Reported are the results of a study of the knowledge, attitudes and practices (KAP) about kala-azar of the inhabitants of two villages (Titaria and Haraincha) situated in terai (plain) areas of Nepal. The villagers had poor knowledge about the transmission of kala-azar, with most villagers perceiving that mosquitos, instead of sandflies, were responsible for transmission of the infection. We describe a case of contact pemphigus, a new subgroup of induced pemphigus. The disease is provoked by a chlorinated hydrocarbon pesticide: dihydrodiphenyltrichlorethane. We suspect that systemic absorption after the topical contact is responsible for the alteration of skin structure and activation of immunological mechanisms leading to blister formation and acantholysis.

The medical literature dealing with hematological disorders following exposure to insecticides (chiefly chlorinated hydrocarbons and organic phosphorus compounds) is briefly reviewed. The development of blood dyscrasias as a consequence of exposure to insecticides is considered unlikely.
