[Experience with the use of new tests for detecting Clostridium difficile].

In recent years, Clostridium difficile has been among the most important nosocomial pathogens. It causes intestinal infectious diseases detectable by numerous laboratory methods. Given the severity of the diseases, some tests have proved inadequately sensitive. The article summarizes the first experience with the latest kits for early and sensitive detection of toxic strains of Clostridium difficile.

[Bacteremia due to Clostridium difficile].

We describe a case of bacteremia due to Clostridium difficile, which was successfully treated by intravenous vancomycin. A 69-year-old woman was admitted to our hospital because of third degree burn injuries. She was treated with cefazolin for two weeks followed with flomoxef for one week before the operation (debridement and grafting of skin). On the third postoperative day high fever (temperature 40 degrees C), abdominal pain and severe watery diarrhea developed. Antibiotic-associated colitis with bacteremia was diagnosed presumptively, flomoxef was stopped, and oral and intravenous therapy

Updated sequence of ramoplanin biosynthetic gene cluster from Actinoplanes ramoplaninifer ATCC 33,076.

Actinoplanes ramoplaninifer ATCC 33,076 is the only known producer of the glycolipodepsipeptide antibiotic ramoplanin. Ramoplanin (Rmp) interferes with peptidoglycan biosynthesis by binding to lipid II and consequently impeding the transglycosylation reactions. Rmp exhibits excellent antimicrobial properties against Gram-positive bacteria, including methicillin- and vancomycin-resistant strains of staphylococci and enterococci, as well as Clostridioides difficile, showing no cross-resistance with glycopeptides and β-lactams. Rmp is poorly absorbed in the gastrointestinal tract when administere…

Comparative evaluation of BD MAX™ Enteric Panels and Luminex NxTAG® GPP for the detection of gastrointestinal pathogens in clinical stool samples.

PURPOSE: To compare the diagnostic performance of BD MAX™ Enteric Panels and the Luminex NxTAG® Gastrointestinal Pathogen Panel (GPP) for the detection of enteric pathogens under routine clinical conditions. METHODS: A prospective single-center study analyzed 165 stool samples (one per patient) from individuals with suspected gastrointestinal infection attending the Clínico-Malvarrosa Health Department (Valencia, Spain) between October and November 2023. Samples tested using all BD MAX™ enteric panels (bacterial, viral, and Clostridioides difficile panels) were also processed by Luminex NxTAG…

Antimicrobial treatment for 7 versus 14 days in patients with bacteremia: a meta-analysis of randomized controlled trials.

PURPOSE: The optimal duration of antibiotic treatment in patients with bacteremia is a matter of ongoing debate. METHODS: We conducted a meta-analysis of randomized controlled trials that compared 7 days with 14 days of antimicrobial treatment in adults with bacteremia. The systematic search included trials published until December 2024. Efficacy outcomes included 90-day all-cause mortality, recurrence of bacteremia and mean length of hospital stay. Safety outcomes included the total number of adverse events, Clostridioides difficile infections, diarrhea, acute kidney injury, rash or emergenc…

High-resolution metabolomic analysis of stool reveals expanded biomarkers of C. difficile colitis and insights into pathophysiology.

Clostridioides difficile infection (CDI) is facilitated by gut microbiome disruption and associated metabolic disturbances. While many prior studies have focused on the microbial composition of CDI stool, fewer have explored the fecal metabolome using untargeted high-resolution metabolomics (HRM). To characterize the metabolic phenotype of CDI, we performed untargeted HRM on stool samples from 55 CDI patients and 72 healthy controls. CDI was associated with marked alterations in stool metabolites, mapping to 14 significantly dysregulated pathways, including tryptophan metabolism, bile acid met…

Pediatric Clostridiodes difficile infection with toxic megacolon successfully treated with fecal microbiota transplantation.

The rising incidence of pediatric Clostridioides difficile infection (CDI), particularly in medically complex patients, calls for novel, individualized treatments. While fecal microbiota transplantation (FMT) is well-described for recurrent CDI, this report describes the first pediatric case of fulminant CDI with toxic megacolon successfully treated with FMT. This critically ill patient experienced failure of standard-of-care antibiotics and was a poor surgical candidate due to neutropenia and abdominal radiation for rhabdomyosarcoma. Ultimately, FMT rapidly resolved the patient's symptoms wit…

Fecal Microbiota Transplantation for the Treatment of Inflammatory Bowel Disease: An Update.

Fecal microbiota transplantation (FMT) has successfully been applied for the treatment of recurrent Clostridioides difficile infection (CDI), which has led to studies on its application to other gastrointestinal diseases and extraintestinal diseases associated with gut microbiota dysbiosis. Recently, the results of FMT for patients with inflammatory bowel disease (IBD) have been encouraging. However, studies have not fully clarified the clinical application of this emerging therapy. Here, we aimed to review the current knowledge in this fast-growing field and characterize the effectiveness, sa…

Optimizing Patient-Zone Environmental Hygiene: The Way Forward.

This review highlights recent advances in understanding the role of environmental surfaces in healthcare-associated infections, focusing on pathogens like Clostridioides difficile, carbapenem-resistant Enterobacteriaceae, and vancomycin-resistant Enterococci. It emphasizes the importance of effective disinfection, including sporicidal cleaning and environmental monitoring, in reducing transmission. Despite progress, challenges remain in implementing sustainable, science-based infection control practices due to resource limitations and variability in hospital settings. The review advocates for …

Fecal Microbiota Transplantation in Dogs.

In people, fecal microbiota transplantation is recognized as the best treatment modality for recurrent Clostridioides difficile infection in people, and its value is currently investigated in the treatment of other diseases associated with an abnormal gut microbiome. In dogs, intestinal dysbiosis has been documented in many acute and chronic digestive diseases as well as in diseases of other organ systems. There are only few published studies evaluating the benefits of fecal microbiota transplantation (FMT) in canine gastrointestinal disorders. They provide evidence that FMT may be beneficial …

[Guidelines for diagnosis, treatment and prevention of clostridium difficile infection in China(2024)].

Clostridium difficile infection(CDI) is an intestinal infection caused by Clostridium difficile, characterized by clinical symptoms such as fever, abdominal pain, watery diarrhea, and loose stools. Mild cases present with diarrhea, while severe cases can progress to pseudomembranous colitis, often accompany by complications such as toxic megacolon, intestinal perforation, and septic shock. CDI is a significant cause of complications and mortality, especially among elderly patients, and is one of the most common hospital-acquired infections. The purpose of this guideline is to provide guidance on the dia
