Psychological correlates of pain catastrophizing and disability in patients with Achilles tendinopathy: The roles of resilience and psychological inflexibility.

OBJECTIVE: This study aimed to clarify the contribution of psychological factors to pain adjustment in patients with Achilles tendinopathy. It specifically investigated the associations of psychological inflexibility, distress, and resilience with pain catastrophizing, and their indirect associations between pain intensity and pain disability. METHODS: A total of 120 participants (65% female; mean age = 49.17 years, SD = 12.06) completed validated measures of visual analog scales for pain level, psychological distress, psychological inflexibility, resilience, pain disability index, and pain c

When Hyperglycemia Turns Black: Acute Necrotizing Esophagitis in a Catastrophic Metabolic Crisis: A Case Report.

BACKGROUND: Acute necrotizing esophagitis (ANE), also known as "black esophagus," is a rare but life-threatening condition typically occurring in critically ill patients with profound systemic disturbances. Extreme hyperglycemic crises represent an underrecognized precipitating factor, capable of inducing severe metabolic, inflammatory, and microvascular injury. CASE PRESENTATION: We report the case of a 54-year-old male admitted with altered mental status and severe dehydration, in whom initial laboratory evaluation revealed extreme hyperglycemia (serum glucose ~1000 mg/dL), metabolic acidos

Kidney transplantation in catastrophic antiphospholipid syndrome survivors: Results from a nationwide series of 12 cases.

Catastrophic antiphospholipid syndrome (CAPS) is a rare, life-threatening subtype of APS that may progress to end-stage kidney disease. CAPS survivors may have limited access to kidney transplantation (KT). We conducted a retrospective nationwide study including all French KTs performed in recipients with a history of CAPS between 2008 and 2024. Twelve KTs were performed in 10 patients (median age, 45 [interquartile range {IQR}, 41-56] years), all from deceased donors. Anticoagulation therapy was resumed 4 (IQR, 0-5) hours after KT and reached therapeutic levels at 12 (IQR, 6-28) hours. Preven

Human-induced climate change amplification on storm dynamics in Valencia's 2024 catastrophic flash flood.

Global warming alters the hydrological cycle, increasing heavy rainfall events worldwide. In October 2024, Valencia (Spain) experienced rainfall accumulations in a few hours surpassing annual averages (771.8 mm in 16 h in the official weather station at Turís) and breaking the record for one hour rainfall accumulation in Spain (184.6 mm), resulting in 230 fatalities. Here, we present a physical-based attribution study employing a km-scale pseudo-global warming storyline approach to assess the contribution of anthropogenic climate change. We show that present-day conditions led to a 20% °C⁻¹ in

Concurrent uterine rupture and refractory uterine atony leading to catastrophic postpartum hemorrhage: a case report.

INTRODUCTION: Postpartum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality worldwide, particularly in low- and middle-income countries. Uterine atony is the most common etiology, while uterine rupture is less frequent but catastrophic. The coexistence of both conditions is rare and poses significant management challenges. CASE PRESENTATION: We report the case of a multiparous woman who developed massive PPH due to a combination of uterine rupture and refractory uterine atony. Despite maximal uterotonic therapy, uterine massage, and fluid resuscitation, bleeding pers

Catastrophe mechanism and early warning indicators of seepage erosion-induced water inrush in karst cavities.

Seepage-erosion-induced water inrush in karst cavities is a typical form of water-inrush disaster in karst tunnels. It is governed not only by the spatial distribution of karst cavities and hydraulic recharge conditions, but also by the particle-size gradation and composition of the cavity fill. During seepage erosion, fill particles are progressively transported by flowing water, which may trigger a sudden water-inrush catastrophe. To reveal the catastrophe mechanism and establish early-warning indicators, this study employs the Smoothed Particle Hydrodynamics (SPH) method to simulate the evo

Targeting the HSPA8-CMA-ATP6V1A Axis Triggers Lysosomal Hyperacidification and Catastrophic Vacuolation in Prostate Cancer.

Prostate cancer (PCa) ranks among the most common and deadly malignancies worldwide. The clinical treatment of advanced prostate cancer is particularly challenging due to acquired drug resistance. Autophagy and lysosome-related pathways are key drivers of this resistance. Targeting the lysosome represents a potential therapeutic strategy for PCa. In this study, we identified Heat Shock Protein Family A Member 8 (HSPA8) as a critical functional node of Aloperine (ALO). ALO suppresses autophagic flux, disrupts lysosomal homeostasis, and induces lysosomal vacuolation in cancer cells by inhibiting

Pain catastrophizing and postoperative satisfaction in cervical disc herniation: a 6-month prospective cohort study.

BACKGROUND: Despite remarkable technical advancements in cervical minimally invasive interventional therapy, postoperative satisfaction remains suboptimal in a subset of patients with Cervical Disc Herniation (CDH). Pain Catastrophizing (PC), a negative psychological state, has been established as a robust predictor of poor outcomes across various pain-related conditions. However, its specific impact within the context of cervical minimally invasive interventions remains unclear. OBJECTIVE: This study aimed to prospectively investigate the influence of preoperative pain catastrophizing levels

Interferon α-induced HERC6 provokes podocyte dysfunction via mitotic catastrophe in lupus nephritis.

BACKGROUND: HECT and RLD Domain Containing E3 Ubiquitin Protein Ligase Family Member 6 (HERC6) is an IFN-stimulated gene (ISG) encoding an E3 ubiquitin ligase that plays significant roles in antiviral innate immunity. However, current research on HERC6 has primarily focused on the liver and immune cells. Its expression, regulation, and function in the kidney, particularly in podocytes remain largely unexplored. The aim of this study is to investigate the pathogenic role and mechanism of the E3 ubiquitin ligase HERC6 in podocyte injury of lupus nephritis (LN). METHODS: Key genes were identified

Catastrophic Antiphospholipid Syndrome: A Rare but Life-Threatening Thrombotic Storm-A Case Report and Literature Review.

Catastrophic antiphospholipid syndrome (CAPS) is a rare, life-threatening variant of antiphospholipid syndrome (APS) characterized by rapid, widespread thrombosis leading to multiorgan failure. Affecting less than 1% of APS patients, CAPS is associated with a high mortality rate of 30%-50%, necessitating prompt diagnosis and aggressive treatment. The mainstay of management includes anticoagulation, high-dose glucocorticoids, and plasma exchange or intravenous immunoglobulins, with biologic therapies such as rituximab and eculizumab reserved for refractory cases. We report a case of a 60-year-o

Delayed Catastrophic Uterine Rupture in a Subsequent Pregnancy After Expectant Management of Placenta Accreta Spectrum.

Placenta accreta spectrum (PAS) is a serious obstetric disorder involving abnormal placental attachment to the uterine wall and is associated with substantial maternal risk. While cesarean hysterectomy remains the standard treatment, conservative management approaches are emerging as alternatives. Minimal data is currently available regarding the long-term complications of conservative management. We present a case of a patient with a history of conservatively managed PAS who presented 14 years later during a subsequent pregnancy with fundal uterine rupture in the second trimester, highlightin

Catastrophic antiphospholipid syndrome with eosinophilia mimicking hypereosinophilic syndromes with disseminated intravascular coagulation: a case report.

BACKGROUND: Catastrophic antiphospholipid syndrome is a rare variant of antiphospholipid syndrome where the presence of antiphospholipid antibodies and systemic inflammation leads to the rapid onset of multifocal thrombosis. Eosinophilia is known to accompany conditions such as allergy, parasite infection, malignancy, or autoimmune diseases; however, catastrophic antiphospholipid syndrome with eosinophilia has not been reported and its clinical influence remains unknown. We describe the first case of probable catastrophic antiphospholipid syndrome with eosinophilia, which mimicked eosinophil-a

Association of postoperative pain and function with a preoperative pain catastrophizing score in temporomandibular joint prosthetic reconstruction.

The Pain Catastrophizing Scale (PCS) assesses cognitive responses to pain through rumination, magnification, and helplessness. Though widely used, its role in evaluating patients undergoing temporomandibular joint (TMJ) replacement surgery has not been studied. This retrospective cohort study analyzed 98 patients who underwent total TMJ prosthetic reconstruction (TMJR) for treatment of severe osteoarthritis between 2018 and 2023. Preoperative PCS, Graded Chronic Pain Scale (GCPS), and Jaw Functional Limitation Scale-8 (JFLS-8) were recorded and compared with postoperative GCPS and JFLS-8 score

Rothia mucilaginosa Prosthetic Valve Endocarditis Presenting as a Thrombotic Microangiopathy Mimic With Catastrophic Intracranial Hemorrhage.

Rothia mucilaginosa is an uncommon but increasingly recognized opportunistic pathogen capable of causing invasive infection, particularly in patients with prosthetic material and immunocompromised states. Rarely, severe systemic infections may present with thrombotic microangiopathy-like syndromes mimicking thrombotic thrombocytopenic purpura (TTP), creating significant diagnostic uncertainty. We present a case of fulminant Rothia mucilaginosa prosthetic valve infective endocarditis initially presenting as a TTP/disseminated intravascular coagulation (DIC) mimic in a 60-year-old man with sever

Adverse Childhood Experiences and Urogenital Pain: Examining the Mediating Effects of Negative Affect and Pain Catastrophizing.

UNLABELLED: BACKGROUND : Adverse childhood experiences (ACE) are associated with later psychological and somatic problems, including the emotional sequelae of adverse events (depression, anxiety, and catastrophizing) and outcomes such as chronic pain intensity and pain-related functioning. ACE have rarely been studied in women with pelvic, urogenital, and/or bladder pain (PUBP). AIMS: Thus, we examined how childhood adversities are related to psychological and pain-related outcomes in adult women. We also tested whether negative affect and pain catastrophizing mediated the relationship betwee
