An Approach for Combining Clinical Judgment with Machine Learning to Inform Medical Decision Making: Analysis of Nonemergency Surgery Strategies for Acute Appendicitis in Patients with Multiple Long-Term Conditions.

BACKGROUND: Machine learning (ML) methods can identify complex patterns of treatment effect heterogeneity. However, before ML can help to personalize decision making, transparent approaches must be developed that draw on clinical judgment. We develop an approach that combines clinical judgment with ML to generate appropriate comparative effectiveness evidence for informing decision making. METHODS: We motivate this approach in evaluating the effectiveness of nonemergency surgery (NES) strategies, such as antibiotic therapy, for people with acute appendicitis who have multiple long-term condit

Moving from a clinical trial to routine care: a qualitative analysis of primary care physicians' experiences with interventions to promote shared decision making for colorectal cancer screening in older adults.

BACKGROUND: The Promoting Informed Decisions About Colorectal Cancer Screening in Older Adults (PRIMED) cluster randomized trial found that 2-hour online training combined with a pre-visit reminder for primary care physicians (PCPs) increased shared decision making (SDM) among patients aged 76-85 years who were due for colorectal cancer screening (CRC) screening discussions. However, the best way to translate findings from the clinical trial setting into routine primary care remains unclear. OBJECTIVE: To explore PCPs' experiences with the PRIMED SDM training and reminders, and to identify ba

Clinical decision-making and health-related quality of life during first-line and maintenance therapy in patients with advanced non-small cell lung cancer (NSCLC): findings from a real-world setting.

BACKGROUND: Maintenance therapy (MT) with pemetrexed has been shown to improve overall and progression-free survival of patients with non-squamous non-small cell lung cancer (NSCLC), without impairing patients' health-related quality of life (HRQOL) substantially. Comprehensive data on HRQOL under real-life conditions are necessary to enable informed decision-making. This study aims to (1) assess HRQOL during first-line chemotherapy and subsequent MT and (2) record patients' and physicians' reasons leading to clinical decisions on MT. METHODS: Patients treated for NSCLC at three Austrian medi

The Clinical Utility of a 7-Gene Biosignature on Radiation Therapy Decision Making in Patients with Ductal Carcinoma In Situ Following Breast-Conserving Surgery: An Updated Analysis of the DCISionRT® PREDICT Study.

BACKGROUND: Breast-conserving surgery (BCS) followed by adjuvant radiotherapy (RT) is a standard treatment for ductal carcinoma in situ (DCIS). A low-risk patient subset that does not benefit from RT has not yet been clearly identified. The DCISionRT test provides a clinically validated decision score (DS), which is prognostic of 10-year in-breast recurrence rates (invasive and non-invasive) and is also predictive of RT benefit. This analysis presents final outcomes from the PREDICT prospective registry trial aiming to determine how often the DCISionRT test changes radiation treatment recommen

Serum Vascular Endothelial Growth Factor and Endostatin as an Adjunct to Clinical Decision Making in Managing Radiation-induced Changes Post Gamma Knife Radiosurgery in Spetzler Martin Grade 3 Arteriovenous Malformations Patients: A Pilot Study.

BACKGROUND: Radiation induced changes (RIC) are the most common complications observed post Gamma Knife radiosurgery (GKRS) and may be observed within 6-18 months post procedure. It has been observed that almost one-third of RICs are symptomatic and half of them are persistent. There is no way to predict which patients will develop these changes and to what extent. METHODS: This was a prospective analytical pilot study with the aim of understanding the role of serum vascular endothelial growth factor (VEGF) and endostatin as predictive factors for clinically symptomatic RIC in intracranial ar

Understanding and Addressing Challenges With Electronic Health Record Use in Gynecological Oncology: Cross-Sectional Survey of Multidisciplinary Professionals in the United Kingdom and Co-Design of an Integrated Informatics Platform to Support Clinical Decision-Making.

BACKGROUND: Electronic health records (EHRs) are a cornerstone of modern health care delivery, but their current configuration often fragments information across systems, impeding timely and effective clinical decision-making. In gynecological oncology, where care involves complex, multidisciplinary coordination, these limitations can significantly impact the quality and efficiency of patient management. Few studies have examined how EHR systems support clinical decision-making from the perspective of end users. This study aimed to explore multiprofessional experiences of EHR use in gynecologi…

CALME: A Cognitive Loop for Ethical Deliberation During Clinical Action.

RATIONALE, AIMS AND OBJECTIVES: Most established approaches to clinical ethics support deliberation before or after care, through processes such as informed consent, institutional guidance, ethics consultation and retrospective case review. Yet ethically significant decisions often arise during clinical action, when clinicians must respond under time pressure, manage competing technical demands and navigate asymmetries of knowledge and power. Although the literature on everyday ethics shows that routine practice is ethically dense, it does not provide a compact reasoning structure designed for

Decision-making in sporadic vestibular schwannoma.

Over the last 20 years, there has been a marked shift from active intervention to radiologic observation in the management of vestibular schwannomas, prompted by our understanding that a substantial proportion of these tumors do not demonstrate significant growth following diagnosis. In parallel with this, when active intervention does take place, our outcome measures are increasingly focused on preserving neurologic function, rather than on complete tumor removal at any cost. These principles of management are now widely accepted in most major skull-base centers throughout the world, but a nu
