A Simulation Model to Evaluate the Potential Impact of Disease-Modifying Treatments on Burden of Illness in Alzheimer's Disease.

INTRODUCTION: Alzheimer's disease (AD) is a progressive neurodegenerative disease that places a substantial burden on patients, caregivers, and society. The advent of disease-modifying treatments (DMTs) would represent a major advancement in the management of AD, particularly in early AD. It is important to understand the potential value of these therapies to individuals and society. METHODS: A modeling framework was developed to estimate the potential clinical and economic burden of AD in the USA by simulating the impact, relative to that of usual care, of a DMT with hypothesized availabilit

The Contribution of Ototoxic Medications to Hearing Loss Among Older Adults.

BACKGROUND: Ototoxicity may interact with the effects of aging, leading to a more severe hearing loss than that associated with age alone. The purpose of this study was to explore the associations between ototoxic medication use and the incidence and progression of hearing loss in older adults with a population-based longitudinal study. METHODS: Epidemiology of Hearing Loss Study participants (n = 3,753) were examined. Medication use was assessed using a standardized questionnaire by the examiners at each examination every 5 year. The ototoxic medications include loop diuretics, nonsteroidal

Viruses and Autoimmunity: A Review on the Potential Interaction and Molecular Mechanisms.

For a long time, viruses have been shown to modify the clinical picture of several autoimmune diseases, including type 1 diabetes (T1D), systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), Sjögren's syndrome (SS), herpetic stromal keratitis (HSK), celiac disease (CD), and multiple sclerosis (MS). Best examples of viral infections that have been proposed to modulate the induction and development of autoimmune diseases are the infections with enteric viruses such as Coxsackie B virus (CVB) and rotavirus, as well as influenza A viruses (IAV), and herpesviruses. Other viruses that have

Integrated molecular landscape of amyotrophic lateral sclerosis provides insights into disease etiology.

Amyotrophic lateral sclerosis (ALS) is a severe, progressive and ultimately fatal motor neuron disease caused by a combination of genetic and environmental factors, but its underlying mechanisms are largely unknown. To gain insight into the etiology of ALS, we here conducted genetic network and literature analyses of the top-ranked findings from six genome-wide association studies of sporadic ALS (involving 3589 cases and 8577 controls) as well as genes implicated in ALS etiology through other evidence, including familial ALS candidate gene association studies. We integrated these findings int

Is there a role for vitamin D in supporting cognitive function as we age?

Globally, an estimated 46 million people are currently living with dementia and this figure is projected to increase 3-fold by 2050, highlighting this major public health concern and its substantial associated healthcare costs. With pharmacological treatment yet to reach fruition, the emphasis on evidence-based preventative lifestyle strategies is becoming increasingly important and several modifiable lifestyle factors have been identified that may preserve cognitive health. These include good cardiovascular health, physical activity, low alcohol intake, smoking and a healthy diet, with growin

Vitamin D as a novel therapy in inflammatory bowel disease: new hope or false dawn?

There is increasing scientific interest in the field of vitamin D research, moving the focus beyond bone health to other disease processes. Low circulating vitamin D levels have been reported as a risk factor for several pathophysiologically divergent diseases, including cancers, diabetes, CVD, multiple sclerosis and inflammatory diseases, including rheumatoid arthritis and inflammatory bowel disease (IBD). But, therein, remains the challenge: can any single nutrient contribute to multiple complex disease mechanisms and, ultimately, have therapeutic potential? The aim of this review is to crit

Acute viral hepatitis - should the current screening strategy be modified?

BACKGROUND: The epidemiology of viral hepatitis has changed. Since the introduction of safe and effective vaccines for hepatitis A and B in 1980s, the incidence of acute infections caused by these viruses has been declining in the UK. At the same time, hepatitis E virus (HEV) has been recognised as an increasingly important cause of acute hepatitis, but testing is not widely available. OBJECTIVES: The aim of this study was to establish the viral causes of acute hepatitis, and use that data to modify the current diagnostic algorithm. STUDY DESIGN: A Cognos search was performed to collate subj

A high throughput multiplex PCR assay for simultaneous detection of seven aminoglycoside-resistance genes in Enterobacteriaceae.

BACKGROUND: The aminoglycoside-resistance genes encoding aminoglycoside modifying enzymes and 16S rRNA methyltransferases are main factors contributing to increasing resistance to aminoglycosides. Characterization and distribution of antimicrobial resistance gene profiles provide important information on the potential difficulty of treatment of bacteria. Several molecular methods have been developed to investigate the prevalence of aminoglycoside-resistance genes. These existing methods are time-consuming, labor-intensive, expensive or limited sensitivity in the epidemiological investigation.

BCG response prediction with cytokine gene variants and bladder cancer: where we are?

PURPOSE: Bladder cancer (BC) is one of the most widespread cancers afflicting men and women and also has major philosophical impact on health care worldwide. Despite elaborate characterization of the risk factors and treatment options, BC is still a major epidemiological problem worldwide and its incidence lingers to upswing each year. Over the last three decades, intravesical immunotherapy with the biological response modifier Mycobacterium bovis-Bacillus Calmette Guerin (BCG) has been established as the most effective adjuvant treatment for averting local recurrences and tumor progression fo

Smoking may be considered an established risk factor for sporadic ALS.

OBJECTIVES: A 2003 evidence-based review of exogenous risk factors for sporadic amyotrophic lateral sclerosis (ALS) identified smoking as the only risk factor that attained "probable" (more likely than not) status, based on 2 class II studies. The purpose of the current, evidence-based, update was to see if the conclusion of the previous review needed to be modified, based on studies published since. METHODS: A Medline literature search was conducted for the period between 2003 and April 2009 using the search terms smoking and (ALS or "amyotrophic lateral sclerosis" or MND or "motor neuron di

Potential for vitamin D receptor agonists in the treatment of cardiovascular disease.

Vitamin D(3) is made in the skin and modified in the liver and kidney to form the active metabolite, 1,25-dihydroxyvitamin D(3) (calcitriol). Calcitriol binds to a nuclear receptor, the vitamin D receptor (VDR), and activates VDR to recruit cofactors to form a transcriptional complex that binds to vitamin D response elements in the promoter region of target genes. During the past three decades the field has focused mainly on the role of VDR in the regulation of parathyroid hormone, intestinal calcium/phosphate absorption and bone metabolism; several VDR agonists (VDRAs) have been developed for

Dyslipidemia as a risk factor for ischemic stroke.

Ischemic stroke is a major cause of morbidity and mortality. Whereas dyslipidemia is a major risk factor for coronary heart disease (CHD), its role in the pathogenesis of ischemic stroke is less clear. Epidemiological studies have provided conflicting findings regarding the association of dyslipidemia with ischemic stroke. Overall, elevated LDL-C levels appear to increase the risk of ischemic stroke. Low HDL-C levels also appear to be associated with a greater risk whereas the importance of high triglyceride levels is less clear. The discordant results of observational studies might result fro

Risk factors associated with neuroendocrine tumors: A U.S.-based case-control study.

Carcinoids are rare neuroendocrine tumors (NETs); however, their incidence has significantly increased in the United States over the past 30 years. Little is known about the epidemiology of these cancers and their associated risk factors. We evaluated the independent effects of multiple risk factors associated with NETs arising at 5 disease sites (small intestine, stomach, lung, pancreas and rectum). We conducted a retrospective, hospital-based, case-control study involving 740 patients with histologically confirmed NETs and 924 healthy controls. Information on different risk factors was colle

An overview of the marine food poisoning in Mexico.

In the course of the last decade, huge events related to harmful algal blooms (HAB) have severely affected the environment in Mexico, even causing several human casualties. The tally of the toxins known up to date in Mexican waters includes: neurotoxin shellfish poisoning (NSP), paralytic shellfish poisoning (PSP), amnesic shellfish poisoning (ASP), tetrodotoxin (TTX) or puffer fish poisoning, ciguatera fish poisoning (CFP) and diarrhetic shellfish poisoning (DSP). Actual epidemiological figures profoundly modified the trends manifested on previous decades. Notwithstanding that the red tides a

[Mouth diseases in a prehistoric agricultural population of northeastern North America].

There is no longer any question that the consumption of refined sugar is a factor in the development of dental caries. In fact, researchers now believe that the production of refined sugar and, particularly, its great availability and use in post-industrial populations has led to a virtual revolution in both the food industry and buccopathology. Clinical and epidemiological studies on the relationship between caries and sugar consumption have been conducted for more than 40 years, and the harmful effects of sugar consumption on the development of dental caries are now well known. Recurrence of low birth weight in siblings.

The recurrence of low birth weight (LBW, less than 2500 g) in full siblings was studied in 3286 singleton infants born between 1966 and 1986 to 1677 male U.S. Army veterans who were part of a nationwide health study. Hospital of birth medical records were abstracted for these children. Mean birth weights, risks of LBW, LBW occurring with preterm delivery (less than 37 weeks) (LBW/p), and LBW in term infants (LBW/t) were examined in successive singleton siblings according to LBW status of prior siblings. The risk of LBW in infants who had prior siblings with LBW was 9.9%, compared with a risk o

Smoking Interacts With APOE ε4 to Influence Dementia: Evidence From the UK Biobank and Validation by Meta-Analysis of Longitudinal Studies.

BACKGROUND: Gene-by-environment interaction plays important roles in influencing dementia occurrence. Smoking is a well-established modifiable risk factor for all-cause dementia (ACD). However, its interaction effects with APOE ε4 as well as the underlying peripheral mechanisms are unclear. METHODS: Panoramic smoking burden (PSB) and smoking history score (SHS) were constructed for 173,366 participants without dementia from the UK Biobank. The associations of PSB and SHS with ACD risk (median follow-up = 13 years) and their interactions with APOE ε4 carrier status were explored in death-compe
