Persistent postural-perceptual dizziness (PPPD) is a functional vestibular disorder, and is the most common cause of chronic vestibular syndrome. However, the pathogenesis of PPPD is currently unclear. This study aimed to analyze the changes of brain spontaneous functional activities in PPPD patients during the resting state, and to explore the underlying pathogenesis of PPPD, particularly the abnormal integration of visual and vestibular information. Ten PPPD patients and 10 healthy controls were enrolled from January to June 2018, and baseline data were collected from all subjects. Videonyst…

In 2017, the term "persistent postural-perceptual dizziness" (PPPD) was coined by the Bárány Society, which provided explicit criteria for diagnosis of functional vertigo and dizziness disorders. PPPD can originate secondarily after an organic disorder (s-PPPD) or primarily on its own, in the absence of somatic triggers (p-PPPD). The aim of this database-driven study in 356 patients from a tertiary vertigo center was to describe typical demographic and clinical features in p-PPPD and s-PPPD patients. Patients underwent detailed vestibular testing with neurological and neuro-orthoptic examinati…

PURPOSE: The prognosis of older patients with dizziness in primary care is unknown. Our objective was to determine the prognosis and survival of patients with different subtypes and causes of dizziness.
METHODS: In a primary care prospective cohort study, 417 older adults with dizziness (mean age 79 years) received a full diagnostic workup in 2006-2008. A panel of physicians classified the subtype and primary cause of dizziness. Main outcome measures were mortality and dizziness-related impairment assessed at 10-year follow-up.
RESULTS: At 10-year follow-up 169 patients (40.5%) had died. Pre…

BACKGROUND: The elderly often complain of faintness, lightheadedness, dizziness, and unsteadiness, which may be associated with the orthostatic hypotension (OH), but most of them are subclinical. The pathogenic mechanism of subjective symptoms in the elderly with OH is controversial.
AIMS: The aim of this study was to assess the involvement of the irregularity of the vertical perception in the dizzy elderly with OH.
METHODS: This study consisted of 403 patients seeking treatment for dizziness in our hospital. The neurotological examinations include the subjective visual vertical (SVV) test a…

Individuals with acrophobia experience anxiety and dizziness when exposed to heights. It may be that their balance system is disturbed and that they therefore have to rely more strongly on visual information. We tested this hypothesis by exposing 20 individuals with high fear of heights and 20 healthy control participants to nine different visual flow stimuli through a head mounted display, thereby inducing a conflict between visual input and somatosensory information. Anxiety and dizziness were assessed repeatedly by means of self-reports, while resultant body sway was measured continuously w…

Panic disorder and agoraphobia have been associated with increased functional disability in individuals who have vestibular problems. The Dizziness Handicap Inventory (DHI), a 25-item self-report questionnaire with three rationally derived subscales, was designed to measure functional, emotional, and physical disability associated with vestibular disturbance. Despite the apparent usefulness of the DHI, there have been few studies of its psychometric properties. The primary purpose of the present study was to assess the factor structure of the DHI. A principal components analysis with oblique r…

Orthostatic dysregulation (OD) generally implies a systemic condition indicating poor circulatory function resulting from autonomic imbalance. It frequently appears in younger individuals at or around puberty, and is predominant in females. It is also known that individuals with this condition experience orthostatic dizziness and/or giddiness, fainting on standing and other various non-specific, subjective symptoms. Three hundred and one young normal females ranging in age from 18 to 21 years were examined using the Schellong test and the OD questionnaire to investigate the relationship betwee…

BACKGROUND: Dizziness is a common but nonspecific symptom that frequently leads to emergency department visits and ambulance transports, particularly when symptoms are sudden or severe. Although meteorological conditions affect various acute health outcomes, we still know little of their associations with dizziness-related emergency transports. This study examined the relationship between short-term meteorological conditions and dizziness-related ambulance transports using population-based data.
METHODS: We conducted a retrospective population-based study using emergency transport record data…

INTRODUCTION: Dizziness and balance impairments are underexplored symptoms in older adults with cancer. Age-related factors, comorbidities, and chemotherapy may contribute to its prevalence and severity, potentially affecting quality of life, increasing fall risk, and delaying treatment. Data on the incidence and prevalence of these symptoms are limited. This systematic review aimed to summarize the evidence and estimate the incidence and prevalence of dizziness or vertigo and impaired postural balance in patients with cancer ≥65 years receiving chemotherapy.

Patients with persistent postural-perceptual dizziness (PPPD) perceive postural instability larger than the observed sway. It is unknown whether the concept of postural misperception prevails during vestibular stimulation and whether it may account for the unsteadiness patients complain during body movements. We tested the hypothesis of an abnormal sensory-perceptual scaling mechanism in PPPD by recording objective, perceived, and the reproduced postural sway under various standing conditions, modulating visual and proprioceptive input, by binaural galvanic vestibular stimulation (GVS). We rel…

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a chronic vestibular syndrome often triggered by acute or episodic vestibular syndromes, such as Meniere's disease (MD). According to the diagnostic criteria, PPPD may coexist with other structural diseases, and the evidence of another active illness does not necessarily exclude PPPD diagnosis. However, persistent symptoms, even those meeting the PPPD criteria even long after Meniere's attack, are often overlooked as potential PPPD precipitated by MD. Some clinicians overlook PPPD in such patients, treating them solely for MD once …
