Biliary tract emergencies. Acute cholecystitis, acute cholangitis, and acute pancreatitis.

Acute cholecystitis, acute cholangitis, and acute pancreatitis represent the most common biliary tract emergencies. Most are due to gallstones in the gallbladder and bile ducts. Acute cholecystitis is treated by surgery in most cases. Laparoscopic cholecystectomy combined with endoscopic sphincterotomy may become more common in the future for treatment of acute cholecystitis as well as in cases of acute cholangitis and pancreatitis if the bile ducts are cleared of gallstones. Although the role of either surgery or endoscopic treatment may be more clearly defined in some biliary tract emergenci…

A descriptive study of emergencies admitted to a surgical department.

A study was undertaken to evaluate the pattern of emergency admissions and their management by the surgical firm in a medium size (700 beds) general hospital. Over a three month period, 258 patients were admitted to one surgical team that was on take every third day. All patients were entered into a protocol that was updated daily by a registrar. In four (13.3%) of the 30 days on take, patients had to be referred to a nearby hospital due to shortage of emergency beds. The accuracy of diagnosis by the Accident and Emergency (A&E) resident, surgical resident and surgical team were 75.7, 77.2 and

["Round the clock"--an information service for malignant hyperthermia emergencies].

A 24-hour 7-day telephone service has been created for emergency consultation: MH-hotline 030/3035504 (daytime) or 030/30351 (after office hours). In emergencies callers should ask for a consultant, indicating the code word "malignant hyperthermia" and give their name, the name of the institution and the telephone number.

Treatment of hypertensive emergencies with the calcium channel blocker nifedipine.

The severe elevations in blood pressure that occur in hypertensive emergencies pose a serious threat to life or vital organ functions. However, use of antihypertensive agents to acutely reduce blood pressure during hypertensive emergencies may cause deficits in the perfusion of the central nervous system or the heart. Therefore, a knowledge of cerebral blood flow regulation during acute treatment of hypertensive emergencies is indispensable. Experience with the calcium channel blocker nifedipine in the acute treatment of patients with hypertensive emergencies has shown that this agent has a pr…

Pharmacokinetic and pharmacodynamic considerations in drug therapy of cardiac emergencies.

In the drug therapy of cardiac emergencies, it is necessary to rapidly achieve therapeutic drug concentrations and adjust drug dose as the patient's clinical status changes. Cardiac dysfunction is often present and may alter drug pharmacokinetics. Circulatory failure causes sympathetically mediated vasoconstriction in most tissues, with relative sparing of the brain and heart due to autoregulation. Blood flow to vasoconstricted tissues is reduced, and the available cardiac output is redistributed so that the heart and brain receive a greater fraction. Drug distribution to tissues is therefore …

[Value of echocardiography in cardiovascular emergencies (not including congenital cardiopathies)].

The principal echocardiographic features of the main cardiovascular emergencies are discussed. After setting out a method of detailed analysis for echocardiography, the changes found in the main causes of severe chest pain, systemic embolisation and sudden heart failure are described. Special emphasis is laid on the signs of aortic root dissection, pericarditis and tamponade, ruptured valve, the mechanical complications of acute myocardial infarction and, in particular, on the assessment of residual myocardial function. Finally, the authors maintain that echocardiography should be a systematic…

Social Vulnerability and Out-of-Hospital Identification of Neurologic Emergencies.

STUDY OBJECTIVE: Timely out-of-hospital recognition and management of neurologic emergencies-including status epilepticus, stroke, intracerebral hemorrhage, and traumatic brain injury-are critical to improving patient outcomes. However, the influence of social risk factors on the delivery and quality of emergency medical services (EMS) care in these time-sensitive conditions remains poorly understood and may contribute to long-term morbidity and mortality. This study examines the association between community-level social vulnerability and out-of-hospital identification of neurologic emergenci…

Exosomal lipidomic alterations as prospective molecular signatures for radiation emergencies.

In radiological and nuclear emergency scenarios, it is crucial to quantify the dose of ionizing radiation exposure. The conventional biodosimetry techniques, such as dicentric assays, are time and resource intensive for widespread usage. Exosomes being stable extracellular vesicles bearing molecular cargo can offer a promising alternative for minimally invasive, high-throughput biodosimetry. In the current study, plasma-derived exosomes were isolated using ultracentrifugation in Sprague Dawley rats at 24h, 48h, and 72h following gamma irradiation 2, 5 and 8 Gy. Exosomes were characterized usin

Nicardipine Versus Nifedipine for Postpartum Hypertensive Emergencies in Severe Preeclampsia.

Severe preeclampsia (sPE)-induced postpartum hypertensive emergencies (PHEs) pose a threat to maternal safety, requiring rapid and stable blood pressure (BP) control. This protocol describes an observational, non-randomized controlled cohort study designed to evaluate the efficacy and safety of two calcium channel blockers-nicardipine (NIC) and nifedipine (NIF)-in patients with sPE-induced PHEs. Key procedural steps include: retrospective enrollment of 131 eligible patients (67 in the NIC group, 64 in the NIF group) admitted between April 2024 and May 2025; propensity score matching to balance…

Cardiological hypertensive emergencies: Real word data compared to guidelines indications.

BACKGROUND: Cardiological Hypertensive Emergencies (CHE) are characterized by markedly elevated blood pressure (BP) values associated with acute cardiac hypertension-mediated organ damage (acute coronary syndrome, acute heart failure, acute pulmonary edema, or aortic dissection). This study aimed to evaluate the prevalence, clinical features and real-world BP management of CHE and to assess adherence to guideline recommendations by comparing ED practice before and after recent guideline release (2017 vs 2019). METHODS: This single-center retrospective analysis included all adult patients admi…

Acute emergencies due to spontaneous abdominal hemorrhage: a pictorial review.

Spontaneous abdominal hemorrhage (SAH) refers to acute intra-abdominal or pelvic hemorrhages occurring from non-traumatic or non-iatrogenic causes. Early recognition of SAH with prompt surgical or interventional management is necessary to prevent morbidity and mortality. The initial clinical presentations are often misleading, and the diagnosis is generally made based on radiological imaging. In this article, we discuss the anatomy of abdominal and pelvic compartments, etiology/pathogenesis, and characteristic imaging findings of spontaneous hemorrhage in each of these compartments, thus aidin

Telemedicine for prehospital respiratory emergencies in a retrospective quality analysis.

Respiratory distress is a common reason for emergency medical service (EMS) physicians to be prehospitally involved. While the availability of telemedical EMS systems increases continuously, there is a gap in research regarding respiratory emergencies in the context of prehospital telemedicine. The aim of this study is to evaluate the quality of care provided in prehospital respiratory emergencies, managed through a tele-EMS system with a specialized EMS physician. Tele-EMS physician missions from 01/01/2019 to 12/31/2021 in Aachen, Germany, were analyzed. Adult patients presenting with dyspne…

[Management of palliative care patients facing emergencies in the community].

With an aging population, an increasing prevalence of chronic illnesses, and rising cancer cases, palliative emergencies in the community are becoming an ever-growing challenge. These situations generate new care needs, calling for general palliative care competencies among first-line healthcare providers and strong collaboration between the various actors involved in community-based care. But what exactly is a "palliative emergency"? Who intervenes, and how? Is it possible, even in the urgency of such situations, to foster patient autonomy and respect their care preferences? This article expl…
