[Antithrombotic treatment in non-rheumatic atrial fibrillation. Do we follow the recommendations of clinical trials?].

OBJECTIVES: Several clinical trials have demonstrated that antithrombotic treatment may be effective in prevention of stroke in nonrheumatic atrial fibrillation (AF). The aim of this study was to assess if we follow clinical trial recommendations in community practice. METHODS: We analyzed 225 medical records of patients diagnosed of nonrheumatic AF in Cáceres, during February and March 1998. Patients who were contraindicated to follow antiagreggation or anti-coagulation treatment were excluded. We compared patients with and without antithrombotic treatment with different demographic characte

Carotid stenting: risk factors for periprocedural stroke.

Carotid artery stenting is rapidly emerging as a minimally invasive alternative to carotid endarterectomy for the treatment of cervical carotid artery occlusive disease and stroke prevention. Periprocedural stroke risk still remains a concern despite improvements in technique and equipment. We will review technical, clinical and radiographic factors associated with an increased risk of stroke during carotid artery stenting. The primary focus will be on technical proficiency of the interventionalist, patient comorbid conditions (i.e., diabetes, coronary artery disease, renal failure and advance

Effectiveness and outcomes of endovascular embolization on arteriovenous malformations with aneurysms: a nationwide multicenter prospective registry study.

OBJECTIVE: This study aims to evaluate the long-term outcomes of cerebral arteriovenous malformations (AVMs) with associated aneurysms in patients who underwent exclusive endovascular embolization. METHODS: Patients with aneurysmal AVMs were identified from China's nationwide MATCH registry. Propensity score matching balanced baseline differences between embolization and conservative management groups. The primary outcome was long-term hemorrhagic stroke or death; secondary outcomes included obliteration rate and neurological status. Subgroup and sensitivity analyses using various designs ass

Histologic differences between in situ and embolized carotid web thrombi: a case report.

BACKGROUND: The significance of carotid webs (CaWs) in ischemic stroke is becoming acknowledged. Histological features of clot composition in situ and secondary cerebrovascular embolized thrombi caused by CaW have not been described concurrently. Understanding clots' histological composition is essential for understanding the pathophysiology of clot formation in CaW. CASE PRESENTATION: A 50-year-old male patient with acute ischemic stroke, which was believed to be caused by ipsilateral CaW, was admitted to the hospital. Mechanical thrombectomy was used to retrieve thromboemboli from the middl

Use of distal embolic protection devices as a bailout strategy for suboptimal flow reversal during transcarotid artery revascularization: illustrative case.

BACKGROUND: Transcarotid artery revascularization (TCAR) has become an increasingly utilized surgical option for carotid disease, relying on flow reversal for distal embolic protection (DEP). However, anatomical variations, such as an isolated middle cerebral artery hemisphere, and severe carotid stenosis may compromise the safety of flow reversal intraoperatively. The authors describe the use of an adjunctive DEP device during TCAR as a "bailout" strategy for high-risk patients. OBSERVATIONS: The authors present the case of an adult patient with high-grade symptomatic carotid stenosis who wa

Optimal Timing of Oral Anticoagulant Therapy Initiation Post-acute Ischemic Stroke: A Narrative Review.

Initiation of oral anticoagulation therapy after acute ischemic stroke requires the best trade-off between prevention of early recurrent embolic events and hemorrhagic transformation avoidance. While guidelines typically recommend starting therapy between 4 and 14 days, current evidence suggests that timing should be tailored to stroke severity, imaging findings, and individualized patient characteristics. This narrative review summarizes evidence from guidelines and clinical trials/observational studies published between 2013 and 2025, identified through PubMed, Web of Science, and Google Sch

Hemiplegia From Disseminated Emboli due to Infective Endocarditis Secondary to Acute Rheumatic Fever in a 10-Year-Old.

Hemiplegia in children needs early cardiology evaluations, just as it does for neurological assessments. Early recognition, evaluation, management, and prophylaxis by a multidisciplinary team for infective endocarditis in pediatric groups with rheumatic heart disease are crucial to prevent complications like embolic stroke in resource-limited settings.

[Mechanisms involved in large subcortical infarcts].

Large subcortical infarcts(maximum diameter of infarct > or = 20 mm) result from various stroke patterns, including striatocapsular infarcts (SCI), corona radiata infarcts, centrum semiovale infarcts, and internal borderzone infarcts. A systematic investigation of stroke pathogenesis involved in large subcortical infarcts, however, has not been performed. This study attempted to clarify the stroke mechanisms involved in large subcortical infarcts, by examining 50 patients with large subcortical infarcts out of 430 ischemic stroke patients consecutively registered in our department. The subject

Volume of subclinical embolic infarct correlates to long-term cognitive changes after carotid revascularization.

OBJECTIVE: Carotid intervention is safe and effective in stroke prevention in appropriately selected patients. Despite minimal neurologic complications, procedure-related subclinical microemboli are common and their cognitive effects are largely unknown. In this prospective longitudinal study, we sought to determine long-term cognitive effects of embolic infarcts. METHODS: The study recruited 119 patients including 46% symptomatic patients who underwent carotid revascularization. Neuropsychological testing was administered preoperatively and at 1 month, 6 months, and 12 months postoperatively

Current Endovascular Approach to the Management of Acute Ischemic Stroke.

PURPOSE OF REVIEW: The review provides an overview of current endovascular management of patients with acute ischemic stroke in the light of recent landmark trials proving unequivocal benefit of the intervention. RECENT FINDINGS: Several randomized trials looking at selective groups of patients presenting after an acute ischemic stroke due to large vessel occlusion in the anterior circulation demonstrated an overwhelming benefit of the endovascular treatment compared to intravenous thrombolysis, leading to expedited changes in the American Heart Association/American Stroke Association guideli

Cerebral protection devices for transcatheter aortic valve replacement.

Stroke is a devastating, potential complication of any cardiovascular procedure including transcatheter aortic valve implantation (TAVI). Even clinically silent lesions as detected by magnetic resonance imaging have been associated with poor long-term cognitive outcomes. As a result, extensive efforts have been focused on developing stroke preventative strategies including the development of novel embolic protection devices. These devices aim to reduce this risk by capturing or deflecting emboli away from the cerebral circulation. Areas covered: This review provides an insight into the inciden

Systemic embolic events with nonbacterial thrombotic endocarditis as manifestations of recurrent ovarian clear cell carcinoma.

OBJECTIVE: To present a rare case of recurrent ovarian clear cell carcinoma (OCCC) with systemic embolic events. CASE REPORT: A 60-year-old woman with a history of OCCC presented with an acute ischemic stroke. Magnetic resonance imaging showed multifocal, bilateral infarctions over the cerebrum and cerebellum. An echocardiogram revealed vegetation growth in the mitral and tricuspid valves and showed no evidence of atrial fibrillation. The serological studies for stroke were negative for all assessed parameters (normal values of protein C, protein S, antithrombin III, and lupus anticoagulant a

Early angiographic and CT findings in patients with hemorrhagic infarction in the distribution of the middle cerebral artery.

Hemorrhagic infarction subsequent to ischemic brain damage, even if small, slight, or marbled, can be detected by CT. The mechanisms that give rise to this transformation in humans are not well elucidated. Previous reports indicate that hemorrhagic infarction is most common in embolic stroke and large infarcts, and can worsen the clinical state of ischemic patients. We examined 36 patients with supratentorial ischemic signs and symptoms within the first hours after onset. CT was used to judge if hypodensity on early CT studies might predict the development of hemorrhagic infarction. Angiograph…

Stroke in the Lehigh Valley: racial/ethnic differences.

We investigated black/white differences in stroke rate (standardized morbidity), severity, and subtype, and the relative frequencies of 5 primary risk factors (hypertension, diabetes, myocardial infarction, other heart diseases, and transient ischemic attack [TIA]) using the Lehigh Valley Stroke Register. Blacks had a statistically significant higher, age-adjusted rate of stroke than whites. We found no differences in stroke severity using our measures but blacks had a statistically higher proportion of lacunar stroke, while whites had a higher proportion of embolic stroke. There were no diffe…

Infective Endocarditis: An Embolic Case.

Infective endocarditis is a sudden illness that rapidly causes cardiac and extracardiac injury. Embolic material travels into the arterial circulation causing embolic events in 20-50% of patients. The brain is one of the most frequent sites of embolism that potentially interferes with treatment options. Neurologic complications are the presenting symptom in 20% of the cases being associated with poor prognosis (45% of deaths versus 24% in patients without these complications). This is the case of a 63-year-old male patient presenting with main clinic of stroke. Multifocal signs and past aortic
