Intracavitary right coronary artery.

Congenital coronary artery anomalies have been reported in fewer than 1.3% of patients undergoing coronary angiography. Most commonly, they … ocalization as well as bypass grafting. In addition, it is of upmost importance that interventional cardiologists and electrophysiologists are aware of this anomaly because inadvertent disruption of an intracavitary artery can occur at the time of invasive coronary angiography, pacemaker implantation, right heart catheterization or electrophysiology procedure. …

[Anesthetic management of a pregnant patient with congenitally corrected transposition of the great arteries for emergency cesarean delivery].

A 30-year-old woman at 35 weeks' gestation with congenitally corrected transposition of the great arteries (CCTGA), whose associated anomalies were VSD, ASD and pulmonary stenosis, underwent emergency cesarean delivery under general anesthesia. She had hypoxemia due to right-to-left shunt. We discussed with obstetricians and pediatric cardiologists about her at 25 weeks' gestation. Discussed points included: 1) delivery after 30 weeks' gestation, 2) her cardiac function and anomalies, 3) cesarean delivery under general anesthesia in case of emergency, 4) management of a sleeping baby after ces…

Adult congenital heart disease.

One million people over the age of 20 suffer from congenital heart disease in the United States. These adult patients can slip through the cracks of our medical system; many are too old to be cared for in most pediatric institutions by pediatric cardiologists and, unfortunately, most adult cardiologists are not trained in congenital heart disease. Therefore, it is important to identify the common lesions in adult congenital heart disease and how they should be managed. Acyanotic congenital heart disease in the adult population primarily involves left-to-right shunts, such as atrial septal defe…

Pharmacologic management of heart failure among older adults by office-based physicians in the United States.

BACKGROUND: Despite the recent availability of new classes of heart failure medications, little is known about national patterns in the actual physician utilization of these drugs. METHODS: In the National Ambulatory Medical Care Survey, 2912 US physicians reported on 16,968 office visits in 1991-1992 with patients aged > or = 65 years. National estimates were obtained from weighted results that accounted for the complex sampling design. RESULTS: An estimated 8.3 million (2.6%) office visits with older adults involved heart failure. This included 9.3% of visits to cardiologists, 4.3% to inte…

Acute Coronary Stent Thrombosis in Modern Era: Etiology, Treatment, and Prognosis.

Acute stent thrombosis (AST) is a rare but life-threatening complication of coronary artery stenting. AST remains a challenging task for cardiologists, despite the application of modern procedural techniques and dual-antiplatelet therapy strategies as well as improved understanding of the underlying pathophysiology. This review focuses on the prevalence, risk factors, prognosis, multiple potential underlying pathogenesis, knowledge gaps, and recommends diagnosis and individualized management strategies of AST.

What to do when everything has failed: alternative treatment strategies for failure revascularisations.

Percutaneous coronary intervention and bypass grafting are effective for relieving symptoms and improving outcome in patients with coronary artery disease. Despite advances in medical treatment and revascularisation procedures, some patients with symptomatic ischaemic cardiomyopathy are not candidates for revascularisation. As life expectancy increases, interventional cardiologists and cardiac surgeons face patients with more complex disease, such as those with diffuse coronary disease that cannot be completely revascularised.

Panic disorder: its association with mitral valve prolapse and appropriate dental management.

Panic disorder affects 0.75% of all adult Americans. Up to 50% of these persons may have concomitant mitral valve prolapse. Modifications in dental therapy for this highly anxious group of patients require that closely coordinated care be given by the dentist, the psychiatrist, and the cardiologist.

Disease evolution and risk-based disease trajectories in congestive heart failure patients.

Congestive Heart Failure (CHF) is among the most prevalent chronic diseases worldwide, and is commonly associated with comorbidities and complex health conditions. Consequently, CHF patients are typically hospitalized frequently, and are at a high risk of premature death. Early detection of an envisaged patient disease trajectory is crucial for precision medicine. However, despite the abundance of patient-level data, cardiologists currently struggle to identify disease trajectories and track the evolution patterns of the disease over time, especially in small groups of patients with specific d…

Congenital cardiac malformations in Iceland from 1990 through 1999.

INTRODUCTION AND BACKGROUND: About 1% of live-born children have congenital malformations of the heart. The aim of our study was to investigate the incidence of such defects in children born in Iceland during a period of 10 years, extending from 1990 through 1999. MATERIALS AND METHODS: Information about the patients was obtained from medical records from two hospitals that cover the whole country, a private clinic of pediatric cardiologists, an echocardiography database, autopsy reports, and death certificates. We investigated the distribution of specific malformations, the age at diagnosis,…

[Severe cardiac arrhythmias].

Cardiac arrhythmias are a common cause of admission in the emergency department. Among these, atrio-ventricular conductive disorders and malignant ventricular arrhythmias are among the most severe, requiring prompt and appropriate management to ensure the best prognosis. Knowledge of the pathophysiology and etiology causing these arrhythmias is mandatory in order to understand its management, acute and chronic, and to facilitate the dialogue between emergency physicians and cardiologists.

Great need, scarce resources, and choice: reflections on ethical issues following a medical mission.

Medical missions to provide cardiac surgical procedures in developing and technologically less advanced countries is a great challenge. It is also immensely gratifying, personally and professionally. Such missions typically present significant ethical dilemmas, especially making difficult choices, given limited time and resources, and the inability to help all children in need of cardiac surgery. We describe some of these issues from our perspective as visiting cardiologists.

Venous hum causing tinnitus: case report and review of the literature.

Hearing one's own heart murmur has been reported in adults, but has not been reported in the pediatric literature. This study reports the case of a young child who clearly heard her own venous hum, causing her to complain of pulsatile tinnitus. This entity should be included in the differential diagnosis of pulsatile tinnitus and by doing so pediatricians and cardiologists may avoid ordering unnecessary diagnostic procedures.

IRIDIA-AF, a large paroxysmal atrial fibrillation long-term electrocardiogram monitoring database.

Atrial fibrillation (AF) is the most common sustained heart arrhythmia in adults. Holter monitoring, a long-term 2-lead electrocardiogram (ECG), is a key tool available to cardiologists for AF diagnosis. Machine learning (ML) and deep learning (DL) models have shown great capacity to automatically detect AF in ECG and their use as medical decision support tool is growing. Training these models rely on a few open and annotated databases. We present a new Holter monitoring database from patients with paroxysmal AF with 167 records from 152 patients, acquired from an outpatient cardiology clinic …

A rare case of Hodkin's lymphoma of the mediastinum imitating retrosternal goiter--retrospective analysis of the diagnostic process.

Primary extranodal sites of development of lymphoid neoplasms are rare and concern about 5% of patients with Hodgkin's lymphoma. Extranodal development is more common in non Hodgkin's lymphoma and may reach 33%. A 27-year-old woman was diagnosed by a cardiologist for a short breath. On the physical examination no other abnormalities were observed. Echocardiography, performed by cardiologist, revealed a large tumor, overlaying the right ventricle and compressing the pulmonary trunk. Chest X-ray, ultrasound and CT-scan confirmed diagnosis. In fine needle aspiration clear, lucid fluid was obtaine

The role of telecardiology in supporting the decision-making process of general practitioners during the management of patients with suspected cardiac events.

We studied the role of telecardiology in reducing unnecessary hospital admissions of patients with suspected life-threatening cardiac events (CEs), evaluated by general practitioners (GPs). Over one month, 456 consecutive patients (mean age 65 years, SD 19) complaining of typical (10%) or atypical (42%) chest pain, palpitations (19%), dyspnoea (19%) or syncope (10%) were enrolled. Before teleconsultation, the GPs recorded their own opinion (based on clinical evaluation only) about the presence of a CE. Following transmission of the electrocardiogram (ECG), this opinion was compared with that o

Metastatic mucinous adenocarcinoma of the heart.

A case of metastatic mucinous adenocarcinoma to the heart is described. The patient presented with neurological symptoms consistent with an embolic cerebrovascular accident. Evaluation by the referring cardiologist at that time showed what appeared to be a left atrial myxoma. In a review of the English language medical literature, no other case of this nature was found.
