Coccidioidomycosis in liver transplant recipients relocating to an endemic area.

Coccidioidomycosis is an endemic fungal infection of the desert southwestern United States. This infection occurs at a rate of 1% to 8% in solid organ transplant recipients residing in the endemic area, and it has a high rate of disseminated infection and mortality. The risk of infection among transplant recipients from nonendemic areas visiting or moving to an endemic region is not known. We reviewed the clinical course of 41 liver transplant recipients who originally resided in and underwent liver transplantation in an area of low coccidioidal endemicity and who later relocated their follow-

Isavuconazole in the Treatment of Chronic Forms of Coccidioidomycosis.

Coccidioidomycosis is a fungal infection with a range of clinical manifestations. Currently used antifungal agents exhibit variable efficacy and toxicity profiles that necessitate evaluation of additional therapeutic options. Improvement was observed in the majority of patients treated with isavuconazole, with clinical failures observed only in those with coccidioidal meningitis.

Coccidioidomycosis: A Contemporary Review.

Coccidioidomycosis, colloquially known as Valley Fever, is an invasive dimorphic fungal infection caused by Coccidioides immitis and C. posadasii. The fungi are found in the arid desert soils of the southwestern US, as well as in parts of Mexico and Central and South America. Acquisition is typically via inhalation of arthroconidia which become airborne after both natural (e.g., earthquakes, dust storms, and fires) and human-related events (e.g., military maneuvers, recreational activities, agriculture, and construction). The incidence of infection in increasing likely a result of both climati…

Diagnostic Contribution of Bronchoalveolar Lavage Sampling and Fungal Culture in a Dog With Pulmonary Coccidioidomycosis.

A 7-year-old, male neutered, Miniature Australian Shepherd from Arizona was presented for evaluation of a 3-month history of progressive cough. Thoracic radiographs revealed a focal alveolar pulmonary pattern and suspected tracheobronchial lymph node enlargement. Serum anti-Coccidioides spp. IgM/IgG antibodies were not detected by agar gel immunodiffusion performed by 2 different reference commercial veterinary laboratories approximately 3.5 and 3.75 months after respiratory tract signs were first noted. The dog failed to respond to empiric therapy with a cough suppressant and various antibiot…

Immunogenetics associated with severe coccidioidomycosis.

Disseminated coccidioidomycosis (DCM) is caused by Coccidioides, pathogenic fungi endemic to the southwestern United States and Mexico. Illness occurs in approximately 30% of those infected, less than 1% of whom develop disseminated disease. To address why some individuals allow dissemination, we enrolled patients with DCM and performed whole-exome sequencing. In an exploratory set of 67 patients with DCM, 2 had haploinsufficient STAT3 mutations, and defects in β-glucan sensing and response were seen in 34 of 67 cases. Damaging CLEC7A and PLCG2 variants were associated with impaired production…

Diagnosis of coccidioidomycosis by antigen detection using cross-reaction with a Histoplasma antigen.

BACKGROUND: In 2005, patients with coccidioidomycosis were observed to have positive Histoplasma antigen test results. METHODS: We performe … ed in bronchoalveolar lavage fluid. CONCLUSIONS: Physicians should be alerted that infections with Coccidioides species may cause positive Histoplasma antigen test results. There is potential for the use of this test in the diagnosis of coccidioidomycosis by taking advantage of this observed cross-reactivity. The greatest benefit appears to be in the populati …

Narrow-spectrum staining pattern of Pityrosporum.

BACKGROUND: Broad-spectrum fungal stains are used to detect fungal organisms, but narrow-spectrum stains can assist in fungal differential diagnosis. These stains include mucicarmine and Fontana-Masson (FM) for Cryptococcus, Alcian Blue for Cryptococcus and Blastomyces, Congo Red for Blastomyces and Coccidioides, and Ziehl-Neelsen for some examples of Blastomyces and Histoplasma. Pityrosporum is increasingly being recognized as a pathogen capable of significant cutaneous and systemic infections, but the narrow-spectrum staining pattern of Pityrosporum has not yet been systematically studied an…

A rare cause of unintentional weight loss, anhedonia, and cognitive decline.

Coccidioidal meningoencephalitis is a rare but serious neurologic disease caused by a fungus found in the southwestern United States. Because symptoms are nonspecific and complications can cause neurologic damage and death, providers should be alert for this disease in patients who have recently traveled to areas where the fungus is endemic.

Disseminated Coccidioidomycosis Presenting as Fever of Unknown Origin and Erythema Nodosum in a 3-Year-Old Child.

Disseminated coccidioidomycosis is a disease caused by Coccidioides species, fungi endemic to the southwestern United States. We present here an uncommon case of a young child with erythema nodosum and fever of unknown origin, found to have the infection. While more common in adults, coccidioidomycosis should be considered in all patients with erythema nodosum.

Coccidioidomycosis Exposure Assessed by Skin Testing and Environmental Factors in Baja California, Mexico.

Baja California is the second-highest state in Mexico for hospital discharges attributed to coccidioidomycosis (CM), yet epidemiological information on exposure patterns in affected communities remains limited. To estimate exposure to Coccidioides and assess its association with environmental factors, we conducted intradermal coccidioidin skin testing among 416 residents across nine regions of Baja California. We analyzed 24 environmental variables, including bioclimatic, topographic, and land use indicators. Overall, 31.9% of participants tested positive. Higher odds of exposure were observed…

Prolonged voriconazole treatment in a patient with chronic lymphocytic leukemia resulting in a litany of chronic overlapping toxicities.

Voriconazole is a triazole antifungal with activity against a number of yeast and mold species including Candida, Aspergillosis, Fusarium, and Coccidioides. Invasive fungal infections are associated with high morbidity and mortality, prolonged treatment courses, and occasionally lifelong suppressive therapy. Voriconazole therapy can result in a number of acute toxicities that clinicians are frequently aware of including hepatotoxicity, visual disturbances, and hallucinations; however, there is limited experience with extended durations of voriconazole therapy. We describe the case of a 62-year…

Pulmonary coccidioidomycosis.

Coccidioidal infection can manifest as pulmonary or extrapulmonary disease. Pulmonary coccidioidomycosis occurs in 95% of all cases and can be divided into three main categories: primary, complicated, and residual pulmonary coccidioidomycosis. The primary infection occurs with inhalation of airborne arthroconidia. As few as 10 arthroconidia are capable of causing an infection in animal models. Sixty percent of infected individuals will remain asymptomatic. This results in a positive skin test and, with rare exception, lifelong immunity. The other 40% will develop symptomatic disease that manif

Coccidioidomycosis in returned Australian travellers.

Coccidioidomycosis is a fungal infection caused by Coccidioides species. The disease has wide clinical presentation and a distinct geographical distribution. We describe two cases of coccidioidomycosis in returned Australian travellers who presented to Nambour Hospital. Knowledge of the international geographical distribution of endemic fungal infections and their clinical manifestations can assist in earlier diagnosis and appropriate management.

[Pericarditis caused by coccidioidomycosis. Report of a case].

A 33 year-old man developed coccidioidomycosis which resulted in pericarditis associated with congestive heart failure. A pericardial effusion developed and progressed to constrictive pericarditis. A pericardiectomy was performed and revealed that the pericarditis was due to Coccidioides immitis. The patient died after surgery. This is the second case of coccidioidomycosis with pericarditis reported in Mexico's literature.

Four great coccidioidomycologists: William Ophuls (1871-1933), Myrnie Gifford (1892-1966), and Charles Edward Smith (1904-1967) and William A. Winn (1903-1967).

The careers of William Ophuls (1871-1933), Myrnie Gifford (1892-1966), Charles Edward Smith (1904-1967) and William A. Winn (1903-1967) are briefly reviewed, with emphasis on their contributions to knowledge of Coccidioides and coccidioidomycosis. All were students with broad interests, and all were quite willing to suggest new concepts and classifications to replace those of their predecessors.

Double electroimmunodiffusion: a rapid diagnostic test for canine coccidioidomycosis.

Double electroimmunodiffusion (EID) was adapted for detection of antibodies to Coccidioides immitis. In a limited experiment with canine serums, the test was found to be qualitatively as sensitive as the complement-fixation (CF) test. Advantages of EID over CF are that EID takes only 30 minutes to perform and requires only 10 mul each of antigen and antibody.
