Decompression of peripheral nerve trunks in leprosy to prevent the development and progression of deformities.

BACKGROUND: Peripheral nerve involvement results in deformities in leprosy. High doses (40-60 mg) of steroids along with anti-leprosy drugs is the preferred treatment, even though 70-75% cases still develop deformity. Early surgical decompression of nerves gives better chances of preventing deformity. We have analyzed the role of early surgical decompression in such cases. MATERIALS AND METHODS: Five hundred nerves (386 ulnar, 60 median and 54 posterior tibial) not responding to the medical treatment in 12 weeks, were undertaken for external and internal nerve trunk decompression. These cases

Early Surgical Intervention in Amniotic Band Sequence for Upper Extremity Motor Nerve Palsies: A Case Report.

Urgent surgical intervention for amniotic band sequence (ABS) is currently indicated for concerns of vascular compromise and progressive lymphedema. Peripheral motor nerve palsies are rare, and reports of surgical intervention in these cases describe persistent motor dysfunction. We report band release and ulnar, median, and radial nerve decompression in a 1-week-old with a severe upper extremity constriction band and signs of ulnar nerve motor dysfunction. A literature review on nerve exploration and outcomes of patients with motor nerve palsy from ABS was performed. Early evidence of ulnar m

Spontaneous iliacus haematoma with femoral nerve palsy: an appeal to involve surgical teams early.

We report the case of a 68-year-old man who was placed on heparin as bridge therapy and subsequently developed an iliacus haematoma with associated femoral nerve palsy. His team involved the orthopaedic surgery team in delayed fashion after his symptom onset. Due to his active medical conditions, he did not undergo surgical decompression of his haematoma until late into his hospital course. Unfortunately, this patient did not regain meaningful function from his femoral nerve deficit. We believe this case highlights the high index of suspicion necessary for making this diagnosis as well as the

Balanced orbital decompression for Graves' ophthalmopathy.

OBJECTIVE/HYPOTHESIS: Surgical management of Graves' ophthalmopathy is an alternative to medical therapy with corticosteroids or external beam radiotherapy. Orbital decompression has commonly been performed via a transantral approach to the medial orbital wall and floor. Although an endoscopic approach to these walls has been described, a balanced approach (incorporating a lateral decompression by an ophthalmology team) is desirable. STUDY DESIGN: Retrospective review. METHODS: Endoscopic medial decompression and extended lateral decompression were accomplished in 18 orbits (11 patients); in

Fatal Abdominal Compartment Syndrome Due to Constipation: A Case Report.

INTRODUCTION: Abdominal compartment syndrome (ACS) is a rare condition in which increased intra-abdominal pressure causes multiorgan dysfunction through decreased perfusion. Causes of this condition are variable, and early recognition is critical for favorable patient outcomes. Measurement of bladder pressure is recommended for diagnosis. CASE REPORT: A 64-year-old female on clozapine with a two-year history of chronic constipation presented to the emergency department in extremis with a protuberant abdomen. After resuscitative measures, computed tomography showed a dilated, stool-filled colo

[Orbital decompression for thyroid orbitopathy].

Thyroid orbitopathy may result in significant proptosis that causes visual lost. Besides pharmacologic and radiotherapeutic procedures surgically decompression of the orbital skeleton occasionally is managed. We present series of 3 patients (5 procedures) who underwent orbital decompressions. The preoperative proptosis ranged from 22 to 31mm. Decompression was performed by an external Walsh-Ogura approach in 3 orbits and in 2 by an endoscopic approach. The reduction in proptosis was 4-6mm in external approach cases and 4-5mm in endoscopic procedures. Diplopia was noted in 2 patients preoperati

Combined Use of Intraoperative Indocyanine Green and Dynamic Angiography in Rotational Vertebral Artery Occlusion.

Rotational vertebral artery occlusion (RVAO) is a well-documented surgically amenable cause of vertebrobasilar insufficiency. Traditionally, patients have been imaged using dynamic rotational angiography. We report a case of RVAO in which intraoperative indocyanine green angiography (ICGA) was used to confirm adequate surgical decompression of the VA. A 57-year-old female who presented with multiple episodes of syncope provoked by turning her head to the right. Rotational dynamic angiography revealed a dominant right VA that became occluded at the level of C5/6 with head rotation to the right.

[Surgical treatment of spinal stenosis].

Surgical treatment of lumbar spinal stenosis is aimed at decompressing the structures of the spinal canal. Several surgical techniques have been described over the last few years. This article gives a survey of the surgical procedures used for the treatment of spinal stenosis. When comparing and discussing indications and current surgical techniques used for spinal stenosis, one can describe some general tendencies: if the symptoms are severe or disabling and do not respond to appropriate conservative treatment, or if the patient is not able to cope with the pain any longer and views his or he

Case report: Fully endoscopic microvascular decompression for trigeminal neuralgia.

Microvascular decompression is safe, effective, and micro-invasive. Due to these advantages, it has become the mainstream treatment for trigeminal neuralgia, glossopharyngeal neuralgia, and hemifacial spasm. Initially, microvascular decompression was performed under a microscope, which limited the light source and visualization capabilities. With the development of endoscopic technology, the endoscope has been used in microvascular decompression, which further improved the visualization range and light source properties. The purpose of the present study was to investigate the efficacy of fully

Use of instrumentation in degenerative disease of the cervical spine.

Degenerative diseases of the cervical spine (including cervical spondylotic myelopathy and ossification of the posterior longitudinal ligament) occur predominantly in elderly persons. Decompressive laminectomy has been the standard of surgical treatment in the United States for several decades. Postlaminectomy kyphosis and instability and inadequate decompression of anterior compressing forces can fail to halt and may even contribute to progression of neurologic deterioration. Instrumentation of the cervical spine provides a means of stabilization and allows safer multilevel anterior decompres

Repeated recurrence of thoracic spine stenosis following decompression alone for ossification of the ligamentum flavum: case report.

Ossification of the ligamentum flavum (OLF) is a well-recognized but rare cause of thoracic myelopathy. Its subtle and variable clinical presentation often makes the diagnosis challenging. The treatment of symptomatic OLF requires surgical intervention, with the most common surgical procedure being a posterior decompression with or without instrumentation. Recurrence of ossification and stenosis after surgery is rare and usually occurs at the same intervertebral level. Multiple recurrences of ossification and stenosis are exceptionally rare. The authors report the case of OLF in a 60-year-old

Treating lumbar spinal canal stenosis by injecting insoluble particles.

Lumbar spinal stenosis is a common indication for surgical intervention. Although traditional "open" surgical decompression or minimally invasive decompression are well proven and successful, its inherent approach morbidity cannot be avoided (massive trauma, spondylolisthesis, nerve root adhesion, dural laceration). Many studies on wear-mediated osteolysis have demonstrated that wear debris can initiate the release of cytokines and other inflammatory mediators, which stimulates increased osteoclastic activity and focal bone resorption at the bone-implant interface. Therefore we hypothesize tha

[Transoral decompression of the brain stem in a basilar impression secondary to rheumatoid arthritis. Description of a case and survey of the literature].

INTRODUCTION: The vertical subluxation or translocation of the odontoid process producing a basilar impression with compression of the brain stem is a late phenomenon in the course of rheumatoid arthritis; the appearance of symptoms in the spinal cord and the lower pairs of spinal nerves is a specific sign of this disorder. In this situation, the standard surgical aims are the decompression of the affected nerve structures followed by craniocervical stabilisation. The objective of this paper is to report on the improvement of the neurological deficit after decompressive transoral surgery (odon

Single and multilevel decompression alone in patients with multilevel lumbar spinal stenosis: a systematic review and meta-analysis.

Multilevel lumbar spinal stenosis (LSS) is a prevalent degenerative condition characterized by lower back pain, intermittent claudication, and radicular leg pain. It ranks as one of the primary indications of spinal surgery in patients aged 65 and older. In this study, we aim to compare single-level and multilevel approaches for decompression alone in LSS considering the incidence of complications, reduction in pain score, and rates of surgical revisions. A systematic search was conducted on PubMed (MEDLINE), Embase, Web of Science, and Cochrane Library databases for studies directly comparing
