Pharmacokinetic interactions between clozapine and valproic acid in patients with treatment-resistant schizophrenia: Does UGT polymorphism affect these drug interactions?

The combination of valproic acid (VPA) and clozapine (CLZ) is regularly prescribed for augmentation therapy in treatment resistant schizophrenia. The VPA has been shown to reduce norclozapine (NCLZ) plasma levels, but the mechanism of this interaction remains unknown. The aim of this study is to examine the differences between patients treated with CLZ and patients treated with CLZ plus VPA. For it, various factors have been evaluated. The study was based on plasma samples from CLZ and CLZ plus VPA treated patients (n = 61) subjected to routine therapeutic drug monitoring considering clinical

Drug-biomolecule interactions: bioelectrometric study of the mechanism of carbachol interactions with the cornea and its relation to miotic activity.

The augmentation of carbachol miotic activity attributable to enhanced transcorneal absorption, which results from the action of cationic adjuvants included in ophthalmic vehicles, suggested a study of carbachol-corneal tissue interaction as a further step toward understanding the phenomenon. The present study was performed in vivo using an innocuous electrometric technique. A fixed charge density of the corneal epithelial surface versus carbachol concentration profile was obtained from the electrometric results; it revealed three distinct concentration regions defined by precipitous decreases

D3I-COMCF: Dissimilarity-driven dual-interaction via co-occurrence motifs and complementary fragments for drug-drug interaction prediction.

Drug-drug interaction (DDI) prediction seeks to identify pharmacodynamic and pharmacokinetic interactions arising from the co-administration of multiple drugs with differing physicochemical properties. However, most existing methods predominantly focus on global molecular or atomic topologies, overlooking the critical role of dynamically derived functional substructures (motifs) in modulating drug activity and the occurrence and strength of DDI. To this end, we propose a dissimilarity-driven dual-interaction via co-occurrence motifs and complementary fragments (D3I-COMCF) framework that deeply

Entecavir Interacts with Influx Transporters hOAT1, hCNT2, hCNT3, but Not with hOCT2: The Potential for Renal Transporter-Mediated Cytotoxicity and Drug-Drug Interactions.

Entecavir (ETV) is one of the most potent agents for the treatment of the hepatitis B viral infection. The drug is principally eliminated by the kidney. The goal of this study was to investigate the potential of ETV to interact in vitro with the renal SLC transporters hOAT1, hOCT2, hCNT2 and hCNT3. Potential drug-drug interactions of ETV at the renal transporters with antiviral drugs known to be excreted by the kidney (adefovir, tenofovir, cidofovir) as well as transporter-dependent cytotoxicity were also examined. Interactions with the selected transporters along with cytotoxicity were studie…

OAT3 Participates in Drug-Drug Interaction between Bentysrepinine and Entecavir through Interactions with M8-A Metabolite of Bentysrepinine-In Rats and Humans In Vitro.

Bentysrepinine (Y101) is a novel phenylalanine dipeptide for the treatment of hepatitis B virus. Renal excretion played an important role in the elimination of Y101 and its metabolites, M8 and M9, in healthy Chinese subjects, although the molecular mechanisms of renal excretion and potential drug-drug interactions (DDIs) remain unclear. The present study aimed to determine the organic anion transporters (OATs) involved in the renal disposition of Y101 and to predict the potential DDI between Y101 and entecavir, the first-line agent against HBV and a substrate of OAT1/3. Pharmacokinetic studies…

Novel Bruton's tyrosine kinase inhibitor remibrutinib: Assessment of drug-drug interaction potential as a perpetrator of cytochrome P450 enzymes and drug transporters and the impact of covalent binding on possible drug interactions.

PURPOSE: Pharmacokinetic drug-drug interactions (DDIs) are investigated to ensure safety for patients receiving concomitant medications. Here, we present a strategy to characterise the DDI potential of remibrutinib, as an inhibitor of drug-metabolising enzymes and drug transporters, and as an inducer. Initial in vitro studies were performed, followed by a biomarker-based assessment of induction in a first in human study, concluded by a clinical study to verify initial results. Remibrutinib is a covalent inhibitor of Bruton's Tyrosine kinase (BTKi) carrying a reactive acrylamide moiety (warhead…

Drug & virus transport across biological barriers: interactions, diffusion, partitioning, permeability, and selectivity.

Biological barriers protect the human body by selectively blocking foreign material. Designing particles with coatings that efficiently transport across these barriers can increase the effectiveness and feasibility of advanced therapeutics. In particular, the mucus barrier protects the intestines, lungs, eyes, etc., complicating oral, inhaled, or ocular drug delivery. Heuristics for particle design are currently limited to the rate of diffusion within the barrier. Relying on first-principles theories for colloidal scale interactions, a cohesive model of the transport of particles through biolo

Combined oral contraceptives and pharmacokinetic drug interactions: impact on efficacy and safety.

INTRODUCTION: Combined oral contraceptives (COCs), commonly referred to as the 'pill,' are a reliable form of contraception used by a vast majority of women of reproductive potential. COCs contain two primary components: estrogens and progestins. In addition to protection against pregnancy, COCs also confer several significant non-contraceptive benefits, such as the treatment of polycystic ovary syndrome. Given that polypharmacy is a common practice, drug interactions between COCs and concomitant therapies can adversely impact efficacy and safety. AREAS COVERED: This article systematically re…

Berberine-Drug Interactions: Mechanisms, Clinical Relevance and Risk Stratification-A Narrative Review.

Background: Berberine, an isoquinoline alkaloid present in Berberis spp., Coptis chinensis and Hydrastis canadensis, is among the most widely consumed metabolic-health supplements, popularized as "nature's Ozempic". Concurrent, often undisclosed use with prescription drugs is common in older adults, yet berberine is far from inert. Objective: To synthesize the evidence on berberine as a perpetrator of supplement-drug interactions, propose a four-axis mechanistic taxonomy, with product quality treated separately as a modifier of exposure rather than as a mechanism, and derive a clinically actio…

Strength of weakness in molecular interactions: toward transient medicines in drug discovery.

Biological systems are governed by a myriad of molecular interactions, many of which remain poorly recognized. Growing evidence suggests that transient interactions (Kdiss >1 µM) dominate the interactome and are crucial for dynamic molecular networks. By contrast, drug discovery has largely focused on high-affinity compounds that bind strongly to their targets. Although this strategy has delivered many successful medicines, exploiting transient interactions can offer new therapeutic opportunities, particularly for complex diseases. Transient medicines are characterized by rapid binding kinetic

Dual-species interactions with intestinal bacteria drive multi-drug resistance in Campylobacter.

OBJECTIVES: Multidrug-resistant (MDR) Campylobacter infections are an increasing clinical concern, as rising fluoroquinolone (FQ) resistance leaves macrolides as the primary treatment option. We investigated multidrug resistance in clinical Campylobacter samples from Germany. METHODS: We analyzed 6980 clinical isolates (2010-2022), performing phenotypic susceptibility testing and sequencing on 2912 genomes. Cultures showing multidrug resistance were studied using scanning electron microscopy (SEM). RESULTS: We found that 453 (6%) Campylobacter samples were resistant to both FQ and macrolides

Managing Drug Interactions with Enzalutamide in Patients with Prostate Cancer: A Podcast.

Enzalutamide is an oral androgen receptor signaling inhibitor used in the treatment of prostate cancer. Elderly patients with prostate cancer commonly have age-related comorbidities that require concomitant, active treatment. As a moderate inducer of the cytochrome P450 (CYP) enzymes CYP2C9 and CYP2C19, and a strong inducer of CYP3A4, there is potential for drug-drug interactions (DDIs) when enzalutamide is coadministered with other drugs that are CYP3A4 substrates-resulting in loss of efficacy or increased risk of unintended drug-related adverse events. In this podcast, we describe enzalutami…

Drug interactions in a sample of inpatients diagnosed with cannabis use disorder.

The majority of patients with cannabis use disorder (CUD) regularly take medication. Cannabinoids influence metabolism of some commonly prescribed drugs. However, little is known about the characteristics and frequency of potential cannabis-drug (CDIs) and drug-drug interactions (DDIs) in patients with CUD. Therefore, our study aimed to determine the prevalence and characteristics of drug interactions in patients with CUD during inpatient treatment on an addiction-specific ward over a six-year-period. To this aim, medication charts were analyzed and screened for potential CDIs and DDIs. Herein…

Interactions between nanoparticles and lymphatic systems: Mechanisms and applications in drug delivery.

The lymphatic system has garnered significant attention in drug delivery research due to the advantages it offers, such as enhancing systemic exposure and enabling lymph node targeting for nanomedicines via the lymphatic delivery route. The journey of drug carriers involves transport from the administration site to the lymphatic vessels, traversing the lymph before entering the bloodstream or targeting specific lymph nodes. However, the anatomical and physiological barriers of the lymphatic system play a pivotal role in influencing the behavior and efficiency of carriers. To expedite research
