Cognitive-behavioral guided self-help for eating disorders: effectiveness and scalability.

Given the well-documented shortage of cognitive-behavioral therapy (CBT) for eating disorders, there is a compelling need for advances in dissemination. Guided self-help based on cognitive-behavioral principles (CBTgsh) provides a robust means of improving implementation and scalability of evidence-based treatment for eating disorders. It is a brief, cost-effective treatment that can be implemented by a wide range of mental health providers, including non-specialists, via face-to-face contact and internet-based technology. Controlled studies have shown that CBTgsh can be an effective treatment

Cognitive regulation of emotion in bipolar I disorder and unaffected biological relatives.

OBJECTIVE: We examined the use of particular cognitive strategies for regulating negative emotion in relation to mood and temperament in BD-I, unaffected relatives of bipolar patients (UR), and healthy controls (HC). METHOD: Participants were 105 patients with BD-I, 124 UR, and 63 HC; all participants completed the Cognitive Emotion Regulation Questionnaire (CERQ), the Depression Anxiety Stress Scales (DASS), and the Hypomanic Personality Scale (HPS). RESULTS: The BD-I group reported more frequent use of rumination, catastrophizing and self-blame, and less frequent use of putting into perspe

Longitudinal changes in hippocampal volumes and cognition in remitted geriatric depressive disorder.

Growing evidences suggest that the abnormality of hippocampal volume may occur in the process of depression. In this longitudinal study, we calculated the hippocampal volume of 14 remitted geriatric depressed (RGD) patients and 19 healthy participants at baseline and follow-up. We found significant improvement of performance in Trail Making Test-A (P=0.038) and Test-B (P=0.032), and the right hippocampal volume increased mildly in RGD. However, in RGD patients, positive correlations were seen between the changes in right hippocampal volumes and Symbol Digit Modality Test scores (r=0.675, P=0.0

The effect of overweight/obesity on cognitive function in euthymic individuals with bipolar disorder.

BACKGROUND: Persistent impairment in cognitive function has been described in euthymic individuals with bipolar disorder. Collective work indicates that obesity is associated with reduced cognitive function in otherwise healthy individuals. This sub-group post-hoc analysis preliminarily explores and examines the association between overweight/obesity and cognitive function in euthymic individuals with bipolar disorder. METHODS: Euthymic adults with DSM-IV-TR-defined bipolar I or II disorder were enrolled. Subjects included in this post-hoc analysis (n=67) were divided into two groups (normal

Efavirenz associated with cognitive disorders in otherwise asymptomatic HIV-infected patients.

BACKGROUND: Despite the availability of potent antiretroviral regimens (combination antiretroviral therapy [cART]), HIV-associated neurocognitive disorders (HAND) are increasingly recognized. Our aim was to investigate the prevalence and treatment-related correlates of HAND, exploring the potential neurotoxicity of antiretrovirals on cognitive functions. METHODS: We performed a cross-sectional single cohort study by consecutively enrolling asymptomatic HIV+ subjects during routine outpatient visits. Each patient was submitted to a comprehensive neuropsychological battery and was considered co

Clinical and cognitive correlates of suicide attempts in bipolar disorder: is suicide predictable?

OBJECTIVE: We conducted a retrospective investigation of potential clinical, demographic, and neuropsychological risk factors for suicide attempts in patients diagnosed with bipolar disorder. METHOD: Participants included 67 adult inpatients and outpatients aged 18-60 years meeting DSM-IV criteria for bipolar disorder (bipolar I and II disorders, bipolar disorder not otherwise specified). We assessed demographic factors, mood symptoms, psychosis, trauma history, trait impulsivity, trait aggression, and reasons for living. The primary outcome measures were the Barratt Impulsiveness Scale-versi

Evaluation of cognitive-analytic therapy (CAT) outcome in patients with panic disorder.

Two categories of treatment have been shown to be effective in treating panic disorder with or without agoraphobia. One is pharmacotherapy using antidepressants and benzodiazepinesand the other is psychotherapy. The present study aims at the assessment of the outcome of Cognitive-Analytic Therapy (CAT), a type of brief psychotherapy, in a sample of 128 psychiatricoutpatients with DSM-IV diagnosis of panic disorder, who attended the Mental Health Center of Northwestern District of Thessaloniki. For this purpose, validated instruments for the evaluation, such as the Minnesota Multiphasic Personal

Clarifying criteria for cognitive signs and symptoms for eating disorders in DSM-V.

OBJECTIVE: This article reviews literature relevant to challenges in clinical ascertainment of cognitively-based diagnostic criteria of anorexia nervosa (AN) and bulimia nervosa (BN) to examine whether revision might enhance their clinical utility. METHOD: We performed a systematic literature search to identify publications relevant to clinical evaluation of cognitive symptoms of AN and BN. RESULTS: The literature supports several reasons that individuals with an eating disorder may not endorse cognitive symptoms, despite their presence. These include limited insight, minimization, or denial

Rapid tryptophan depletion following cognitive behavioural therapy for panic disorder.

OBJECTIVE: The aim of this study was to examine the effect of rapid tryptophan depletion (RTD) combined with a panicogenic challenge in patients with panic disorder who had responded to treatment with cognitive behavioural therapy (CBT). We hypothesised that RTD (compared with the control drink) would result in an increase in anxiety symptoms when provoked by a panicogenic challenge with the benzodiazepine antagonist, flumazenil. METHODS: Nine patients with panic disorder who had responded to CBT received a tryptophan-free amino acid drink on one occasion and a control drink on the other in a

Cognitive content and process in eating-disordered patients with obsessive-compulsive features.

OBJECTIVE: This study examined the schema-level cognitive content and processes that might explain the presence of compulsive behaviours among patients with eating disorders. METHOD: Each of 62 eating-disordered women completed standardised measures of schema-level core beliefs and cognitive processes, and was interviewed for diagnosis and obsessive compulsive features. RESULTS: Compulsive behaviours were associated with four core beliefs--'mistrust/abuse', 'defectiveness/shame', 'dependence/incompetence' and 'subjugation'. Higher levels of avoidance of emotional arousal were also associated

[Cognitive biases in an experimental task of focalized selective attention in eating disorders].

INTRODUCTION: Cognitive processing differences based on attentional biases of words pertaining to eating disorders were investigated. We performed a computerized Stroop color-naming task in which the subjects named the colors of four word groups (food-related, eating disorders behaviours-related, emotional and neutral) to measure differences in the processing of these stimuli. METHOD: Participants were 144 females with eating disorders (anorexia nervosa, bulimia nervosa, or a combination of both) and 172 female controls. RESULTS: In agreement with predictions, the three eating disorder group

Impact of cognitive-behavioral therapy for panic disorder on comorbidity: a controlled investigation.

This study examined the effects of cognitive-behavioral therapy (CBT) for principal panic disorder with or without agoraphobia, on comorbidity in 30 individuals (16 female). To test the hypothesis that improvements in co-existing conditions were not due to spontaneous fluctuations across time, patients receiving immediate CBT were compared to those assigned to wait list (n = 11). Results indicated clinician-rated severity of comorbid specific phobia declined significantly following immediate CBT compared to no change after wait list. The number of patients without comorbidity of any severity i

A pilot study of two-day cognitive-behavioral therapy for panic disorder.

The present study investigated the short-term efficacy of brief, intensive cognitive-behavioral therapy (CBT) for panic disorder (PD). The treatment involved 9h of therapist contact over two consecutive days and was developed for the purpose of delivering CBT for PD to a largely rural patient population that must travel long distances to find a treatment provider. Ten patients who elected to participate in brief, intensive CBT instead of weekly CBT were recruited from routine clinical practice in a hospital-based anxiety disorders clinic. Patients were not excluded based on the presence of ago

Effects of group experiential cognitive therapy for the treatment of panic disorder with agoraphobia.

A treatment protocol, called experiential cognitive therapy (ExCT), was developed. It integrated traditional cognitive behavior therapy (CBT) with virtual reality exposure for the treatment of panic disorder with agoraphobia (PDA). The objective of this study was to test the efficacy of short-term (four sessions) ExCT compared with a traditional 12-session panic control program (PCP) for the treatment of PDA. Forty patients diagnosed as having PDA by the diagnostic criteria of DSM-IV were randomly assigned to ExCT and PCP groups of 20 patients each. The treatment effects were measured with sel

Sense of hyper-positive self and response to cognitive therapy in bipolar disorder.

INTRODUCTION: Cognitive therapy (CT) for bipolar disorder emphasizes the monitoring and regulation of mood, thoughts and behaviour. The Sense of Hyper-Positive Self Scale (SHPSS) measures the extent to which bipolar patients value themselves and perceive themselves to possess personal attributes (e.g. dynamism, persuasiveness and productiveness) associated with a state of being 'mildly high', which does not reach the severity of clinical hypomania. It is hypothesized that patients who score highly on the SHPSS do not respond well to cognitive therapy. METHOD: One hundred and three bipolar-I p

Cognitive and behavioral treatments for anxiety disorders: a review of meta-analytic findings.

Behavioral and cognitive psychotherapies are the most widely studied psychological interventions for anxiety disorders. In the present article, the results of ten years of meta-analytic studies on psychotherapies for the various anxiety disorders are reviewed and the relative effectiveness of cognitive and behavioral therapeutic methods is examined. Meta-analytic results support the effectiveness of combined cognitive and behavioral approaches for anxiety disorders. Pure behavioral therapies also are effective and appear to work as well as combined treatment for some disorders. Due to the smal

Symptom presentation and outcome of cognitive-behavioral therapy for obsessive-compulsive disorder.

Previous researchers have classified obsessive-compulsive disorder (OCD) patients by the themes of their obsessions and compulsions (e.g., washing, checking); however, mental compulsions have not been adequately assessed in these studies. The authors conducted 2 studies using a large sample of OCD patients (N=132). In the 1st study, they categorized patients on the basis of symptom presentation, giving adequate consideration to mental compulsions. Five patient clusters were identified: harming, contamination, hoarding, unacceptable thoughts, and symmetry. Mental compulsions were most prevalent

[Process analysis of cognitive-behavioral group psychotherapy for patients with panic disorder].

The study explores the verbal interaction relationship between therapists and members, and the interaction model of this group. Six subjects were chosen by convenience sampling from the psychiatric OPD of a medical center in the Taichung area. Subjects with informed consent participated in a 12-week course of closed group psychotherapy. All data was tape-recorded and the Hill Interaction Matrix (HIM) scale was applied for simultaneous triangulation, in order to analyze the group's dynamic development of cognitive-behavioral psychotherapy for patients with panic disorder. The results indicated
