Psychotic episodes and epilepsy can occur without mental retardation in tuberous sclerosis. The diagnosis may depend on elucidating the family history since physical signs may not be obvious. Establishing the diagnosis makes genetic counselling possible.

Single-gene mutations in mice initiate specific heritable neuronal diseases featuring different patterns of epilepsy. Special aspects of antiepileptic therapy during pregnancy in epileptic women are described. A monotherapy should be preferred. Due to changes in the pharmacokinetics of antiepileptic drugs, plasma level checks must be performed every four weeks during early pregnancy. The possible teratogenetic effects of anticonvulsants and other factors tending to increase of malformation rate are discussed. An explanation is given for the use of prophylactic folic acid and Vitamin K treatment in connection with antiepileptic therapy. The benefits of counselling prior to pregnancy are discussed.

Once a decision to treat epilepsy has been made it is necessary to have a thorough knowledge of the benefits, disadvantages, toxic effects, pharmacokinetics and interactions associated with each anticonvulsant. However, by determining the serum concentration and manipulating it within the therapeutic range it should be possible to find a regimen that adequately controls seizures without introducing unnecessary toxic effects. There are special considerations in choosing anticonvulsants to treat epilepsy in pregnancy, febrile convulsions and status epilepticus.

A comprehensive review, evaluating 51 randomized double-blind controlled studies, covering different aspects of epileptology, is presented. An analysis of four cases of lipomas of the corpus callosum with epilepsy, and a review of the literature, have led to the following conclusions: (1) Epilepsy as an almost constant feature is often severe, nearly always partial, and begins before the age of 15. A open clinic assay with barbexaclone which is an association of phenobarvital with light stimulant of the central nervous system (tau-l-ciclohexil-2-methil-aminopropane) for the treatment of epileptic disorders is reported. The authors have studied 40 patients aged between 0-12 years of age, of both sexes, all of them having convulsive seizures of the Grand Mal type associated or not to other epileptic manifestations. After analysing the different aspects of the group they consider satisfactory the results obtained: very good or good in 82,5% of the cases.

Sodium valproate has a simple chemical structure unlike other anticonvulsants. It has been given to 63 patients with various types of epilepsy whose attacks were frequent and of whom 40 had failed to respond to other anticonvulsants. Fits stopped completely in 27 patients (43%) and 14 (22%) showed a reduction of attacks by more than 50%. Ferroptosis, an iron-dependent form of regulated cell death characterized by lipid peroxidation, has emerged as a potential contributor to neuronal injury across neurological disorders.
