Feasibility of community-based hypertension screening and referral by village health teams in eastern Uganda: A quasi-experimental study.

BACKGROUND: Uganda has a high prevalence of hypertension (HTN) and low diagnosis rates due to healthcare system deficiencies, like scarce diagnostic resources and a shortage of health workers. Task-shifting roles to community health workers (CHWs) could potentially fill gaps in the control of HTN, but this is still underexplored. This study determined the feasibility of leveraging CHWs, called Village Health Teams (VHTs), for HTN screening and referral in Eastern Uganda. METHODS: We conducted a quasi-experimental study from June to November 2023 in Bugembe town council, Jinja City, Eastern Ug…

Community-based epilepsy care in an onchocerciasis-endemic area: A 3-year cohort study in Mahenge, Tanzania.

OBJECTIVE: In onchocerciasis-endemic areas, limited access to antiseizure medications (ASMs) contributes to a high epilepsy burden. This study evaluated the impact of a community-based epilepsy care program in Mahenge, Tanzania, an onchocerciasis-endemic area with high epilepsy prevalence. METHODS: A baseline survey (2017-2018) identified persons with epilepsy (PWE) in four rural villages. Subsequently, PWE were invited to enroll in the epilepsy treatment program (2019-2022), where trained community health workers (CHWs) screened for epilepsy, promoted ivermectin intake to treat onchocerciasi…

Changes in publicly insured general practitioner consultations before and after workplace injury: interrupted time series by consultation duration and care plans in Australia.

BACKGROUND: General practitioners (GPs) are central to the treatment and management of injured workers. Australian workers' compensation funds GP consultations for work-related injuries. Almost all other GP consultations are subsidised by public healthcare insurance (Medicare Benefit Scheme). This study aimed to examine changes in publicly-insured GP consultations before and after injury for injured workers with long-duration claims, focusing on consultation duration types and GP-provided mental health and chronic diseases management plans. METHODS: This retrospective cohort study using inter

Self-care strategies that support PrEP adherence: a qualitative study with female sex workers in rural Uganda.

The World Health Organization (WHO) recently released revised guidelines on self-care interventions for health and well-being, which, in 2022, included recommendations supporting equitable access to information about and availability of pre-exposure prophylaxis (PrEP) as a self-care strategy. Successful implementation of PrEP as an HIV prevention strategy extends beyond providing access to medication. It hinges on individuals adopting self-care strategies to ensure adherence to PrEP in their daily lives. This paper aims to explore self-care strategies that bolster adherence to oral PrEP among

Awareness and coverage of mass drug administration for elimination of lymphatic filariasis: a community based cross sectional study in Nepal.

Lymphatic filariasis (LF) is among the major public health problems in Nepal. The disease is a major cause of morbidities primarily, lymphedema of legs and hydrocele and it impedes socio economic development in many endemic areas of the country. This study is aimed at exploring the understanding of people about mass drug administration (MDA) of the said disease and the status of compliance of MDA in Nepal. This study is a cross sectional study carried out among 894 household samples in three of the sixty LF endemic districts. The selected districts were Dhading, Kapilvastu and Kailali. The sen

Fetal alcohol syndrome prevention in American Indian communities of Michigan's upper peninsula.

Attitudes and knowledge about Fetal Alcohol Syndrome (FAS) were examined among American Indian communities of Michigan's Upper Peninsula. Indian health workers and community women were interviewed. Education about FAS was provided in each community. The results indicate that information on FAS is reaching many women in these communities and that traditional cultural patterns can support the development of a strong Indian women's health program. At the same time, more must be done in the near term to help those women who are at greatest risk.

Assessing the impact of a community-based psychodrama intervention on mental health promotion of adolescents and young adults in Mozambique: A mixed-methods study.

BACKGROUND: While mental health is an emerging issue in low-income countries, its promotion remains poor, with little context-oriented evidence available. Here we aimed to assess the impact and acceptability/feasibility of a community-based psychodrama intervention involving both adolescents and young adults (AYA) living with HIV (AYALHIV) and without HIV (AYAHIV-). METHODS: We used a mixed-methods approach, where the quantitative part was based on pre/post questionnaires and the qualitative one on content analysis of semi-structured interviews. Trained community health care workers delivered

Harmonization of research and control in schistosomiasis.

I have been employed by several different organizations during over 30 years working on schistosomiasis, the majority spent in endemic areas of the Caribbean, South America, Africa and the Western Pacific. Much of the work is best classified as applied research but sometimes it strayed to the extremes of either public health control programmes or pure research. Over this period, there have been several significant research developments that have altered our whole approach to control. Ideally, research and control should complement each other but, in reality, they sometimes have conflicting obj

Co-designing the 'LEAD' strategy: A theory-informed stakeholder-led behavioural intervention to improve mass drug administration compliance for lymphatic filariasis elimination in an urban block of Odisha, India.

INTRODUCTION: Sustained compliance with Mass Drug Administration (MDA) is essential for interrupting transmission of Lymphatic filariasis (LF). However, behavioural and system-level barriers persist to hinder its uptake. We aimed to explore determinants of MDA participation using behavioural frameworks and to co-design a theory-informed, stakeholder-led intervention to enhance MDA compliance. METHODS: A mixed-methods formative study was conducted between January and June 2025 in an LF-endemic block of Odisha, India. The quantitative component comprised a community-based KAP survey(n = 400) as

Mitigating moral distress by enhancing healthcare workers' understanding of challenges faced by carers of children with disabilities in low-resource settings in Kenya.

BACKGROUND: Little is known about the psychological wellbeing and the potential moral distress faced by female carers of children with disab … l well-being through simple changes, like explaining a child's condition in non-technical language. Community health workers can help strengthen the caregivers' already existing resources by accompanying them in the day-to-day care of their children and by helping them establish self-support groups. Consequently, improved training of healthcare- and community health worke …

Work satisfaction and health in the middle-aged and elderly.

BACKGROUND: There is evidence that occupational factors that cause psychological strain can affect physical and mental health, but findings are not consistent. METHODS: Work satisfaction was used as a convenient global index of job-related psychological strain. Relationships with general, physical and emotional health, appraised by the use of questions, were examined in workers aged > or = 50 years in a longitudinal community health study in Jerusalem. RESULTS: Positive associations were found both in 1975-1976 and in 1985-1987, with age, origin, education, sex and other variables controlled

Changing Perceptions of Rural Frontline Workers and Caregivers About Management of Childhood Diarrhea and Pneumonia Despite Several Inequities: The Nigraan Plus Trial in Pakistan.

PURPOSE: Diarrhea and pneumonia greatly contribute to high childhood mortality in Pakistan. Frontline community health workers or the Lady Health Workers (LHWs) provide care at the doorstep of over 60% of Pakistan's rural residents. Difficult terrain, lack of supplies, and inadequate supervision put these LHWs at an added disadvantage in the timely diagnosis and delivery of known treatment options to community caregivers (CCGs). This study aims to assess whether a supportive supervision intervention through Lady Health Supervisors (LHSs) using enhanced mentorship and written feedback cards hav…

Sign me up: rules of the road for humanitarian volunteers during the Ebola outbreak.

The current Ebola outbreak is the worst global public health emergency of our generation, and our global health care community must and will rise to serve those affected. Aid organizations participating in the Ebola response must carefully plan to carry out their responsibility to ensure the health, safety, and security of their responders. At the same time, individual health care workers and their employers must evaluate the ability of an aid organization to protect its workers in the complex environment of this unheralded Ebola outbreak. We present a minimum set of operational standards deve

Peer-led community HIV/syphilis testing with linkage within seven days for MSM and transgender women in Indonesia: a mixed-methods process evaluation.

We evaluated a peer-led, community-based service (CBS) model providing integrated HIV/syphilis testing and referral for men who have sex with men (MSM) and transgender women (TGW) in South Sulawesi, Indonesia. Trained community health workers (CHWs) delivered finger-prick HIV RDTs and, where available, dual HIV/syphilis tests through three outreach modalities: door-to-door outreach, "static community posts", and mobile hotspot events. Primary outcomes were acceptability, feasibility , first-time testing, syphilis testing coverage, and linkage to confirmatory testing and ART within ≤7 days. Amo…

Barriers and enablers to engagement with green social prescribing in primary care: a scoping review using the socio-ecological model.

BACKGROUND: Green social prescribing (GSP) connects individuals with nature-based interventions (NBIs) through healthcare providers, link workers, and social and community networks. While evidence supports the health benefits of NBIs, further research is needed to understand the factors that influence engagement. AIM: To explore barriers and enablers to GSP and NBIs for prescribers, service users and community providers. DESIGN & SETTING: A scoping review in primary care and community settings. METHOD: We systematically searched Medline, Embase, CINAHL, Web of Science, and SCOPUS from 2019.

Contact Tracing and the COVID-19 Response in Africa: Best Practices, Key Challenges, and Lessons Learned from Nigeria, Rwanda, South Africa, and Uganda.

Most African countries have recorded relatively lower COVID-19 burdens than Western countries. This has been attributed to early and strong … ommunications; leveraging of technology innovations; applied public health expertise; deployment of community health workers; and robust community engagement. Challenges include an overwhelming workload of contact tracing and case detection for healthcare workers, misinformation and stigma, and poorly sustained adherence to isolation and quarantine. Important lessons lear …

Understanding reproductive health challenges during a flood: insights from Belkuchi Upazila, Bangladesh.

Background: Bangladesh is exposed to natural hazards such as floods, cyclones and droughts. As such, its health systems and health infrastructure are exposed to recurrent disasters. Research studying the impacts of natural disasters on reproductive health in particular is lacking. This research contributes to this knowledge gap by studying the challenges related to menstrual regulation and post-abortion care at both the facility and community levels, and the care-seeking patterns of pregnant women during the 2016 flood in Belkuchi, Bangladesh. Methods: Six government-run primary health care fa

The use of learning collaboratives to support integrated mental health and opioid use disorder services: lessons from the HEALing Communities Study.

Mental health concerns (e.g., psychiatric disorders, traumatic experiences, suicidal ideation) are common among those with opioid use disorder (OUD) and are associated with an increased risk of overdose. Yet, few people with OUD receive integrated treatment for psychiatric disorders or other concerns, underscoring the need to reduce barriers to integrated care, such as a lack of knowledge about mental health conditions among frontline workers and providers. In this Community Case Study, we describe an effort at a single site (Massachusetts) of the HEALing Communities Study (HCS) to address bar

The sero-epidemiology of Rift Valley fever in people in the Lake Victoria Basin of western Kenya.

Rift Valley fever virus (RVFV) is a zoonotic arbovirus affecting livestock and people. This study was conducted in western Kenya where RVFV outbreaks have not previously been reported. The aims were to document the seroprevalence and risk factors for RVFV antibodies in a community-based sample from western Kenya and compare this with slaughterhouse workers in the same region who are considered a high-risk group for RVFV exposure. The study was conducted in western Kenya between July 2010 and November 2012. Individuals were recruited from randomly selected homesteads and a census of slaughterho
