BACKGROUND: Current surveillance recommendations after thoracic endovascular aortic repair (TEVAR) include contrast-enhanced computed tomography (CT) scans at 1-, 2-, 6-, and 12-month intervals, with annual scans thereafter. Patient compliance with such schedules remains inconsistent for all etiologies of aortic disease. It remains unclear which boundaries prevent compliance and whether compliance is associated with improved aorta-specific outcomes.
METHODS: A retrospective analysis was performed of a multicenter health-care system's electronic medical records to include all patients who unde

Infectious complications in patients with multiple myeloma remain the main cause of mortality because of disease-related immunodeficiency. A mycotic aortic aneurysm caused by Burkhoderia cepacia, which has been recognized as nosocomial pathogen in immunocompromised populations, is very rare and only few cases have been reported in the literature. We describe an unusual case of a ruptured mycotic aneurysm of the descending thoracic aorta with a DeBakey IIIb aortic dissection caused by Burkhoderia cepacia in a patient with active multiple myeloma during chemotherapy with anti-myeloma agents. A 60-year-old man was admitted to our hospital with chest and back pain. Electrocardiogram, echocardiography, and contrast-enhanced computed tomography (CT) confirmed a Stanford type A acute aortic dissection with right ventriclar infarction and left ventricular inferior wall asynergy due to right coronary artery malperfusion. The patient presented with shock vital signs. So, immediately percutaneous coronary intervention (PCI) was performed to obtain the right coronary revascularization, after which total arch replacement and frozen elefant trunk was performed. Postoperatively, the patient re

Aortic dissection is a life-threatening pathology characterized by the progressive delamination of adjacent lamellar units within the aortic media. Because this internal damage propagates predominantly along the radial direction of the arterial wall, radial tensile testing has emerged as a particularly relevant experimental configuration to reproduce the mechanical conditions associated with dissection. Several experimental studies have reported the mechanical response of arterial tissues under radial tension, highlighting pronounced viscoelasticity, fluid-driven effects, and progressive damag

Aortic dissection often initiates a few centimeters distal to the coronary ostia in the right lateral wall, with an intimal-medial tear that tends to be transversely directed and occupy half of the aortic circumference, sometimes less, but seldom the entire circumference. To elucidate these clinical observations, tear tests were presently used to determine the layer-specific resistance to tear propagation in ascending thoracic aortic aneurysms, assessing variations over the four circumferential quadrants and two directions. Aneurysmal tissue strips of standardized dimensions from sixteen patie

34 patients with aneurysms of the ascending and descending thoracic aorta are analyzed with regard to treatment (surgical, n = 23, or conservative, n = 11) and survival. In 17 patients the aneurysm was true and in 17 dissecting (acute in 3, chronic in 14). Mean age, sex, symptoms, main clinical data and the relation between true and dissecting aneurysm were comparable in both groups. Conservative treatment was commenced either due to contraindications (4 patients) or in the absence of an immediate life-threatening situation ( n = 7). At the end of 1977 (mean survival time 27 months), 5 of 11 p

BACKGROUND: Thoracic aortopathy associates with extracellular matrix remodeling and altered biomechanical properties. We sought to quantify the natural history of thoracic aortopathy in a common mouse model and to correlate measures of wall remodeling such as aortic dilatation or localized mural defects with evolving microstructural composition and biomechanical properties of the wall.
METHODS: We combined a high-resolution multimodality imaging approach (panoramic digital image correlation and optical coherence tomography) with histopathologic examinations and biaxial mechanical testing to c

Ruptured chronic type B aortic dissection (TBAD) is a life-threatening emergency requiring prompt intervention. A 65-year-old woman with chronic TBAD presented with severe chest and epigastric pain. Computed tomography showed a ruptured false lumen (FL) at the proximal descending thoracic aorta. Emergency thoracic endovascular aortic repair sealed the primary entry tear, but haemodynamics instability owing to retrograde FL perfusion required urgent FL occlusion using a custom-made Candy Plug. She recovered with postoperative management including delayed hemothorax drainage. This case highlight

We report the case of a 69-year-old man with chronic type B aortic dissection and progressive aneurysmal dilatation. Zone 2 thoracic endovascular aortic repair was performed with carotid-subclavian bypass. Nonobstructive general angioscopy identified additional tiny entry tears in the descending aorta, guiding adequate stent graft coverage. Subsequent left subclavian artery coil embolization under balloon occlusion revealed satisfactory packing on fluoroscopy, but angioscopy revealed residual flow through coil gaps, prompting additional coil placement. Blood flow cessation was confirmed by ang

A 68-year-old male with sudden, persistent chest and back pain was referred to our hospital. Contrast-enhanced computed tomography (CT) scans revealed 56 mm thoracic aortic aneurysm (TAA)located from the distal aortic arch to the proximal descending thoracic aorta and Stanford type B acute aortic dissection (AAD) with the patent false lumen distal to the aneurysm. The AAD was not extended within the TAA. Although anti-hypertensive therapy was initiated, the patient unfortunately deceased of sudden cardiopulmonary arrest 6 days later. In the postmortem CT scans, ruptured TAA was diagnosed. Rupt

The endovascular management of blunt aortic injuries is being used more frequently in the trauma patient. Traumatic aortic injuries usually occur in the descending thoracic aorta near the origin of the left subclavian artery. Many reports in the literature demonstrate the efficacy of endovascular repair of blunt thoracic aortic injury. We report here an unusual case of abdominal aortic dissection secondary to blunt abdominal trauma following a fall. The patient also had associated intra-abdominal injuries requiring bowel resection and repair of small bowel mesenteric lacerations. He was treate

Heritable thoracic aortic disease (HTAD) is a term used to define a large group of disorders characterized by the occurrence of aortic events, mainly represented by aneurysm or dissection. These events generally involve the ascending aorta, although the involvement of other districts of the aorta or peripheral vessels may occur. HTAD can be classified as non-syndromic if the disorder is limited to the aorta, and syndromic when associated with extra-aortic features. About 20-25% of patients with non-syndromic HTAD exhibit a family history of aortic disease. Thus, a careful clinical evaluation o

BACKGROUND: The precise role collagen plays in acute aortic dissection (AAD) was investigated in an animal model of β-aminopropinitrile (BAPN)-induced AAD.
METHODS: The 30 3-week-old male specific-pathogen free (SPF)-grade Sprague-Dawley (SD) rats were randomly divided into two groups: 10 in the Control group and 20 in the Model group. The Model group was treated with 0.1% BAPN for 4 weeks, while the Control group received untreated water. Histopathological staining and western blot were used to detect changes of the extracellular matrix (ECM) and collagen content in the aorta.
RESULTS: At t

BACKGROUND A non-infectious inflammatory reaction against replaced aortic graft for aortic dissection often manifests as fever, malaise, and peri-graft effusion. It usually lasts less than 1 month and subsides spontaneously without immunosuppressive treatment. CASE REPORT A 49-year-old man underwent ascending aorta and total arch replacement for acute thoracic aortic dissection. He had fever, malaise, nausea, and elevated serum C-reactive protein for 1 month postoperatively. Pathological examination of the aorta revealed no aortitis, and repeated blood cultures were negative. We also noted per
