In a collective study of 22 surgical clinics 1235 closed diaphyseal tibial fractures in children from 0 to 14 years were analysed. The incidence of this type of fracture in relation to all fractures in childhood, was 6.2%. In pre-school children isolated tibial fractures and incomplete fractures predominated, while in children from 6 to 14 years tibial plus fibular fractures as well as displaced fractures occurred more frequently. Closed reduction and plaster cast were used as standard treatment. Traction was used only exceptionally. In only 3.0% osteosynthesis was performed under the followin

Because of small direct contact to the fracture fragments reduction of metatarsal fractures can be achieved only indirectly by applying traction to the toes. In this article a method for simple direct manipulation of the distal main fragment of metatarsal fractures is described: A K-wire is drilled in dorsi plantar direction across the head of the fractured metatarsal and mounted on a traction bow. In this manner the distal fracture fragment can be moved in all directions and placed exactly onto the proximal fragment. All 8 patients with multiple metatarsal fractures could be treated by closed

Fractures of the distal end of the tibia in children often involve the physis. They are of particular importance because partial growth arrest can occur and result in angular deformity, limb-length discrepancy, or incongruity of the joint surface (or a combination of these). We evaluated the cases of thirty-two children who had a fracture leading to established partial growth arrest of the distal end of the tibia. Most of this group had had a Salter-Harris Type-III or Type-IV fracture. Twenty-eight of the fractures had been treated by gentle closed reduction and immobilization in a plaster cas

This report describes technical tools for the treatment of pediatric displaced and unstable diaphyseal forearm fractures, which are difficult to treat by manual closed reduction. During intramedullary fixation with Kirchner wires, we propose our original small distractor as an intraoperative distraction apparatus without open reduction. This apparatus is made using parts of the Ilizarov external fixator. Apart from general external fixation devices, our apparatus is adequate for the effective shortening and lateral dislocation of the fracture site. This adequacy is not only because of the thre

BACKGROUND: Monteggia fracture is an uncommon lesion, which associates ulnar shaft fracture and radial head dislocation. Equivalent Monteggia fracture can occur by associated lesions such as olecranon fracture or radial neck or head fracture.
CASE PRESENTATION: We present the case of a five-year-old girl, right handed, involved in domestic accident falling off a bed and sustained an isolated trauma of the left upper limb. Radiograph showed an unusual case of Monteggia equivalent lesion associating a fracture of the proximal third of the ulna shaft and a growth plate fracture Salter I of the p

INTRODUCTION: Distal radius fractures commonly result from falls onto an outstretched hand, making them a frequent presentation in orthopedic practice. However, pisiform dislocation associated with a distal radius fracture is uncommon, due to the pisiform's protected anatomical position and the distinct biomechanical forces typically required for its displacement. When this combined injury pattern is further complicated by ulnar nerve neuropraxia, it becomes rare and may be easily overlooked. The clinical subtlety of pisiform dislocation can lead to diagnostic delay, potentially resulting in i

CASE: A 19-year-old man sustained combined, ipsilateral inferior hip and posterior knee fracture-dislocations secondary to a motor vehicle collision. He underwent immediate closed reduction of the knee and delayed open reduction internal fixation but required emergent open hip reduction for an irreducible femoral head incarcerated on a pubic root fracture. At the 1-year follow-up, he demonstrated excellent functional outcome with painless and full hip and knee range of motion.
CONCLUSION: Irreducible inferior femoral head dislocation in combination with a knee dislocation requires thoughtful

Single-contrast arthrography was performed in six infants who had an injury to the elbow because the exact nature of the injury was uncertain from interpretation of the plain radiographs and clinical findings. A correct diagnosis was made in all six patients, and it significantly altered the treatment in five. Four lesions were a Salter-Harris Type-I fracture and two were a Salter-Harris Type-II fracture. Three patients had a closed reduction, two required no reduction, and one had an open reduction. Child abuse of two infants was confirmed, and it was suspected in another two.

Pediatric emergency physicians at Kaplan Medical Center regularly reduce forearm fractures under point-of-care ultrasound guidance in the pediatric emergency department. We present 3 children who arrived at our department with shortened, angulated forearm fractures within the span of a single week. We report on the successful reduction of these fractures under point-of-care ultrasound and on the maintenance of anatomic alignment at follow-up 1 week postreduction.

The incidence of diacapitular (intracapsular) fractures of the mandibular condyle has increased in recent years, but their treatment remains inadequate, particularly of type B fractures. To evaluate and compare outcomes of open and closed treatments we created a sheep model of a type B fracture through an osteotomy. Eight sheep were randomly divided into two groups of four each to be given closed treatment or open reduction and internal fixation (ORIF) and were evaluated by veterinary and radiological examinations. Compared with before the osteotomy, there were significant reductions in maximu

BACKGROUND: Supracondylar humerus fractures are widely considered the most common fracture of the elbow in children. Fractures can range from a less severe, nondisplaced type I fracture to a more severe, displaced type III fracture with no cortical contact. Type III fractures can lead to adverse physical, social, and emotional consequences if they are not treated effectively. The American Academy of Orthopaedic Surgeons recently carried out a systematic review of the literature to develop a clinical practice guideline. The guidelines provided answers for the following questions regarding the t

BACKGROUND: Recent studies indicate that formal postreduction radiographs may be unnecessary for closed, isolated pediatric wrist, and forearm when mini C-arm fluoroscopy is used for reduction. Our institution changed the Emergency Department (ED) management protocol to reflect this by allowing orthopaedic providers to determine if fluoroscopy was acceptable to assess fracture reduction. We hypothesized that using fluoroscopy as definitive postreduction imaging would decrease total encounter time, without an increase in the rate of rereduction or surgery.
METHODS: Patients with closed, isolat

OBJECTIVE: Emergency department (ED) reduction of pediatric fractures occurs most commonly in the forearm and can be challenging if fluoroscopy is not available. We sought to assess the ability of point of-care ultrasonography (POCUS) to predict adequacy of reduction by fluoroscopy.
METHODS: We prospectively enrolled ED patients 0-17 years of age with radial and/or ulnar fractures requiring reduction under fluoroscopic guidance. Post-reduction POCUS (probe dorsal, volar, and coronal) and fluoroscopic (AP and lateral) fracture images were recorded. Fracture angles were compared between blinded

BACKGROUND Supracondylar humeral fracture is a common fracture in the pediatric population. Although extension-type is the most common fracture pattern (97% to 98%), flexion-type supracondylar fractures are rarely encountered (2% to 3%). The combination of a flexion-type supracondylar humeral fracture with an ulnar nerve injury represents a real challenge for an orthopaedic surgeon. CASE REPORT We report 2 cases of flexion-type supracondylar humeral fracture with ulnar nerve injury that open reduction and fixation was necessary because closed reduction could not achieve an acceptable result.
