosis of brain death in Germany. On the basis of a case report some important aspects of the new recommendations of the German guidelines are discussed.
CASE REPORT: We present the case of a 41-year old patient who was admitted to our clinic due to acute subarachnoid hemorrhage (SAH). Angiography revealed an aneurysm of the posterior inferior cerebellar artery. The patient was comatose withou…

In Japan, from July 2010, an infant or a child with brain death will be legally regarded as a candidate of donor for organ transplantation under the consent of his or her family members. Official diagnostic criteria of brain death in children are currently under compilation. The causes and incidence of brain death remarkably differ among individuals belonging to different age groups. Secondary brain damages resulting from asphyxia, drowning, hypoxemia, and cardiopulmonary arrest more commonly occur in childhood than in adulthood. Child abuse or neglect is suspected to be involved in many of th…

A patient with the clinical picture of brain death resulting from brainstem hemorrhage and subsequent infarction is presented. The EEG showed activity similar to what has been described in the cerveau isolé animal preparations. Cortical evoked potentials were unobtainable from auditory or somatosensory stimulation, but of unusually high amplitude to flash stimuli. The point is made that a diagnosis of brain death cannot be made on clinical grounds alone when a patient is on life support systems, and the differences between cerebral death, brainstem death and brain death are discussed.

This article offers a philosophical foundation for the Uniform Determination of Death Act as it first examines death per se, and then examines brain death and the non-heart beating donor criteria for determining death. The author suggests that many of the debates over death can be bypassed by changing the terms of the debate: what matters is not whether death is a process or an event, but death as a state. Understanding death as a state allows us to determine death in a functional manner that is compatible with the needs of law and medicine. The second part examines objections that arise from …

BACKGROUND: It is known that the risk of death in elderly patients with moderate to severe traumatic brain injury is increased. However, the relationship between mild traumatic brain injury and death has never been established. We investigated the mortality rates of older patients with mild traumatic brain injury in Taiwan to evaluate if there is a higher risk of death compared with the general population.
METHODS: We utilized a sampled National Health Insurance claims database containing one million beneficiaries. We followed all adult beneficiaries older than 65 years from January 1, 2005 t

There is international variability in the determination of death. Death in donation after circulatory death (DCD) can be defined by the permanent cessation of brain circulation. Post-mortem interventions that restore brain perfusion should be prohibited as they invalidate the diagnosis of death. Retrieval teams should develop protocols that ensure the continued absence of brain perfusion during DCD organ recovery. In situ normothermic regional perfusion (NRP) or restarting the heart in the donor's body may interrupt the permanent cessation of brain perfusion because, theoretically, collateral

Brain-dead donor (DBD) is the basis for most organ transplants and has been the exclusive method of choice for heart transplantation (HTx). However, the severe shortage of donors has been the common problem worldwide, especially in Japan. Meanwhile, the number of donors after cardiac (circulatory) death (DCD) has increased rapidly in the last 10 years, especially in Europe, thus expanding the donor pool. This strategy has been introduced in HTx since 2014, starting in Australia and the United Kingdom, with acceptable results. In most DCD, organs are removed under withdrawal of life support (WL

OBJECTIVES: Infant organ donation is rare but when it does occur, infants are more commonly eligible for donation after circulatory death (DCD) than donation after neurological death. The aims of this scoping review were to identify: 1) existing research, ethical, and policy information about infant organ DCD and 2) knowledge gaps to guide future research.
DATA SOURCES: A literature search of MEDLINE, Embase, CINAHL, Scopus, and the Web of Science up to 2024. No other date or search limits were applied. A gray literature search for policy and clinical guideline documents was also conducted.

Whole-organism performance, including locomotor activity, is an important fitness trait in many animals. Locomotor activity is often classified into sprint speed and locomotor endurance and differences in sprint speed and locomotor endurance affect on other traits such as life-history traits. Previous studies found that locomotor endurance, sprint speed and brain dopamine (DA) levels are correlated with artificial selection for death feigning (an anti-predator behaviour that we refer to as 'death-feigning syndrome') in some insect species. Thus, if the syndrome has a genetic basis, death feign

The Bexar County Medical Examiner's Office in San Antonio, Texas, has encountered 3 cases within a 15-month period involving decedents who were pronounced dead by brain death or cardiac death examination and who had elevated, if not toxic concentrations of pentobarbital present at the time of examination. The elevated levels of pentobarbital were discovered during an autopsy examination performed for medicolegal reasons. The diagnosis of brain death and the implications of pentobarbital intoxication during a brain death examination are discussed.

In a retrospective study at a large children's hospital, we identified 92 children who had received barbiturates that were simultaneously discontinued at the time they were being evaluated for brain death in the presence of electrocerebral silence and clinical brain death by physical exam. Of these 92 children, 67 had barbiturate levels that were monitored from initial therapeutic or subtherapeutic levels. Repeat EEGs were obtained in 76 patients, and in all electrocerebral silence and clinical brain death (by exam) persisted despite the lower barbiturate levels. The study suggests that therap…

The absence of standardization in the donation after circulatory death (DCD) organ donation process is well known across the transplant comm … a, since 2023 to develop a hospital-based donor care unit to centralize both DCD and donation after brain death organ recoveries from surrounding areas. By centralizing the DCD donor process, one can ensure that DCD protocols are standardized and aligned with best medical practices. Herein, we describe key components of our standardized DCD model, including p …

BACKGROUND: Marginal donors exposed to the full array of effects induced by brain death are characterized by low success rates after transplantation. This study examined whether organs from marginal brain dead animals show any change in organ function or tissue activation making them eventually more susceptible for additional damage during preservation and transplantation.
METHODS: To study this hypothesis we first focused on effects of brain death on donor organ quality by using a brain death model in the rat. After induction of brain death, Wistar rats were ventilated for 1 and 6 hr and the…

Some patients who have been diagnosed as "dead by neurologic criteria" continue to exhibit certain brain functions, most commonly, neuroendocrine functions. In this chapter, we review the pathophysiology of brain death that can lead either to neuroendocrine failure or to preserved neuroendocrine functioning. We review the evidence on continued hypothalamic functioning in patients who have been declared "brain dead," examine potential mechanisms that would explain these findings, and discuss how these findings create additional confounds for brain death testing. We conclude by reviewing the evi…

BACKGROUND: Kidney donation after circulatory death is a valid alternative to donation after brain death. However, ischemia-reperfusion injury can damage the graft and make it non-viable. The Karpinski score, currently used to evaluate kidney quality, does not account for this type of damage and is, therefore, insufficient for predicting expected functional recovery.
METHODS: Sixty-seven kidney biopsies from donors after circulatory death and seventy-two kidney biopsies from donors after brain death were analyzed histologically. A Karpinski score was assigned to all samples. Additionally, bi…

OBJECTIVES: Pancreas transplant provides definitive treatment for uncontrolled insulin-dependent diabetes, yet use of donation after circulatory death pancreases remains limited due to concerns about higher complication rates and reduced graft survival versus donation after brain death. We (1) assessed circulatory death simultaneous pancreas-kidney transplants from 2015-2019 and 2020-2024, (2) reviewed donor/recipient characteristics, and (3) compared outcomes of circulatory versus brain death donation in simultaneous pancreas-kidney transplants.
