Causes and mechanisms of acid-base and electrolyte abnormalities in cancer patients.

Patients with cancer frequently exhibit acid-base and electrolyte disturbances that complicate their management and prolong their hospitalization. The mechanisms encountered for these abnormalities are multifactorial in origin. Both the underlying disease and the therapeutic interventions can contribute to the development of these disturbances. An understanding of the mechanisms involved in their pathogenesis is of paramount importance for their prevention and treatment in cancer patients. This article briefly reviews the causes and the pathophysiology of acid-base and electrolyte abnormalitie

Some interactions between gastrointestinal function and fluid and electrolyte homeostasis.

PURPOSE OF REVIEW: The gastrointestinal tract plays an important role in the maintenance of fluid and electrolyte balance, and its diseases cause disturbances in that balance. Conversely, fluid and electrolyte disorders may disturb gastrointestinal function. The purpose of this review is to highlight some recent literature describing these interactions against a background of fluid and electrolyte physiology in normal and disease states. RECENT FINDINGS: Gastrointestinal disease is the most common cause of fluid and electrolyte disturbance, and this can usually be corrected by the oral route,

Brugada pattern electrocardiographic changes associated with profound electrolyte disturbance.

Only a few case reports of Brugada pattern ECG changes caused by electrolyte disturbance exist in the literature, all of which lack adequate electrophysiological exclusion (or inclusion) of an underlying Brugada syndrome. This report describes a case of an otherwise healthy 38-year-old man who presented to the hospital with diabetic ketoacidosis, profound electrolyte disturbance, and Brugada pattern ECG changes. Subsequent flecanide drug challenge and electrophysiological studies ruled out an underlying Brugada syndrome.

Heat cramps: fluid and electrolyte challenges during tennis in the heat.

Sweat losses during tennis can be considerable. And while most players make a genuine effort to stay well hydrated to maintain performance and reduce the risk of heat illness, regular and copious water intake is often not enough. Besides an extraordinary water loss, extensive sweating can lead to a concomitant large electrolyte deficit too--particularly for sodium. Although a variety of other mineral deficiencies and physiological conditions are purported to cause muscle cramps, evidence suggests that, when a tennis player cramps in warm to hot weather, extensive and repeated sweating during t

Endovascular occlusion of basilar bifurcation aneurysms with electrolytically detachable coils.

OBJECT: To describe the clinical and angiographic results of endovascular occlusion of basilar bifurcation aneurysms with electrolytically detachable coils, and to identify factors which should be considered in deciding upon surgical or endovascular treatment. METHODS: We report our experience with 40 patients in whom occlusion of basilar bifurcation aneurysms with electrolytically detachable coils was attempted. All patients underwent superselective angiography and attempted embolization with Guglielmi detachable coils (GDCs). Angiographic and clinical results were prospectively recorded. Tw

Interactions between glucocorticoids and mineralocorticoids in the regulation of renal electrolyte transport.

The enzyme 11 beta-hydroxy steroid dehydrogenase (11 beta-OHSD) was described and its location in various organs noted more than 30 years ago (Mahesh and Ulrich, 1960; Jenkins, 1966). 11 beta-OHSD inactivates circulating glucocorticoids by transforming the hydroxyl group at the 11-carbon to a keto group. This chemical reaction has taken on a greater degree of physiologic and clinical significance in recent years. It has been suggested that 11 beta-OHSD, present in mineralocorticoid target tissues, can act as a 'guardian' over the mineralocorticoid receptor by transforming circulating endogenou

Electrolyte disorders associated with respiratory distress syndrome and bronchopulmonary dysplasia.

In understanding the various electrolyte disorders associated with respiratory failure, one has to appreciate how the otherwise healthy premature infant maintains extracellular fluid homeostasis. The same physiologic processes that regulate body fluids are affected to varying degrees in the infant with respiratory failure. Maintenance of sodium conservation by the kidney is of critical importance for survival and growth. The immature kidney tends to exhibit excessive sodium wasting especially early on, but appropriate positive sodium balance is achieved generally within the first week. Appropr

Hypothalamic endothelin: presence and effects related to fluid and electrolyte homeostasis.

Endothelin-3-like immunoreactivity (ET-3-ir) was detected in extracts of rat hypothalamic median eminence, and in the anterior and neurointermediate lobes of the pituitary at levels (ng ET-3/mg protein) exceeding those present in extracts of abdominal aorta. This ET-3-ir appeared authentic because radioimmunoassay (RIA) dose-response curves parallel to those of synthetic ET-3 could be constructed and this ET-3-ir comigrated on C-8 high-pressure liquid chromatography (HPLC) with synthetic ET-3. Endothelin-1-like immunoreactivity, on the other hand, was abundant in extracts of abdominal aorta an

[Water-electrolyte and acid-base imbalance. IX. Respiratory alkalosis].

Respiratory alkalosis is the consequence of primary hypocapnia of divergent etiologies. Any pathologic process that increases ventilation to levels beyond that required to excrete the CO2 byproduct of metabolism, will result in an inappropriately low systemic pCO2 and a tendency to an alkaline systemic pH. The increased drive to ventilation may be due predominantly to a primary increase in central nervous system activity, either within the respiratory center itself or from more centrally placed areas with neural projections that extend to and control the respiratory center. Alternatively, an i

[Electrolyte solution with polyethylene glycol to cleanse the colon for colonoscopy or enema].

The efficacy and safety of an electrolyte-polyethylene glycol solution (SE-PEG) for colonic lavage, was compared with standard bowel preparation (SBP) in a randomized blinded study of volunteers and patients undergoing colonoscopy and barium enema examination. Side effects, biochemical and hematologic changes and quality of examinations were monitored. Colonoscopy and barium enema was scored by colonic segment for type of residual stool and percentage of bowel wall visualized. For colonoscopy and barium enema, preparation with SE-PEG allowed better visualization and produced more optimal exams

[Changes in water-electrolyte balance and central hemodynamics in hypertensive crises].

Sodium, potassium and water distribution, central hemodynamic parameters, and plasma renin activity and aldosterone levels were examined in 48 patients with stable essential hypertension, including 10 males and 11 females with hypertensive crises aggravating the course of the disease, and in 18 controls. The individuals showing signs of heart and renal failure were excluded from the analysis. In female patients, the crise pattern of the disease was associated with marked changes of sodium metabolism, namely, raised extracellular fluid and plasma sodium levels, the patients with the crises havi

Muscle cell growth and the distribution of water and electrolyte in human pregnancy.

Ten normal pregnant women had muscle composition analyses (rectus abdominis) carried out at 39-40 weeks of pregnancy. Water, chloride (Cl), chloride space (ECV), non-chloride space (ICW), potassium (K), sodium (Na), magnesium (Mg) and zinc (Zn) determinations were carried out. Analyses for DNA (cell number), protein: DNA ratio (cell size), RNA and collagen were also performed. Similar analyses were performed on uterine muscle and placentae before and after perfusion with Earle's solution. Data from pregnant patients were compared with similar estimations carried out on rectus abdominis samples

Diagnosis of hydatid cyst by percutaneous aspiration: value of electrolyte determinations.

A study was made of the fluid from 11 hydatid cysts (10 hepatic, one renal) and from 50 nonparasitic cysts (25 renal, 10 ovarian, 10 pancreatic and five hepatic). Our analysis indicated that the chloride (range 62-95 mEq/liter), the sodium (range 125-139 mEq/liter) and the potassium content (range 5.1-7.5 mEq/liter) of the former differed significantly (p less than 0.01) from those of the controls. Hydatid cysts may be indistinguishable from others with both CT and ultrasound and therefore the electrolyte data are of diagnostic value when a hydatid cyst is accidentally punctured percutaneously

Structural adaptation to altered electrolyte metabolism by cortical distal segments.

The four different renal cell types in the cortical segments beyond the macula densa--distal convoluted tubule (DCT) cell, the connecting tubule (CNT) cell, the principal (P) cell, and the intercalated (I) cell--each respond to prolonged specific stimuli with changes in cell membrane area. Increases in basolateral cell membrane area, reflecting the transport capacity of the cell, are associated in DCT cells with luminal, chronically high sodium load, in CNT cells with mineralocorticoids and tubular solute load, and in P cells with mineralocorticoids. In I cells the luminal cell membrane area s

Corticosteroid alteration of active electrolyte transport in rat distal colon.

To determine the effect of corticosteroids on active transport processes, unidirectional fluxes of 22Na, 36Cl, and 42K were measured under short-circuit conditions across isolated stripped distal colonic mucosa of the rat in control, secondary hyperaldosterone, and dexamethasone-treated animals. In controls net sodium and chloride fluxes (JNanet and JClnet) and short-circuit current (Isc) were 6.6 +/- 2.2, 7.6 +/- 1.6, and 1.3 +/- 0.2 mu eq X h-1 X cm-2, respectively. Although aldosterone increased Isc to 7.3 +/- 0.5 mu eq X h-1 X cm-2, JNanet (6.9 +/- 0.7 mu eq X h-1 X cm-2) was not altered a

[Prograde colonic irrigation without disturbance of the water-electrolyte balance].

The effects of four different irrigation solutions for preparation of colonoscopy were assessed in 64 patients. Using saline lavage solutions similar to plasma led to predominant symptoms of volume overload. Body weight transitorily increased by 1.7 +/- 0.2 kg, water and electrolytes were retained. Time until appearance of clear lavage fluid was 145 +/- 5 min. Irrigation by mannitol solution was badly tolerated and frequently led to vomiting. Body weight decreased and loss of water and electrolytes was reflected in changes of blood composition. Lavage with a hypertonic salt and mannitol soluti…
