RATIONALE AND OBJECTIVES: The medial plantar proper digital nerve, also called Joplin's nerve, arises from the medial plantar nerve, courses along the medial hallux metatarsophalangeal joint, and can be a source of neuropathic pain due to various etiologies, following acute injury including bunion surgery and repetitive microtrauma. We describe our clinical experience with diagnostic ultrasound assessment of Joplin's neuropathy and technique for ultrasound-guided therapeutic intervention including both injection and cryoablation over a 6-year period.

A left ventricular aneurysm (LVA) generally results from myocardial infarction, but rarely LVA can be associated with dilated cardiomyopathy (DCM). We herein report a surgical case of malignant ventricular tachycardia (VT) in a patient with LVA associated with DCM. The present review will discuss the technical aspects of a new patent invented in 1999 which allows cryoballoon ablation, an emerging technology used for the non pharmacological treatment of paroxysmal atrial fibrillation. A general evaluation of the safety and the reliability of the technique will be given together with an overview on its short and midterm outcome. Finally future directions of the technology and its possible implementation will be briefly treated.

We report a case of a 35-year-old male patient who presented with chronic atrial fibrillation secondary to a massive congenital left atrial appendage aneurysm. Minimal invasive endoscopic resection of the left atrial aneurysm with cryoablation was performed. The patient was discharged home in sinus rhythm.

BACKGROUND: We have shown that atrial flutter (AF) in dogs with sterile pericarditis is commonly due to a single-loop reentrant circuit in the lower right atrium comprised of a functional or functional/anatomic obstacle and a slow zone of conduction (SZ) between the central obstacle and the atrioventricular (AV) ring. The goals of the present study were 1) to establish that the epicardial SZ is the critical component of circus movement AF and 2) to identify the optimal site within the epicardial SZ at which interruption of circus movement can be accomplished by ablative techniques.
