A patient with respiratory muscle weakness due to alveolar hypoventilation was treated with nocturnal bilateral phrenic nerve pacing for one year. Treatment was associated with a progressive increase in diaphragmatic strength and endurance.

Phrenic nerve palsy (PNP) is seen in infants and young children usually resulting from operative trauma or birth injury. Spontaneous recover … developed recurrent respiratory distress 4 months postoperatively; he had a recurrent elevated hemidiaphragm requiring a second plication.(ABSTRACT TRUNCATED AT 250 WORDS)

Herniation through a "cribriform" diaphragm was incidentally detected in a 65-year-old woman. Liver tissue protruding into the right hemithorax showed signs of strangulation. The defects were surgically corrected. The condition presumably was an extremely rare variant of congenital diaphragmatic hernia.

Repair of a congenital posterolateral diaphragmatic defect (CPLDD) by direct suture results in a flat drum-head diaphragm, which has very little function. Other harmful effects of such a repair include enlargement of the thoracic cavity, resulting in more overexpansion of the hypoplastic lungs which may contribute to pulmonary vascular obstruction, and decrease in volume of the abdominal cavity, making closure of the laparotomy difficult. Reconstruction of a diaphragmatic dome would result in a more functional diaphragm and would prevent the above complications. This was substantiated in a ser…

The authors study 21 cases of traumatic rupture of the diaphragm. All were due to road accidents and were associated with multiple injuries. They emphasise the importance of clinical and radiological examination and the often negative results of peritoneal dialysis. Assessment of the lesions should be carefully carried out by a medical and surgical team familiar with these problems. The mortality remains high in spite of rapid and early surgical treatment.

The heart and diaphragm efficiency seems to be an actively regulated variable. It alters in prolonged adaptation (raises in adaptation to hypoxia; diminishes in cold adaptation which means an increase in heat production per unit of muscle's contractile activity). Enhancement of noradrenaline calorigenic effect on skeletal muscles during cold adaptation is corroborated in an isolated diaphragm. Diminishing of the heart and diaphragm efficiency in thyrotoxicosis suggests participation of thyroid hormones in formation of adaptive decrease of muscular contraction efficiency in cold adaptation. The…

Gastric outlet obstruction in infants and children may be due to a partial, prepyloric antral diaphragm. Twelve new patients are added to the 32 described previously. Onset of symptoms varied from shortly after birth to five years. Nonbilious vomiting was the most common presenting symptom. Radiographic evaluation requires specific technique for demonstration of the web and to differentiate this from pylorospasm and pyloric stenosis. Gastroscopy was employed in three patients. Repair usually consisted of incision of the web and construction of a patulous gastric outlet. All patients remained a…

This is a report of 11 cases of traumatic rupture of the diaphragm, 8 left, 2 right and 1 bilateral. In 10 cases the rupture occurred in multiple trauma victims of a violent accident. In one exceptional case a right rupture occurred after a minor trauma; it was followed by herniation of the right colon and half of tranverse, the liver remaining in the abdomen. The mechanisms and types of ruptures are analyzed. Early diagnosis is possible if one is conscious of the clinical signs in all cases of thoracic and abdominal trauma, and when correct and repeated roentgenograms are made of the thorax. …

Diaphragm plication is an effective treatment for diaphragmatic paralysis, in which the affected diaphragm is sutured in place. Robotic-assisted thoracoscopic (RATS) diaphragm plication combines the advantages of standard open thoracotomy and conventional thoracoscopic technique, gaining popularity as a less invasive alternative to improve the overall lung function. We present a case, to our knowledge, of the first described instance of abdominal compartment syndrome in the literature following RATS diaphragmatic plication. Abdominal compartment syndrome potentially developed as a consequence …

Lumbar hernias are uncommon defects of the posterolateral abdominal wall and may be difficult to distinguish from other thoracoabdominal abn … t posterolateral thoracoabdominal region, which was concerning for a closed-loop obstruction from a diaphragmatic hernia. Emergent exploratory laparotomy instead demonstrated an intact diaphragm, with a 5-cm defect in the right superior posterolateral abdominal wall immediately inferior to the twelfth rib containing the incarcerated small bowel. The finding …

Lung-protective ventilation is the current standard for mechanical ventilation of patients with acute respiratory distress syndrome (ARDS). Traditionally, this approach has focused on the controlled phase of mechanical ventilation, but emerging data suggest that how patients are managed during assisted ventilation may also impact clinical outcomes. Experimental and observational clinical data indicate that excessive respiratory effort may further damage already injured lungs and may also lead to diaphragm myotrauma. Conversely, insufficient effort and prolonged passive ventilation are associat…

Early screening for respiratory sarcopenia lacks sensitive tools. We evaluated whether diaphragm ultrasound parameters can independently predict respiratory and overall muscle strength in healthy adults. In this cross-sectional study, 149 healthy adults underwent handgrip strength (HGS), maximal inspiratory/expiratory pressures (MIP/MEP), and right ("R-") and left ("L-") diaphragm ultrasound. Diaphragm ultrasound included thickness at functional residual capacity (ThFRC) and total lung capacity (ThTLC), diaphragm thickening ratio (DTR), excursion (EFRC and ETLC), and dynamic indices during max…

BACKGROUND: Positive pressure ventilation has multiple adverse cardiopulmonary effects leading to impaired lung mechanics and hemodynamic performance. Controlled levels of diaphragm activity may mitigate these. The objectives of the study were to assess the effect of varying levels of diaphragm neurostimulation on respiratory mechanics and hemodynamics during positive pressure ventilation and to establish whether these effects depend on the level of applied positive end-expiratory pressure (PEEP). METHODS: Patients enrolled in a clinical trial of diaphragm neurostimulation underwent a nested r…
