Looking into the future: a machine learning powered prediction model for oocyte return rates after cryopreservation.

RESEARCH QUESTION: Could a predictive model, using data from all US fertility clinics reporting to the Society for Assisted Reproductive Technology, estimate the likelihood of patients using their stored oocytes? DESIGN: Multiple learner algorithms, including penalized regressions, random forests, gradient boosting machine, linear discriminant analysis and bootstrap aggregating decision trees were used. Data were split into training and test datasets. Patient demographics, medical and fertility diagnoses, partner information and geographic locations were analysed. RESULTS: A total of 77,631 …

Development of an algorithm for intraoperative autofluorescence assessment of parathyroid glands in primary hyperparathyroidism using artificial intelligence.

BACKGROUND: Previous work showed that normal and abnormal parathyroid glands exhibit different patterns of autofluorescence, with the former … tween normal and abnormal glands, as well as between different pathologies. Using machine learning, decision trees were created. RESULTS: Normal parathyroid glands were brighter (higher normalized autofluorescence pixel intensity) and more homogenous (lower heterogeneity index) compared to abnormal glands. Optimal cutoffs to differentiate normal from abnormal p …

Computer-aided heuristics in orthodontics.

INTRODUCTION: During the decision-making process, physicians rely on heuristics that consist of simple, useful procedures for solving problems, intuitive shortcuts that produce reliable decisions based on limited information. In clinical situations characterized by a high degree of uncertainty such as those encountered in orthodontics, cognitive biases and judgment errors related to heuristics are not uncommon. This study aimed at promoting trust in the effective interface between the intuitive reasoning of the orthodontic practitioner and the computational heuristics emerging from simple stat

LDH/ADA ratio in pleural fluid for the diagnosis of infectious pleurisy.

Pleural effusion (PE) is a common medical concern, often requiring thoracentesis for a definitive diagnosis. An elevated pleural fluid adenosine deaminase (ADA) may indicate tuberculosis, but this is not always the case. This study aimed to evaluate the accuracy of biomarkers determined in pleural fluid and propose a new diagnostic strategy for PE in patients with high levels of ADA in pleural fluid. This retrospective analysis studied patients with PE who received thoracentesis for the first time with an ADA level of > 33 U/L in the pleural fluid analysis at two tertiary hospitals from March

What treatment for humeral shaft non-union? Case-series assessment of a strategy.

INTRODUCTION: Humeral shaft non-union is frequent, with severe clinical impact. Management, however, is poorly codified and there is no clear decision-tree. HYPOTHESIS: Analyzing our experience over the last 15years could enable a reproducible strategy to be drawn up, with a decision-tree based on the 2 main causes: failure of internal fixation, and infection. MATERIAL AND METHOD: Sixty-one patients were included in a retrospective cohort, with a mean 94 months' follow-p. The treatment strategy was based on screening first for infection then for mechanical stability deficit in case of prior

A new diagnostic algorithm for antibody-mediated microcirculation inflammation in kidney transplants.

We studied the significance of microcirculation inflammation in kidney transplants, including 329 indication biopsies from 251 renal allograft recipients, who were mostly nonpresensitized (crossmatch negative). Glomerulitis (g) and peritubular capillaritis (ptc) were often associated with antibody-mediated rejection (65% and 75%, respectively), but were also found in other diseases in the absence of donor-specific antibody (DSA): T-cell-mediated rejection (ptc, g), glomerulonephritis (g) and acute tubular necrosis (ptc). To develop rules for reducing the nonspecificity of microcirculation infl

Classification of patients on the basis of otoneurological data by using Kohonen networks.

Machine learning methods such as neural networks, decision trees and genetic algorithms can be useful to aid in the classification of patients. We tested Kohonen artificial neural networks, which are known to be effective for classification tasks. Our sample included patients with six different diseases. The Kohonen network algorithm recognized the four largest groups reliably, but the two smallest groups were too small for the method. Neural networks seem to be promising for the computer-aided classification of otoneurological patients provided that the number of patients used is sufficiently…

[Mediators and their antagonists in shock therapy].

The list of shock mediators currently comprises more than 150 candidates. A careful analysis using the criteria of Koch-Dale together with decision trees for exclusion of bias revealed that only histamine, C5a, beta-endorphin, tumor necrosis factor (TNF) thromboxane B2, platelet-activating factor (PAF), and oxygen free radicals are shown to be causally associated with shock symptoms. Although experimental studies with inhibitors of these mediators were convincing, there is still a lack of evidence under clinical conditions (exception histamine: anaphylactic shock). Combinations of antagonists …

Contrast-enhanced ultrasound for diagnosing subtypes of intrahepatic cholangiocarcinoma: a comparative study with poorly differentiated hepatocellular carcinoma.

BACKGROUND: Pathologically, intrahepatic cholangiocarcinoma (ICC) is classified into small-duct (SD) type and large-duct (LD) type, each with distinct clinicopathological characteristics. The contrast-enhanced ultrasound (CEUS) features of the two ICC types remain insufficiently explored. PURPOSE: To evaluate liver CEUS imaging for differentiating the SD and LD types of ICC and further compare them with poorly differentiated hepatocellular carcinoma (pHCC). MATERIALS AND METHODS: A single-center retrospective study enrolled 252 patients with SD-type ICC, LD-type ICC, or pHCC between October

Differential Diagnosis of Parotid Tumors on Ultrasound: Interobserver Variability and Examiner-Specific Decision Rules-A Machine Learning Approach.

Background/Objectives: Noninvasive differentiation of parotid gland tumors remains challenging despite ultrasound being the primary imaging modality for salivary gland lesions. Given its examiner dependence, improving diagnostic consistency and transparency is crucial. We quantified interobserver variability in parotid ultrasound, modeled examiner-specific decision patterns using machine learning surrogates, and tested whether surrogate complexity relates to examiner performance. Methods: In this retrospective, single-center study, six examiners independently rated ultrasound images of 149 par

Set-up of a population-based model to verify alcohol abstinence via monitoring of the direct alcohol marker phosphatidylethanol 16:0/18:1.

BACKGROUND AND AIMS: Phosphatidylethanol 16:0/18:1 (PEth) is a biomarker for alcohol intake. It has a half-life of 7.9 days. Chronic alcohol consumption causes high PEth values. It can take weeks before PEth values fall below the decision limit for 'alcohol abstinence'. Our aim was to validate whether alcohol abstinence can be determined based on two consecutive PEth results above the decision limit. DESIGN: Observational study. SETTING: Belgium, February 2019. The study was linked to a social initiative in Belgium, 'Tournée Minérale'. PARTICIPANTS: Adults (aged > 18 years, n = 796) with va

Decision-Making among Experts in Advanced Hodgkin Lymphoma.

INTRODUCTION: In the treatment of advanced Hodgkin lymphoma (aHL), based on guidelines a multitude of treatment options are available. The availability of PET-guided decision-making and new therapeutic agents increase the complexity of the decision-making process. METHODS: Thirteen experts of Swiss university and cantonal hospitals in Switzerland were asked to describe their institutional decision-making practice in aHL. Variables influencing the decision-making process were identified, standardized, and converted into decision trees for analysis of consent and discrepancies. The algorithms o…

Cost-effectiveness of five strategies for gonorrhea and chlamydia control among female and male emergency department patients.

BACKGROUND: Previous studies have shown screening for gonorrhea and chlamydia to be cost-effective for limiting the sequelae of infection and the associated costs of management. GOAL: To evaluate the cost-effectiveness of enhanced screening for gonorrhea and chlamydia in an emergency department (ED) setting. STUDY DESIGN: Five strategies were compared with use of decision analysis for theoretical cohorts of 10,000 female and 10,000 male ED patients aged 18 years to 31 years: standard ED practice, three enhanced screening strategies, and mass treatment. Main outcome measures were untreated go

Optimization of management plan with a machine learning model for ovarian torsion cases: operative vs. conservative.

BACKGROUND: Ovarian torsion is a gynecologic emergency requiring timely management to preserve ovarian function and future fertility. Clinical presentation is heterogeneous and frequently non-specific, making the decision between operative and conservative management challenging, particularly in reproductive-aged patients. OBJECTIVE: Develop and evaluate a machine learning-based prognostic model to support individualized management decisions in ovarian torsion. METHODS: We conducted a retrospective cohort study of 219 patients diagnosed with ovarian torsion. Demographic characteristics, repr

Machine learning to predict the decision to perform surgery in hepatic echinococcosis.

BACKGROUND: Cystic echinococcosis (CE) is a significant public health issue, primarily affecting the liver. While several management strategies exist, there is a lack of predictive tools to guide surgical decisions for hepatic CE. This study aimed to develop predictive models to support surgical decision-making in hepatic CE, enhancing the precision of patient allocation to surgical or non-surgical management pathways. METHODS: This retrospective analysis included 406 hepatic CE patients treated at our center (2009-2021). Clinical, imaging, and treatment data were used to develop a Cox regres

A Primer on Bayesian Decision Analysis With an Application to a Kidney Transplant Decision.

A clinical decision support system (CDSS) is a computer program, which is designed to assist health care professionals with decision making tasks. A well-developed CDSS weighs the benefits of therapy versus the cost in terms of loss of quality of life and financial loss and recommends the decision that can be expected to provide maximum overall benefit. This article provides an introduction to developing CDSSs using Bayesian networks, such CDSS can help with the often complex decisions involving transplants. First, we review Bayes theorem in the context of medical decision making. Then, we int
