Anatomical and clinical research has shown that the entire lateral epicondylar region is innervated only by radial nerve branches. Based on these investigations we have developed a surgical procedure for complete denervation which is indicated only in resistant cases of tennis elbow. Only one nerve pathway calls for careful exposure, otherwise denervation is accomplished blindly by disinsertion of certain muscles. This is a report of three cases of complete dislocation of both the radius and ulna at the elbow in infants born with obstetric palsy. All three patients had painless elbow-flexion contractures. The formation of a painful neuroma after operations on the medial or lateral sides of the elbow is a common problem. Our aim was to determine the relationship of the cutaneous nerves to the three usual skin incisions around the elbow. We reviewed the records of 72 professional baseball players who underwent arthroscopic or open elbow surgery. The most common diagnoses were posteromedial olecranon osteophyte (65%), ulnar collateral ligament injury (25%), and ulnar neuritis (15%). Intraarticular loose bodies were found in 39% of the patients. Fifty-nine patients (82%) were observed for a minimum of 24 months, with an average of 42 months' followup. Ten below-elbow joint arterial bypass grafts performed in nine patients for chronic upper extremity ischaemia are reviewed. The cause of the ischaemia was thromboangiitis obliterans in four patients, iatrogenic trauma in four and unknown aetiology in one. Graft revision was required in two patients with thromboangiitis obliterans who underwent axillary-brachial bypass. Rapid, self paced and self terminated elbow flexion movements were studied in a group of 10 patients with dystonia affecting the arms. The movements were slower and for small amplitude movements, more variable than those recorded in normal subjects. The duration of the first agonist burst was prolonged, even when compared with normal subjects deliberately moving slowly. A case report of factitious extension contracture of the elbow due to injection-induced fibrous myopathy of the triceps is presented. Crystalline material was demonstrated in muscle biopsies preserved in absolute alcohol and surgical section of the triceps resulted in correction of the deformity. A review of the literature and differentiation between Weber-Christian systemic panniculitis and factitial panniculitis is discussed.

In 15 patients with arthrogryposis multiplex congenita, 18 operations were performed to improve elbow function. These operations were for seven cases of pectoralis transfer, seven cases of anterior transfer of triceps, two cases of posterior elbow release, and two cases of Steindler flexoroplasty. Results were evaluated on two simple criteria: single-hand feeding and improved elbow joint motion. These criteria showed that the pectoralis transfer was the most effective procedure.
