Fast-dissolving microneedles with tip-loaded ovalbumin for transcutaneous allergen-specific immunotherapy.

The popularization of allergen-specific immunotherapy (AIT) is impeded by the frequent administration and extended periods with high risk of adverse reactions. Considering mass antigen-presenting cells in epidermal and dermal regions, we propose a transcutaneous AIT strategy for desensitization and protection against anaphylaxis. Structurally, a hyaluronic acid (HA)-based fast-dissolving microneedle array is prepared by two-step casting method, in which the needle tips are mixed encapsulated with ovalbumin allergen (OVA-MN). The composition of polymer-additive complexes is optimized to balance

Patient sensitization profile and fish species' tolerance in children with IgE-mediated fish allergy.

BACKGROUND: Fish allergy remains a poorly investigated topic despite its potential severity and impact on child well-being. There is a paucity of data on IgE fish allergy in pediatric patients in the Mediterranean region. METHODS: 52 children with IgE-mediated fish allergy underwent OFCs with different fish species. The adopted OFC protocols were according to PRACTALL Consensus. Nonparametric quantitative variables were compared between two groups using the Mann-Whitney U test, and the data were visualised using box plots. Fisher's exact test was used to assess the significance between dichot

Generalized Bullous Fixed Drug Eruption due to Clopidogrel Hypersensitivity Successfully Treated Using the Jefferson Procedure.

Generalized bullous fixed drug eruption (GBFDE) is a rare variant of fixed drug eruption that is known to clinically mimic Stevens-Johnson syndrome (SJS) or toxic epidermal necrolysis (TEN). Numerous medications have been reported to cause GBFDE, including antiplatelet agents like ticagrelor, but few reports exist linking this reaction to clopidogrel specifically. We report a case of a medically complex 80-year-old male who was found to have clopidogrel hypersensitivity manifesting as GBFDE based on clinical characteristics and pathology. He was successfully treated using the Jefferson desensi

Local allergy in the atopic march: new insights into adenotonsillar hypertrophy.

Adenoid hypertrophy (AH) and combined adenotonsillar hypertrophy (ATH) are primary causes of childhood sleep-disordered breathing (SDB), and they are strongly correlated with atopic diseases affecting the respiratory mucosa. Allergen sensitization, class-switching of B cells, and IgE production in the adenotonsillar tissue, namely local atopy, are crucial steps in the pathogenesis of allergic rhinitis (AR) and asthma. The adenotonsillar tissue is also responsible for a considerable part in circulating specific IgE, potentially contributing to the pathogenesis of atopy in other organs. Atopic c

Cockroach immunotherapy modulates dominant T-cell responses independent of allergen extract content.

BACKGROUND: T-cell responses to the individual components of allergen extracts have not been fully elucidated in subcutaneous allergen immunotherapy (SCIT). Specifically, it is unknown whether T-cell responses to immunodominant allergens are more or less sensitive to modulation, and whether allergen abundance in the immunotherapy extract influences T-cell response modulation. OBJECTIVE: To fill these gaps, we evaluated CD4+ T-cell reactivity specific to each of the main cockroach allergens in the double-blinded, placebo controlled, multicenter CRITICAL (NCT03541187) SCIT trial. METHODS: Part

Successful desensitization with FVIII/Von Willebrand Factor concentrate in Type III Von Willebrand Disease.

Von Willebrand Disease (VWD) Type 3 is a rare and severe bleeding disorder characterized by an almost complete deficiency of Von Willebrand Factor (VWF). Plasma-derived Factor VIII (FVIII)/VWF concentrates are used both on demand and for prophylactic treatment. However, allergic reactions to these products pose significant challenges in clinical management. A 40-year-old female patient with VWD Type 3 presented to our clinic with symptoms including jaw numbness and tightness, chills, fatigue, nausea, and dyspnea following administration of a FVIII/VWF concentrate (Haemate® P). Skin prick tests

Improved quality control of allergen products: Assessing the molecular allergen composition by mass spectrometry.

BACKGROUND: Natural allergen sources contain a variety of allergens, against which allergic subjects have developed individual sensitization profiles. Ideal allergen products for skin prick testing (SPT) and allergen immunotherapy (AIT) should contain the complete set of allergens of the respective allergen sources to cover all sensitization profiles. However, commercially available allergen products were shown to vary regarding their allergen composition. METHODS: The qualitative allergen composition of different SPT and AIT products produced from pollen of grasses, birch, mugwort and from h

New evidence in food allergies treatment.

PURPOSE OF REVIEW: To acknowledge, the newly available treatments for food allergy described in the latest scientific literature, such as oral immunotherapy (OIT), biologics and the combination of them in managing patients with IgE-mediated food allergies. RECENT FINDINGS: Recent studies suggest that OIT and biologics, alone or together, can have a role as disease-modifying treatments for food allergies. The FDA has recently approved omalizumab as a treatment for food allergy. Other biologics are currently under evaluation and further studies are needed to assess the efficacy and safety of th

Successful Long-Term Aspirin Desensitization in a Pilot With Aspirin-Exacerbated Respiratory Disease.

Aspirin-exacerbated respiratory disease (AERD) is a triad of asthma, chronic rhinosinusitis with nasal polyposis (CRSwNP), and adverse respiratory reactions to the ingestion of aspirin/non-steroidal anti-inflammatory drugs.1 Patients with AERD are frequently plagued with CRSwNP that is difficult to manage with systemic steroids, nasal steroids, and surgical polypectomy, often requiring multiple endoscopic sinus surgeries and frequent otolaryngology follow-up.2,3 There are an abundance of therapies to treat CRSwNP in the setting of AERD, all with varying costs, efficacies, and indications for t

Desensitization to trimethoprim-sulfamethoxazole in a toxoplasmic encephalitis patient who was intolerant to conventional treatments.

Toxoplasma gondii is an obligate intracellular protozoan that causes toxoplasmic encephalitis (TE) in immunocompromised patients. We describe a case of a 29-year-old Japanese man presenting with headache and vomiting. He had previously been diagnosed with human immunodeficiency virus infection. Magnetic resonance imaging identified some nodules in his brain. We suspected TE and began treatment successively with parenteral trimethoprim-sulfamethoxazole (TMP/SMX) plus clindamycin. After that, we switched to pyrimethamine plus sulfadiazine (PMT/SDZ) because these drugs are the first-line treatmen

Re-sensitization of desensitized histamine H1 receptors in the human skin takes more than 18 hours.

BACKGROUND: It is known since the time of testing histamine on pieces of guinea pig's jejunum that histamine receptors can develop insensitivity. The aim was to find evidence for desensitization of histamine H1 receptors in the human skin in vivo and, if found, to study the time for such receptors to regain normal sensitivity. MATERIALS AND METHODS: A skin prick test with histamine (10 mg mL-1 ) was set in areas where a large histamine wheal was evoked 2, 6, 18, 24 or 72 hours earlier. A skin prick test with histamine (10 mg mL-1 ) was also set in an area where an allergen wheal was evoked 2

Pollen-Food Allergy Syndrome: A not so Rare Disease in Childhood.

Seasonal allergic rhinoconjunctivitis (SAR) affects millions of people worldwide, particularly in childhood and adolescence. Pollen food allergy syndrome (PFAS) is a common adverse reaction occurring few minutes after the consumption of vegetable foods in patients with pollen-induced SAR. PFAS has rarely been investigated in the pediatric population, as it has been mainly examined as an adult disease. Recent studies suggested that PFAS might be more frequent in childhood than previously recognized. The present review aims to give an overview of the epidemiology, pathophysiology, diagnosis, man

Hymenoptera Venom Allergy: How Does Venom Immunotherapy Prevent Anaphylaxis From Bee and Wasp Stings?

Hymenoptera stings may cause both local and systemic allergic reactions and even life threatening anaphylaxis. Along with pharmaceutical drugs and foods, hymenoptera venom is one of the most common causes of anaphylaxis in humans. To date, no parameter has been identified that may predict which sensitized people will have a future systemic sting reaction (SSR), however some risk factors, such as mastocytosis and age >40 years are known. Venom immunotherapy (VIT) is the most effective method of treatment for people who had SSR, which is shown to be effective even after discontinuation of the th
