Health care worker sensitivity to chlorhexidine-based hand hygiene solutions: A cross-sectional survey.

BACKGROUND: Health service hand hygiene programs have seen widespread use of chlorhexidine solutions. Reports of both immediate and delayed hypersensitivity to chlorhexidine are increasing among health care workers. This study examined the prevalence of self-reported symptoms of sensitivity to chlorhexidine solutions among health care workers. METHODS: This study was a cross-sectional online anonymous survey of all workers at a single health service. RESULTS: Of the 1,050 completed responses, 76.3% were female, 35.3% were nurses and midwives, 28% were medical staff, and 8.7% were working in

Contact allergies to cosmetics: testing with 52 cosmetic ingredients and personal products.

Fifty patients of both sexes with clinically suspected cosmetic dermatitis were subjected to patch testing with a cosmetic and fragrance series, approved by the Contact and Occupational Dermatosis Forum of India (CODFI), and with selected allergens from the Indian Standard Series (ISS). Most of these patients were young adults between 10-29 years; the mean age was 27.5 years. The majority of the patients had cosmetic dermatitis of <1 year duration (68%). The occupational profiles of the patients included students (46%), housewives (18%), teachers (10%) and laborers (4%). A miscellaneous group,

Hand dermatitis--differential diagnoses, diagnostics, and treatment options.

The pathogenesis of hand dermatitis is multifactorial, and includes factors such as genetic predisposition and exposure. A high incidence rate is associated with female gender, contact allergy, atopic dermatitis, and wet work. The most important risk factors for the persistence of hand dermatitis include its extent, contact allergic or atopic etiology, childhood dermatitis, and early onset (before the age of 20). The cost of illness of hand dermatitis corresponds to this seen in moderate to severe psoriasis. The diagnostic workup of hand dermatitis and its differential diagnoses requires a det

Occupational skin diseases.

Occupational skin diseases are the most commonly reported notifiable occupational diseases. In Germany, 23 596 out of a total of 71 263 reported occupational diseases in 2010 were classified as occupational skin diseases (BK No. 5101: "severe or recurrent skin diseases which have forced the person to discontinue all occupational activities that caused or could cause the development, worsening, or recurrence of the disease"). Contact dermatitis (allergic, irritant) of the hands is the most common skin disease and atopic skin diathesis is often an important co-factor. The number of work-related

Results of evaluating health care workers with prick and patch testing.

BACKGROUND: Health care workers are exposed to many agents that can cause irritant or allergic contact dermatitis. Recently, much attention has been focused on latex sensitivity, which commonly causes contact urticaria. Most studies have examined the conditions of irritant or allergic contact dermatitis and contact urticaria independently. Therefore, we have little information about the possible occurrence of these conditions in the context of combined assessment including both prick and patch testing. OBJECTIVE: To determine the prevalence of irritant and allergic contact dermatitis and cont

Skin Barrier Dysfunction in Contact and Atopic Dermatitis: A Comparative Review.

The epidermis, the outermost layer of the skin, consists of 5 distinct layers, with the stratum corneum (SC) serving as the primary barrier. Disruption of skin barrier leads to inflammatory skin conditions, including irritant contact dermatitis (ICD), allergic contact dermatitis (ACD), and atopic dermatitis (AD). ICD arises from direct damage to the SC by irritants, while ACD is mediated by a type IV hypersensitivity. AD is characterized by chronic inflammation and genetic defects that compromise barrier integrity. Accordingly, strategies aimed at barrier restoration, such as the use of cerami

Contact sensitivity to systemically administered drugs.

Allergic occupational dermatitis to drugs usually administered systemically is found most often in nurses. Affected less frequently are physicians, veterinarians, dentists, hospital cleaning people, and pharmaceutical manufacturing workers. The drug most often involved in the author's experience is penicillin.

Black rubber sensitization by bicycle handgrips in a child with palmar hyperhidrosis.

Black rubber is mainly an occupational source of contact sensitization; however, several unusual causes of non-occupational black rubber allergy are reported in the literature. A 10-year-old nonatopic Turkish boy with palmar hyperhidrosis developed acute vesicular bilateral palmar dermatitis following 2 to 3 weeks of intensive use of a new bicycle. The handgrips of the bicycle were made of black rubber. Patch testing showed a strong positive reaction to N-isopropyl-N-phenyl-4-phenylenediamine (the main sensitizer in black rubber) and questionable papular reactions to pieces of the black rubber

Acrylate contact allergy: patient characteristics and evaluation of screening allergens.

BACKGROUND: Acrylates are present in a wide variety of products and cause occupational and non-occupational allergic contact dermatitis. There is no clear guidance from the literature as to which allergens should be used for patch-test screening for acrylates. OBJECTIVES: To characterize patients with contact allergy to acrylates and to evaluate the allergens used to screen for acrylate allergy. METHODS: Charts of patients visiting an outpatient contact dermatitis clinic from January 1998 to February 2008 were reviewed retrospectively. RESULTS: Forty-four patients were found to have contact

Prevalence and cause of methylisothiazolinone contact allergy.

BACKGROUND: Methylchloroisothiazolinone/methylisothiazolinone (MCI/MI) has been one of the most frequent sensitizers since the 1980s. In 2005, the use of MI alone was approved for the preservation of cosmetic and household products in the EU. Before that, MI was used in industrial products, and the first cases of isolated MI contact allergy were published. OBJECTIVES: To present the prevalence and causes of MI contact allergy. MATERIALS AND METHODS: Patch test results from 2536 dermatitis patients tested with MI at Gentofte University Hospital between May 2006 and February 2010 were analysed

Cutaneous sensitization to aziridine preceding the onset of occupational asthma.

We describe a 47-year-old non-atopic woman, working as a spray painter in a tannery for 23 years, with a 16-year history of cutaneous symptoms and a subsequent 2-year history of asthmatic symptoms after exposure to aerosol and vapour of polyfunctional aziridine (PFA) at work. To confirm the occupational origin of the dermatitis and asthma we performed a skin prick test with PFA and a specific inhalation challenge (SIC) with PFA. Prick test with PFA elicited an immediate positive skin reaction. She developed an immediate asthmatic reaction upon SIC with PFA. The onset of occupational dermatitis

Erythema multiforme like reaction to santos rosewood (Machaerium scleroxylon) and Torreya nucifera.

Erythema multiforme is a skin disorder characterised by target epithelial eruption, which is mainly caused by infection or drugs. In this case, we report an erythema multiforme like reaction caused by contact dermatitis against wood, especially santos rosewood. During the hospitalisation, we performed a patch test with lumber used in the patient's workplace, and recognised a positive response to multiple woods and a simultaneous recurring eruption (flare up) outside of the test site. The findings from this case of contact dermatitis caused by frequently used industrial wood type is important f

Irritant contact dermatitis from a black henna tattoo without sensitization to para-phenylendiamine.

Allergic contact dermatitis from nonpermanent black henna tattoos has been frequently reported, particularly in children. Contamination or adulteration of the dyes with para-phenylendiamine has been identified as major cause of active sensitization and elicitation of severe allergic contact dermatitis. Sequelae include permanent sensitization, hyper- or hypopigmentation, scarring, keloids, and hypertrichosis. We report a rare case of irritant dermatitis to an unknown ingredient in a black henna tattoo with consecutive hypopigmentation. Sensitization to para-phenylendiamine and other para-compo

Hand dermatitis: review of etiology, diagnosis, and treatment.

Hand dermatitis is a common condition seen in the primary care setting. Occupational exposures and frequent hand washing often lead to symptoms that are irritating and may cause discomfort. Irritant dermatitis, atopic hand dermatitis and contact hand dermatitis account for at least 70% of all diagnoses. A unifying feature in most cases is an underlying disruption in the stratum corneum, altering its barrier function. Transepidermal water loss increases with barrier disruption and is exacerbated by additional exposure to water. Precise diagnosis and subsequent treatment present a considerable c

Nickel skin levels in different occupations and an estimate of the threshold for reacting to a single open application of nickel in nickel-allergic subjects.

BACKGROUND: Nickel is a frequent allergen throughout the world. However, the extent to which nickel is relevant as an occupational contact allergen as opposed to being simply a reflection of jewellery exposure has been unclear. Some thresholds for cutaneous nickel exposure to induce a dermatitis reaction in nickel-allergic individuals have been defined. Over recent years it has become possible to measure accurately the quantity of nickel on the skin of individuals in a number of occupations. OBJECTIVES: To measure the quantities of nickel on the skin of the fingers in workers employed in occu

Chromate-Induced Allergic Contact Dermatitis Treated With Dupilumab.

Chromate causes persistent, difficult to treat irritant and allergic contact dermatitis in cement-handling occupational workers. When therapeutics such as topical corticosteroids, topical calcineurin inhibitors, phototherapy and immune-modulating treatments like methotrexate fail, many patients are advised that avoidance may be the only remaining option – an option that may be particularly challenging if the patient’s occupation necessitates chromate exposure. We report a case of severe chromate-induced allergic contact dermatitis in a 55-year-old cement mason that presented to the

Risk factors for contact allergy to nickel - results of a multifactorial analysis.

To quantify the independent impact of potential risk factors for nickel contact allergy (NCA), a multifactorial Poisson regression analysis of standardized anamnestic and patch test data (with nickel sulfate, 5% in petrolatum) was performed, comprising 74 940 patients assessed in the 33 German and Austrian contact dermatitis units of the Information Network of Departments of Dermatology (IVDK) between 1992 and 2000. NCA was observed in 15.5% patients. Female sex was the strongest risk factor (prevalence ratio 3.74, 95% CI: 3.51-3.98). Risk increased monotonically and significantly with decreas

Hand/face/neck localized pattern: sticky problems--resins.

Plastic resin systems have an increasingly diverse array of applications but also induce health hazards, the most common of which are allergic and irritant contact dermatitis. Contact urticaria, pigmentary changes, and photoallergic contact dermatitis may occasionally occur. Other health effects, especially respiratory and neurologic signs and symptoms, have also been reported. These resin systems include epoxies, the most frequent synthetic resin systems to cause contact dermatitis, (meth)acrylics, polyurethanes, phenol-formaldehydes, polyesters, amino resins (melamine-formaldehydes, urea-for
