Characteristics Associated With High-Impact Pain in People With Temporomandibular Disorder: A Cross-Sectional Study.

High-impact (disabling) pain diminishes the quality of life and increases health care costs. The purpose of this study was to identify the variables that distinguish between high- and low-impact pain among individuals with painful temporomandibular disorder (TMD). Community-dwelling adults (N = 846) with chronic TMD completed standardized questionnaires that assessed the following: 1) sociodemographic characteristics, 2) psychological distress, 3) clinical pain, and 4) experimental pain. We used high-impact pain, classified using the Graded Chronic Pain Scale, as the dependent variable in logi

Healthcare team trust, care coordination, and care access among cancer survivors with multiple chronic conditions: a mixed method analysis.

PURPOSE: Cancer survivors with multiple chronic conditions (MCC) are a growing population with complex health needs, yet little is known about their care experiences and healthcare team trust. METHODS: This mixed methods study used a convenience sample of 441 adult cancer survivors recruited through online survey platforms. Participants completed surveys assessing care coordination, care access, healthcare team trust, and whether their care team included a social worker, navigator, or care coordinator during the 12 months after diagnosis. Twelve survivors also participated in semi-structured

Cost-effectiveness of multidisciplinary care for patients with chronic kidney disease in Japan.

BACKGROUND: Multidisciplinary care (MDC), where a team of various health professionals provides guidance to patients, is an effective approach for chronic kidney disease (CKD) and has been reimbursed by Japanese public healthcare insurance in 2024. To sustainably achieve medical benefits with limited resources, we examined the cost-effectiveness of MDC in Japan. METHODS: A Markov model from a healthcare system perspective was used to evaluate the cost-effectiveness of adding MDC to the standard treatment for CKD in Japan over lifetime. Each cohort with an initial CKD stage (G3a, G3b, and G4)

Primary care clinician adherence to guidelines for the management of chronic musculoskeletal pain: results from the study of the effectiveness of a collaborative approach to pain.

OBJECTIVE: We assessed primary care clinician-provided guideline-concordant care as documented in patients' medical records, predictors of documented guideline-concordant care, and its association with pain-related functioning. Patients were participants in a randomized trial of collaborative care for chronic musculoskeletal pain. The intervention featured patient and primary care clinician education, symptom monitoring and feedback to clinicians by the intervention team. METHODS: To assess concordance with the evidence-based treatment guidelines upon which our intervention was based, we deve

Musculoskeletal Health: Addressing the Leading Causes of Disability.

Musculoskeletal conditions are not only the most common group of chronic health problems in our country, but they have a profound impact on critical domains of health including pain, disability, work participation, mental health, and potentially mortality. There are challenges to the treatment of common musculoskeletal conditions, including the current epidemic of opioid misuse and abuse. However, there are also evidence-based therapies that can be more effectively incorporated into care models for these conditions. This issue of the NCMJ describes the impact of common musculoskeletal conditio

Precision and Personalized Medicine: How Genomic Approach Improves the Management of Cardiovascular and Neurodegenerative Disease.

Life expectancy has gradually grown over the last century. This has deeply affected healthcare costs, since the growth of an aging population is correlated to the increasing burden of chronic diseases. This represents the interesting challenge of how to manage patients with chronic diseases in order to improve health care budgets. Effective primary prevention could represent a promising route. To this end, precision, together with personalized medicine, are useful instruments in order to investigate pathological processes before the appearance of clinical symptoms and to guide physicians to ch

Patterns of HIV, TB, and non-communicable disease multi-morbidity in peri-urban South Africa- a cross sectional study.

BACKGROUND: Many low and middle-income countries are experiencing colliding epidemics of chronic infectious (ID) and non-communicable diseases (NCD). As a result, the prevalence of multiple morbidities (MM) is rising. METHODS: We conducted a study to describe the epidemiology of MM in a primary care clinic in Khayelitsha. Adults with at least one of HIV, tuberculosis (TB), diabetes (DM), and hypertension (HPT) were identified between Sept 2012-May 2013 on electronic databases. Using unique patient identifiers, drugs prescribed across all facilities in the province were linked to each patient

Loss of antinociceptive effectiveness of morphine and oxycodone following titration of levothyroxine: case reports and a brief review of published literature.

Chronic pain management is a complex process involving numerous facets of care. Although pharmacotherapy is a part of the treatment plan for these patients, it often represents the most complex of the modalities to manage. Two chronic pain patients with loss of pain control following dosage increase in levothyroxine supplementation are presented. The authors sought to identify relationships among thyroid status, opioid pharmacokinetics, and nociceptive processing. In conclusion, well-designed human studies using pain models and controlling for thyroid status are warranted to better understand

Bioimpedance spectroscopy for breast cancer-related lymphedema assessment: clinical practice guidelines.

PURPOSE: Breast cancer-related lymphedema (BCRL) represents a significant concern for patients following breast cancer treatment, and assessment for BCRL represents a key component of survivorship efforts. Growing data has demonstrated the benefits of early detection and treatment of BCRL. Traditional diagnostic modalities are less able to detect reversible subclinical BCRL while newer techniques such as bioimpedance spectroscopy (BIS) have shown the ability to detect subclinical BCRL, allowing for early intervention and low rates of chronic BCRL with level I evidence. We present updated clini

User profiles and personas in the design and development of consumer health technologies.

BACKGROUND: "The graying of the globe" has resulted in exponential rise in health care expenses, over-worked health care professionals and a growing patient base suffering from multiple chronic diseases, one of which is diabetes. Consumer health technologies (CHT) are considered important catalysts for empowering health care consumers to take a proactive role in managing their health and related costs. Adoption rate and usability of such devices among the aging is far from being satisfactory. Past studies noted the motivation for adoption by the aging is dependent on the suitability/relevance,

Diagnostic innovation and models of care to improve fibrosis detection and risk stratification in steatotic liver disease.

Steatotic liver disease (SLD) is the leading cause of chronic liver disease in Europe, with liver fibrosis representing the strongest predictor of liver-related outcomes and an important contributor to cardiometabolic risk. This Series paper examines diagnostic innovation and models of care to improve fibrosis detection and risk stratification across the continuum of care for SLD. A growing range of non-invasive tests for fibrosis assessment is now available, including blood-based biomarkers, imaging modalities, automated laboratory algorithms, and artificial intelligence-enabled tools. The benefits of physician-pharmacist collaboration.

Over the past decade, physician-pharmacist collaborative practices have gained traction in primary care as a way to implement team-based-care models. And there is evidence pointing to the effectiveness of this multidisciplinary heath care team approach, in which pharmacists are typically responsible for such things as obtaining medication histories, identifying barriers to adherence, and adjusting medication regimens. Several studies have shown the significant impact that physician-pharmacist collaborative management (PPCM) can have on blood pressure control among patients with hypertension. A

Randomized Trial on the Clinical Utility of a Novel Biomarker Panel to Identify Treatable Determinants of Chronic Pain.

Millions suffer daily from chronic pain diagnosed anatomically and treated with opioids. Research shows that underlying nutritional, metabolic and oxidative stressors, which drive the development or worsening of chronic pain, are not diagnosed despite the fact that treatment of these primary pain pathways relieves pain and increases function. One of the main reasons for this gap in care is the lack of a simple diagnostic assay to help clinicians make these diagnoses. We examined the clinical utility of a urine-based pain biomarker panel. Primary care physicians were randomized into the test gr
