This study was conducted to detect human metapneumovirus (hMPV) and respiratory syncytial virus (RSV) in young children hospitalized with acute bronchiolitis, using reverse transcriptase polymerase chain reaction (RT-PCR). Nasopharyngeal secretions were collected from 170 children between 1 and 24 months of age admitted to two tertiary hospitals in northeastern Thailand, between 2002 and 2004. Acute bronchiolitis was defined as the first episode of wheezing associated with tachypnea, increased respiratory effort and an upper respiratory tract infection. Two-thirds (115/170) were positive for v…

BACKGROUND: Because of the shortage of donor lungs, liberalization of donor selection criteria in terms of age, gas exchange, and smoking history has been proposed.
METHODS: We evaluated a single-institution population of lung transplant recipients (n = 98) for donor-recipient gender matching. We measured overall survival, time to acute allograft rejection, and time to development of obliterative bronchiolitis (OB).
RESULTS: We found significant improvement in overall survival for gender-mismatched donor and recipient pairs (p = 0.078) and a significantly shorter OB-free period for male dono…

This review examines the relationship between severe pulmonary disease caused by respiratory syncytial virus (RSV) infection in infancy and later development of asthma or reactive airway disease (RAD). RSV infection accounts for 70% or greater of all cases of infantile bronchiolitis and has been linked to subsequent asthma or RAD, either directly or through a shared common predisposition. Several studies suggest that RSV bronchiolitis is an important factor in the development of asthma and possibly atopy, although the association is lost by the age of 13 years. The mechanism is as yet unclear,…

The objective of this study was to identify wheezing recurrences and related risk factors in two groups of infants with bronchiolitis: respiratory syncytial virus (RSV)+ and RSV- as determined by RSV enzyme immunoassay. A 1-year prospective cohort study was conducted with infants younger than 2 years old. Follow-up was made monthly, by a clinical visit and/or by telephone, checking the number of wheezing episodes per month and possible related risk factors. There were 96 subjects enrolled, of whom 77 reached complete follow-up: 36 were RSV+ (46.8%), and 41 were RSV- (53.2%). In the RSV+ group,…

We report a patient who developed bronchiolitis obliterans organizing pneumonia (BOOP) after syngeneic BMT for ALL. The patient complained of persistent low-grade fever and non-productive cough after engraftment. Chest CT scan showed patchy infiltration bilaterally in the lower lung fields. Antibiotics were ineffective. Cultures, serological studies and polymerase chain reaction detected no infectious pathogens. We finally made a diagnosis of BOOP by thoracoscopical lung biopsy. The lung lesion disappeared in a month with corticosteroid therapy. While BOOP following allogeneic BMT has been rep…

Bronchiolitis obliterans organizing pneumonia (BOOP) is defined by endoluminal obstruction of distal airspaces by a fibrous granulation tissue consisting of inflammatory cells, fibroblasts, and connective tissue. The morphogenesis of the process is the following: diffuse alveolar injury leading to basement membrane denudation and intra-alveolar exudate of fibrinogen, immunoglobulins, coagulation factors, fibronectin; intra-alveolar migration of interstitial fibroblasts through gaps of the epithelial basement membrane; intra-alveolar secretion of loose connective tissue (codistribution of colla…

OBJECTIVE: To clarify the predictive role of club cell 10-kDa protein (CC10) and cysteinyl leukotrienes (CysLTs) for subsequent wheezing after RSV infection.
METHODS: From October 1, 2022, to October 31, 2023, children diagnosed with RSV-induced bronchiolitis hospitalized in the pediatric ward were included. Nasopharyngeal aspirates (NPAs) were obtained on the day of admission. NPAs from hospitalized children without respiratory infections served as the control group. We analyzed the correlation between CC10 and CysLTs levels in NPAs and disease severity. Follow-up assessments were conducted …

BACKGROUND: Bronchiolitis remains the commonest acute lower respiratory tract infection in younger children globally. Bronchiolitis is often misdiagnosed as World Health Organization (WHO)-classified pneumonia due to closely related clinical manifestations. Our primary objective was to assess the prevalence of viral bronchiolitis among WHO-classified pneumonia, and the secondary objective was to evaluate their management.
METHODS: This was a prospective observational study. Infants (aged <2 years) suffering from WHO-classified pneumonia attending the country's largest academic paediatric hosp…

BACKGROUND: The simultaneous identification of multiple respiratory viruses is common in infants hospitalized with respiratory tract infections. Respiratory syncytial virus (RSV) is one of the main pathogens in bronchiolitis, although codetection of rhinovirus, influenza and other respiratory viruses may occur in about one-third of cases. The relevance of viral codetection on disease severity is still controversial. This multicenter cohort study aimed to assess the clinical outcomes of infants under 24 months hospitalized with bronchiolitis, comparing those testing positive for RSV alone, RSV …

Bronchiolitis is one of the leading causes of hospitalization among infants. Established risk factors include young age, prematurity, and exposure to tobacco smoke. Emerging evidence suggests that air pollution may also contribute to the burden of respiratory diseases. However, its link with bronchiolitis hospitalizations remains debated. To address this, we conducted a systematic review and meta-analysis to assess whether exposure to air pollutants is associated with an increased risk of hospitalization for bronchiolitis in infants. A systematic review and meta-analysis were conducted followi…

BACKGROUND: Neurally Adjusted Ventilatory Assist (NAVA) is a ventilator mode providing improved patient-ventilator synchrony compared to flow- or pressure-triggering. We examined the effect of various NAVA, pressure support (PSV), and PEEP combinations in infants with acute viral bronchiolitis.
METHODS: This was a mechanistic study, nested within a randomized controlled feasibility trial. A set of 16 ventilatory combinations (each of 10-15 min), including PEEP of 1, 5, and 10 cm H2O; PSV of 20 cm H2O; and NAVA support ranging from 0.5 to 2.0 cm H2O/μV, were administered on each of 2 consecuti…
