Utility of Exogenous Ketones and Modified Atkins Diet in a Child With GSD VII (Tarui Disease).

Glycogen storage disease VII (GSD VII), also known as Tarui disease, is caused by a deficiency of the skeletal muscle enzyme, phosphofructokinase (PFK). Classical GSD VII is characterised by reduced exercise capacity and exercise-induced myopathic symptoms, exacerbated with carbohydrate intake. No recommended medical or dietary therapy currently exists to improve exercise symptomology in GSD VII. An 11-year-old female was recommended four trial dietary therapies for 8 weeks each: (1) usual diet, (2) 150 mg/kg exogenous ketones pre-exercise, (3) modified Atkins diet (MAD), (4) MAD and pre-exerc

Diet-related poor nutritional status as a major challenge in the treatment of patients with amyloidosis: A systematic review.

INTRODUCTION: Amyloidoses are systemic diseases associated with organ function deterioration and severe management challenges. The most common types of systemic amyloidosis are AL (immunoglobulin light chain) and ATTR (transthyretin related) amyloidosis, which are increasingly diagnosed, and cause serious health problems and lead to significant present-substantial nutritional challenges. AIM: The present systematic review aimed to collect existing data on dietary intake and the dietary-related problems emerging in patients with AL or ATTR amyloidosis. DATA SOURCES: The review was conducted f

Low-carbohydrate ketogenic diet in Mc Ardle's disease: a single-blinded randomized controlled trial.

Glycogenosis type 5 (GSD5, Mc Ardle's disease), the most common muscle glycogen storage disease characterized by exercise intolerance, recurrent myoglobinuria and possible myopathic evolution, is still missing and effective treatment. A dietetic approach to circumvent the metabolic failure in GSD5 seems attractive and has been attempted in various forms but with mixed results. We ran a single-blind randomized controlled trial comparing 6 months of individualized 3:1 low-carbohydrate ketogenic diet (LCKD) to diet as usual in 21 adult patients with GSD5. Primary endpoints were safety and tolerab

Clinical and Developmental Outcomes After 50 Years of Newborn Bloodspot Screening for Classical Galactosaemia in the Republic of Ireland.

Classical Galactosaemia (CG) is an inborn error of carbohydrate metabolism. In untreated neonates, CG leads to a multi-organ toxicity with life-threatening symptoms. Newborn Screening for CG began in the Republic of Ireland in 1972. In Ireland, two forms of neonatal screening occur. High-risk infants are fed lactose-free/galactose-free formula until the result of their Beutler screening test on day 1. All other infants are fed as per parental preference and are screened on day three to five. While immediate or early implementation of a strict lactose-free diet together with medical interventio

The impact of low-energy total diet replacement with behavioural support for remission of type 2 diabetes on disordered eating (ARIADNE): Protocol for a non-inferiority randomised controlled trial.

INTRODUCTION: The National Health Service (NHS) in England is currently piloting a weight loss programme for remission of newly diagnosed type 2 diabetes (T2D), where participants replace all food with low-energy nutritionally complete formula products for 12 weeks (total diet replacement, TDR) and receive behavioural support. In a clinical trial, this programme led to remission in nearly half the participants. However, this weight loss programme might also worsen disordered eating and prompt eating disorders in susceptible people. We aim to investigate if the TDR programme is non-inferior to

A systematic review examining the impact of blended diets on the gastrointestinal symptoms of people who are enterally fed.

BACKGROUND: Adults and children who are enterally tube-fed can experience adverse gastrointestinal symptoms (GIS). Observational data suggests that blended diets (BD) could mitigate such symptoms, with potential to improve quality of life and clinical outcomes. We present a novel systematic review examining the impact of BD, compared to commercial feeds, on GIS of adults and children who are tube-fed. METHODS: The protocol was registered with PROSPERO (CRD42021261147). Four electronic databases were searched from inception to August 2021. Search terms included 'tube-fed', 'blended diet' and '

Infectious and digestive complications in glycogen storage disease type Ib: Study of a French cohort.

Glycogenosis type Ib (GSD1B) causes not only hypoglycemia but also infections and "Crohn's disease like" inflammatory bowel disease (IBD) that can significantly impair patient's quality of life. We retrospectively evaluated infectious and digestive complications in 9 French patients (3 girls, 6 boys) diagnosed at 0.8 years on average, with a mean follow-up of 19.1 years. Infections occurred earlier than IBD, at mean ages of 1.7 and 3.8 years, respectively. The number of acute hospitalizations was 0.7/year due to infectious (0.4/year) or digestive symptoms (0.4/year). Clinical presentations all

Inadequate energy and protein intake in geriatric outpatients with mobility problems.

To individualize nutritional interventions for the prevention and treatment of malnutrition and sarcopenia, it is required to understand the nutritional needs of older adults. This study explores the nutritional needs of geriatric outpatients. We hypothesized that inadequate energy and protein intake is common in geriatric outpatients. Data were retrieved from 2 cohort studies encompassing community-dwelling older adults referred to geriatric outpatient mobility clinics in Amsterdam, The Netherlands and Melbourne, Australia. Indirect calorimetry and a food diary, respectively, were used to ass

Nutritional management of lactose intolerance: the importance of diet and food labelling.

Worldwide, 70% of the adult population has limited expression of lactase enzyme with a wide variation among different regions and countries. Lactase deficiency may lead to lactose intolerance (LI). Depending both on the amount of lactose ingested and on the lactase activity, people who suffer from lactose malabsorption might experience numerous gastrointestinal and extra-intestinal symptoms and manifestations. Treatment of LI mainly consists of reducing or eliminating lactose from the diet until the symptoms disappear as well as supplementing lactase, and inducing colon microbiome adaptation b

Triple Adaptation of the Mediterranean Diet: Design of A Meal Plan for Older People with Oropharyngeal Dysphagia Based on Home Cooking.

BACKGROUND: Oropharyngeal dysphagia (OD) and malnutrition are highly prevalent in older patients that are discharged from general hospitals (47% and 30%, respectively). AIMS: To develop a nutritional plan for these patients involving a triple adaptation of their traditional diet: (a) rheological adaptation (texture and viscosity) for safe deglutition, (b) nutritional adaptation (water, calories, and proteins), and (c) organoleptic adaptation to improve compliance. METHODS: Two fluid viscosities (250 and 800 mPa·s) were selected according to previous studies on optimal viscosities in older pa

British Society of Gastroenterology consensus guidelines on the management of inflammatory bowel disease in adults.

Ulcerative colitis and Crohn's disease are the principal forms of inflammatory bowel disease. Both represent chronic inflammation of the gastrointestinal tract, which displays heterogeneity in inflammatory and symptomatic burden between patients and within individuals over time. Optimal management relies on understanding and tailoring evidence-based interventions by clinicians in partnership with patients. This guideline for management of inflammatory bowel disease in adults over 16 years of age was developed by Stakeholders representing UK physicians (British Society of Gastroenterology), sur

[Eduard Reich's "Hygiene of Culture"-A retrospective on the occasion of the 100th anniversary of his death].

In contrary to the developing significance of the bacteriology and the chemical and physical noxious influences of the environment for Eduard Reich the main cause of decision between health and illness are the functions of the "soul". Components of this as important for the normal life are virtue (i. e. solidarity, performance of duty, self-knowledge), good sense to realize correlations, morality, charity and mercifulness (social behaviour). The goal of the hygiene therefore is to promote the good and to hinder the evil, as well for a complete person or only one of his organs as for the whole

Caloric restriction attenuates aging-induced cardiac insulin resistance in male Wistar rats through activation of PI3K/Akt pathway.

BACKGROUND AND AIM: Caloric restriction (CR) improves insulin sensitivity and is one of the dietetic strategies most commonly used to enlarge life and to prevent aging-induced cardiovascular alterations. The aim of this study was to analyze the possible beneficial effects of caloric restriction (CR) preventing the aging-induced insulin resistance in the heart of male Wistar rats. METHODS AND RESULTS: Three experimental groups were used: 3 months old rats (3m), 24 months old rats (24m) and 24 months old rats subjected to 20% CR during their three last months of life (24m-CR). After sacrifice h

Energy and protein intake in 330 geriatric orthopaedic patients: Are the current nutrition guidelines applicable?

BACKGROUND AND AIMS: Elderly orthopaedic patients are potentially at a high nutritional risk. The reasons for this are numerous as i.e. reduced appetite, sensation of satiety after eating small amounts of food, multi-medication or immobility. This is in contrast to the increased energy and protein recommendations for geriatric orthopaedic patients. METHODS: Oral intake during hospitalization of more than 1000 geriatric orthopaedic patients aged over 80, with or without fracture, was recorded, calculated and then compared to energy and protein requirements by clinical dietitians according to i

Impact of a nutrition/educational program in a group of type 2 diabetes patients, already involved in a wider "Group Care" plan and not achieving complete target: an observational study.

Type 2 diabetes is one of the most frequent chronic degenerative diseases in the world (5.4% in Italy); it has many chronic consequences with a significant impact either on expectation or quality of life. About 80-90% of type 2 diabetes patients is either overweight or obese, condition that can cause many more negative consequences than in individuals in the same conditions but with normal weight. The aim of the research is to assess if diabetic people who cannot reach the goals of glycemic control, good eating habits and weight loss can eventually obtain, with an individualized care plan, lon
