[A Family with Peutz-Jeghers Syndrome Having Various Malignant Diseases].

We report a 27-year-old female who was diagnosed with adenocarcinoma of the uterine cervix associated with Peutz- Jeghers syndrome(PJS). She had undergone 5 surgeries for intestinal intussusception and had been diagnosed with PJS. She was referred to our hospital, complaining of watery vaginal discharge, and was diagnosed with adenocarcinoma of the uterine cervix. Following neoadjuvant chemotherapy, radical hysterectomy and small intestinal polypectomy by intraoperative endoscopy were performed. Among the third-degree relatives of her family, 10 had been diagnosed with PJS; of these 10, uterin

A novel treatment approach prolonging survival in an uncommon metastatic primary bladder adenocarcinoma.

Primary bladder adenocarcinoma (PBA) is an epithelial malignancy with pure glandular differentiation, without evidence of typical urothelial (transitional cell) carcinoma. PBA is rare, accounting for 0.5%-2% of all malignant bladder neoplasms, and it is seen more frequently in men than in women and is commonly diagnosed in the sixth decade of life.¹⁻³ Clinical presentation includes hematuria and symptoms of bladder irritation.² PBA is common in schistosomiasis-endemic regions and among patients with congenital bladder exstrophy (ectopia vesicae); it mostly arises in the trigone and posterior b

[Renal and upper urinary tract tumors: what the nephrologist should know].

Urinary tract carcinomas are among the most common malignancies and are derived from neoplastic transformation of the urothelium. They can be located in the lower (bladder, urethra) or upper (pyelocaliceal cavities, ureter) urinary tract. Urothelial carcinomas are the fourth most common cancer type after prostate or breast cancer, lung cancer, and colorectal cancer. Bladder cancer accounts for 90-95% of urothelial carcinomas and it is the most common malignancy of the urinary tract. Renal cancer also belongs to the urothelial carcinomas and is a relatively uncommon solid tumor, accounting for

Acromegaly and intestinal neoplasms.

Acromegalic subjects show increased frequency of neoplastic lesions in the colon and rectum with respect to the general population. Recent prospective studies using colonoscopy have shown a 3 time higher prevalence of intestinal polyps and up to 4 time increased presence of colorectal cancer in acromegaly, independently of sex, age, duration of disease and clinical status of the patients. The polyps are distributed throughout the extension of the large bowel and are often multiple, showing at least two different histologic types: hyperplastic and adenomatous. Sometimes they are associated with

Pattern of opioid prescriptions among patients with breast, lung, and colorectal cancer diagnosed with pre-existing chronic non-cancer pain.

BACKGROUND: Chronic non-cancer pain (CNCP) due to comorbid conditions presents a barrier to optimal pain management in patients with cancer. While opioid is widely used for the treatment and management of CNCP, it is also associated with misuse and addictions. Therefore, we determined the impact of CNCP on opioid prescribing patterns in patients with the three most common cancer types- breast, lung, and colorectal cancers. METHODS: A retrospective cohort study was conducted utilizing Surveillance Epidemiology and End Results (SEER)-Medicare (2006-2019) linked database. Patients, at least 66 y

Mapping the evolving trends of robotic surgery in gastric and colorectal cancer: a clinical trial landscape analysis.

Robot-assisted surgery provides important support for the treatment of gastric cancer (GC) and colorectal cancer (CRC) with its minimally invasive advantages. Through a systematic analysis of 188 clinical trials registered on ClinicalTrials.gov, this study aims to synthesize the research landscape in the field, identify evidence gaps, and provide a basis for optimizing clinical practice and guiding the design of future clinical trials. The analysis of 71 interventional trials with robotic surgery as the core intervention shows that the research activity has increased significantly after 2020.

Joint nested frailty models for clustered recurrent and terminal events: An application to colonoscopy screening visits and colorectal cancer risks in Lynch Syndrome families.

Joint models for recurrent and terminal events have not been yet developed for clustered data. The goals of our study are to develop a statistical framework for modelling clustered recurrent and terminal events and to perform dynamic predictions of the terminal event in family studies. We propose a joint nested frailty model for colonoscopy screening visits and colorectal cancer onset in Lynch Syndrome families. The screening and disease processes could each depend on individuals' screening history and other measured covariates and be correlated within families; our approach allows for familia

Chemoprevention in hereditary colorectal cancer syndromes.

Understanding of the neoplastic events involved in heritable colorectal cancer syndromes is increasing at the molecular and clinical levels. This knowledge is foundational to cancer chemoprevention, which attempts to inhibit or reverse the tumorigenic process through pharmacologic interventions applied before cancer occurrence. We reviewed all relevant published reports identified from Medline databases (1966-1998) regarding cancer chemoprevention in familial adenomatous polyposis, hereditary nonpolyposis colorectal cancer, and heritable colorectal cancer cohorts, including rodent models of th

[Familial occurrence of colorectal cancer: prevention, aftercare and genetic counseling].

Colorectal cancer (CRC) is the third most common type of cancer in Switzerland in terms of annual new cases and cancer deaths. Since most cantons have an organized screening program, more people are recorded with a positive family history of CRC. In the majority, the familial form of CRC is present, a hereditary form is much less common. Relatives of patients with CRC are a heterogeneous group whose CRC risk varies. A good assessment of disease risk can optimize the benefit-risk ratio for more intensive prevention. These recommendations («expert opinion statement») are intended to serve as a b

Merged testing for colorectal cancer syndromes and re-evaluation of genetic variants improve diagnostic yield: Results from a nationwide prospective cohort.

Approximately 5% of patients with colorectal cancer (CRC) have a Mendelian predisposition for the disease. Identification of the disease-causing genetic variant enables carrier testing and tailored cancer prevention within affected families. To determine the panorama and genetic variation of Mendelian CRC syndromes among referrals at the cancer genetics clinics in Sweden, 850 patients clinically selected for CRC genetic investigation were included in a prospective study that tested for all major hereditary polyposis and nonpolyposis CRC conditions. Genetically defined syndromes were diagnosed

Impact of Common Vitamin D-Binding Protein Isoforms on Supplemental Vitamin D3 and/or Calcium Effects on Colorectal Adenoma Recurrence Risk: A Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: Variants in the vitamin D-binding protein (DBP) gene (GC) encode DBP isoforms that may affect vitamin D metabolism. However, whether these isoforms modify the effects of vitamin D3 and/or calcium supplementation on colorectal adenoma recurrence is unclear. We hypothesized that supplementation effects may be stronger among those with the DBP2 isoform (encoded by the rs4588*A allele), which is associated with vitamin D deficiency and modified the associations of circulating vitamin D with risk for colorectal neoplasms in observational studies. OBJECTIVE: To estimate supplemental vit…

METACHRONOUS COLORECTAL LIVER METASTASIS.

Deaths related to colorectal cancer are generally associated with its metastases that affect the liver (50%) through the hematogenous route. Approximately 20-25% of these patients already have synchronous metastases in the liver at the time of primary tumor diagnosis. In others, liver metastases will occur during the course of the disease and are called metachronous. Metachronous metastases are believed to have a better prognosis; however, 20-25% of metastatic cases can be resected during the course of the disease. There is a lack of consensus on the diagnostic time interval for metastases to

[Methods for colon perfusion assessment].

Colorectal cancer is a malignant tumor arising from mucosa of large intestine (colon, rectosigmoid part and rectum). Currently, surgery is the primary treatment for colorectal cancer regarding oncological perspective. However, it is associated with high risk of postoperative complications, and one of these is anastomotic leakage (AL). According to various data, the incidence of AL can reach 35%. One of the main factors for AL is impairment of perfusion in anastomotic area. We hypothesize that preserving the left colic artery in rectosigmoid and rectal surgeries, as well as the first sigmoid ar

Revisiting the lateral-to-medial approach in robotic surgery for left-sided colorectal cancer.

Robotic surgery has been widely adopted for left-sided colorectal cancer; however, mesenteric mobilization is predominantly performed using the medial-to-lateral approach. This technique can be technically demanding because dissection must be performed within a narrow working space dorsal to the mesenteric vessels, requiring the identification and preservation of the ureter and gonadal vessels. In robotic surgery, arm rigidity and the lack of tactile feedback may increase the risk of inadvertent pressure on the deep retroperitoneal structures. To address these challenges, we describe a lateral

What Do Population-based Studies Tell Us about the Ideal Timing of Surgical Resection for Colorectal Cancer?

Timely access to curative-intent colorectal cancer (CRC) surgery is a central component of surgical quality. Yet contemporary evidence challenges historical assumptions that shorter wait times always confer better outcomes. Drawing on population-based studies which offer the most robust and generalizable data this article synthesizes what is known about the relationship between surgical delay and oncologic outcomes in CRC. We outline key conceptual considerations in defining wait-time intervals, discuss lessons from the coronavirus disease 2019 pandemic, and identify gaps for future research.

Urinary volatile organic compounds for colorectal cancer screening: A systematic review and meta-analysis.

BACKGROUND: The faecal immunochemical test (FIT) suffers from suboptimal performance and participation in colorectal cancer (CRC) screening. Urinary volatile organic compounds (VOCs) may be a useful alternative. We aimed to determine the diagnostic potential of urinary VOCs for CRC/adenomas. By relating VOCs to known pathways, we aimed to gain insight into the pathophysiology of colorectal neoplasia. METHODS: A systematic search was performed in PubMed, EMBASE and Web of Science. Original studies on urinary VOCs for CRC/adenoma detection with a control group were included. QUADAS-2 tool was u

Establishing a sustainable and high-utilization robot-assisted surgery platform in a regional cancer center: insights from a multidisciplinary, break-even-guided operational strategy.

Adoption of robot-assisted surgery (RAS) in regional non-urban cancer centers is constrained by high fixed costs and insufficient case volumes to achieve financial sustainability. Break-even thresholds vary widely (120-300 cases/year) across institution types; however, operational models for resource-constrained settings remain poorly characterized. To evaluate whether a financially sustainable, high-utilization RAS platform centered on colorectal cancer surgery can be established in a regional, non-urban cancer center through a four-phase, cross-departmental, volume-driven strategy guided by
