BACKGROUND: Jarcho-Levin syndrome (JLS) is a rare congenital disorder characterized by different clinical and radiologic findings. The disease was first reported by Jarcho and Levin in 1938, and it was described as the presence of various malformations or abnormal fusion in the thoracic vertebrae and ribs, short trunk, and respiratory distress.
CASE DESCRIPTION: In our case, fusion at the thoracic and cervical vertebrae, butterfly vertebrae, and a crablike-shaped thorax was present. The patient had a short trunk, short stature, and long extremities. Moreover, he had a syndromic face and restr

BACKGROUND AND PURPOSE: Determining the source of a patient's pain in the upper thoracic region can be difficult. Costovertebral (CV) and costotransverse (CT) joint hypomobility and active trigger points (TrPs) are possible sources of upper thoracic pain. This case report describes the clinical decision-making process for a patient with posterior upper thoracic pain.
CASE DESCRIPTION: The patient had a 4-month history of pain; limited cervical, trunk, and shoulder active range of motion; limited and painful mobility of the right CV/CT joints of ribs 3 through 6; and periscapular TrPs. Interve

Subclavian artery injuries represent an uncommon complication of blunt chest trauma, this structure being protected by subclavius muscle, the clavicle, the first rib, and the deep cervical fascia as well as the costo-coracoid ligament, a clavi-coraco-axillary fascia portion. Subclavian artery injury appears early after trauma, and arterial rupture may cause life-treatening haemorrages, pseudo-aneurysm formation and compression of brachial plexus. These clinical eveniences must be carefully worked out by accurate physical examination of the upper limb: skin color, temperature, sensation as well

The Natural History Collections, Bergen Museum, The University of Bergen, Norway, houses a large collection of Baleen whale skeletons within which one mature Fin (Balaenoptera physalus) and one juvenile Humpback (Megaptera novaeangliae) whale skeleton exhibit notable osteological pathologies that are reported in this brief communication. The Humpback skeleton has a large lesion on the maxilla characterised by loss of bone and remodelling. The lesion would appear to be a healing injury caused by trauma to the area of the gums and adjacent soft tissue. This specimen also exhibits numerous locali

From October 1996 to August 2001, 721 patients with vertebral fractures were admitted to our unit. Ten patients suffered from vertebral fractures and concomitant sternal fractures. The clinical notes and plain film radiographs of these patients were studied. The average age of the patients was 37 (20-69) years. Nine had been involved in road traffic accidents. Three patients had fractures of the cervical spine, six of the upper thoracic spine (T1-T6) and one had a lumbar spine fracture. The extra-thoracic fracture group included two patients with neurological compromise and two patients who we

OBJECTIVE: To determine factors of outcome following surgical intervention for neurologic thoracic outlet syndrome (NTOS).
METHODS: In a retrospective study of patients surgically treated for NTOS, outcome was evaluated by postoperative symptoms and the ability of patients to return to work.
RESULTS: Good, fair, and poor results were obtained in 26 (48%), 21 (39%), and 7 (13%) patients, respectively. The best predictor of a good outcome was occupation. Nonlaborers were more likely to have good outcome (21 of 32, 66%) when compared with laborers (5 of 22, 23%; P = 0.0025). Only 6 of 20 (30%)

The major clinical stigmata of basal cell naevus syndrome (BCNS) appear in adolescence and adult life but some occult skeletal abnormalities are congenital. BCNS is dominantly inherited and it would be useful to identify, as early in life as possible, which of the offspring of patients with BCNS are at risk of developing the syndrome. Radiographs of the neck and chest of 80 patients with BCNS diagnosed confidently on clinical criteria have been examined for abnormalities which were considered to be congenital skeletal anomalies. Congenital abnormalities of the cervical and thoracic spine, main

Primitive-streak and early-organogenesis-stage mouse embryos were treated with Mitomycin C (MMC) by intraperitoneal injection of pregnant females. Skeletal preparations of newborn pups were made and the axial skeleton examined. The treated animals showed a high incidence of:- (1) changed vertebral numbers, (2) malformation of the vertebral column, (3) changed rib numbers and (4) rib abnormalities. These skeletal disturbances tend to be located more posteriorly with later MMC treatment. There is regional variation in the susceptibility of vertebrae to abnormal development. Extra elements may oc

The lack of an objective method for diagnosing thoracic outlet syndrome and monitoring the results of treatment has prevented a clear understanding of its natural history and the indications for surgery. Although one can detect vascular compression, the majority of patients have symptoms that are primarily neurogenic; and neither electromyograms nor studies of nerve conduction velocity has proved consistently helpful in their evaluation. We have studied evoked responses in 22 patients and believe that this technique allows selection of patients with significant neural compression for operative

Acute onset deafness with vestibular failure in the cervical syndrome could be due to embolism of the labyrinthine artery. Probably the symptomatology of the cervical syndrome developes independantly. Two patients with sudden deafness and vestibular failure are reported. Embolism of the labyrinthine artery in at least one of these patients appears to have occurred. The prognosis in respect of recovery of function is unfavourable.

The 22q11.2 Deletion Syndrome (22q11.2DS) is the most common microdeletion syndrome with an estimated prevalence of 1:4,000 live births. 22q11.2DS is known to have wide phenotypic variability, including orthopaedic manifestations. The purpose of this systematic review is to increase the awareness of orthopaedic manifestations associated with 22q11.2DS. This systematic review was performed according to the PRISMA Guidelines. Original epidemiological studies on the prevalence of orthopaedic manifestations within 22q11.2DS were systematically searched for in PubMed and EMBASE. The included articl

STUDY DESIGN: Case report.
OBJECTIVE: We report here on an 18-month-old boy with brachytelephalangic chondrodysplasia punctata (BCDP), whose atlantoaxial instability was successfully managed with occipitocervical instrumented fusion (OCF) using screw and rod instrumentations.
SUMMARY OF BACKGROUND DATA: Recently, there have been a number of reports on BCDP with early onset of cervical myelopathy. Surgical OCF is a vital intervention to salvage affected individuals from the life-threatening morbidity. Despite recent advancement of instrumentation techniques, however, rigid OCF is technically

BACKGROUND: Thoracic outlet syndrome (TOS) is one of the most controversial diagnoses in clinical medicine. Despite many reports of operative and non-operative interventions, rigorous scientific investigation of this syndrome leading to evidence-based management is lacking. This is the first update of a review first published in 2010.
OBJECTIVES: To evaluate the beneficial and adverse effects of the available operative and non-operative interventions for the treatment of TOS a minimum of six months after the intervention.
SEARCH METHODS: On 23 June 2014 we searched the Cochrane Neuromuscular

We have attempted throughout this review to identify the issues surrounding thoracic outlet syndrome as well as to highlight their origins. It should be clear that many aspects of TOS remain controversial from the definition of the entity through pathogenesis, diagnosis, and treatment. The conflicts surrounding TOS are underlined most poignantly in the many letters to the editor of the New England Journal of Medicine in response to Urschel's 1972 publication. It is incumbent upon those of us who treat patients with TOS to dispel the ignorance surrounding this syndrome with astute, accurate, an

Using various modalities, we analyzed 110 patients with thoracic outlet compression syndrome who had extensive evaluation. Nerve conduction velocities and Directional Doppler studies are the most useful adjuncts in diagnosis. Diagnosis and selection of the proper treatment of these patients will be outlined.

BACKGROUND: Severe scapular instability can be a considerable problem for people with high-level cervical spinal cord injury. Scapular instability reduces the effectiveness of the already weakened shoulder flexors and abductors, thereby limiting hand-to-mouth and hand-to-head activities. The winged scapula may cause inferior pole skin breakdown, as well as neck and shoulder pain.
OBJECTIVE: To report the efficacy of a fusionless scapular stabilization procedure as a means to enhance function in a consecutive group of patients with high-level cervical spinal cord injury.
METHODS: Four people
