Rhythmic ictal nonclonic hand (RINCH) motions: a distinct contralateral sign in temporal lobe epilepsy.

PURPOSE: To describe a new ictal sign in temporal lobe seizures-rhythmic ictal nonclonic hand (RINCH) motions and to determine its lateralizing significance and other ictal manifestations associated with it. METHODS: We identified 15 patients with temporal lobe epilepsy who demonstrated RINCH motions and reviewed video-EEG recordings of all their seizures. We analyzed the epilepsy characteristics and all clinical features of recorded seizures, with particular attention to RINCH motions. RESULTS: RINCH motions were unilateral, rhythmic, nonclonic, nontremor hand motions. RINCH motions were us

Deficient memory acquisition in temporal lobe epilepsy is predicted by hippocampal granule cell loss.

BACKGROUND: The hippocampal formation is essentially involved in the formation of conscious memories for facts and events and neurologic diseases affecting the hippocampus associate with severe memory deficits, i.e., temporal lobe epilepsies. METHODS: We studied the degree of declarative memory dysfunction in 24 human subjects with unilateral mesial temporal lobe epilepsy, using the unique possibility to access memory performance of each isolated hippocampus by intracarotid amobarbital anesthesia. Subsequently, hippocampal specimens from the same patients were available for neuropathologic an

Hippocampal pathology in refractory temporal lobe epilepsy: T2-weighted signal change reflects dentate gliosis.

BACKGROUND: The MR and pathologic features of hippocampal sclerosis (HS) are well described and include volume decrease and T2-weighted signal increase for MRI, and neuron cell loss and gliosis for pathology. OBJECTIVE: To confirm the established correlation between hippocampal volumes and neuron cell counts, and to study the still controversial association between signal change and gliosis. METHODS: The authors studied 44 patients (22 men and 22 women; mean age at surgery, 37 years) with refractory temporal lobe epilepsy. Quantitative assessment of hippocampal volumes and T2 relaxometry, an

Interictal cardiac autonomic dysfunction in temporal lobe epilepsy demonstrated by [(123)I]metaiodobenzylguanidine-SPECT.

We studied the post-ganglionic cardiac sympathetic innervation in patients with chronic temporal lobe epilepsy (TLE) by means of [(123)I]metaiodobenzylguanidine-single photon computed tomography (MIBG-SPECT) and evaluated the effects of carbamazepine on cardiac sympathetic innervation. TLE is frequently associated with dysfunction of the autonomic nervous system. Autonomic dysregulation might contribute to unexplained sudden death in epilepsy. Anticonvulsive medication, particularly with carbamazepine, might also influence autonomic cardiovascular modulation. MIBG-SPECT allows the quantificati

Mild non-lesional temporal lobe epilepsy. A common, unrecognized disorder with onset in adulthood.

OBJECTIVE: To compare mild vs. severe non-lesional temporal lobe epilepsy (TLE). METHODS: Data from 104 consecutive patients with non-lesional TLE were reviewed. Seventy-three of the 104 fulfilled the criteria for inclusion in this study of a follow-up period longer than three years at our Institute. Patients were considered to have a mild TLE if they were seizure free for at least three years after appropriate antiepileptic medication, or had rare (< or = 2/year) complex partial or secondarily generalized seizures for at least three years with or without appropriate antiepileptic therapy. Interictal 99Tc(m) HMPAO SPECT and 1H MRS in children with temporal lobe epilepsy.

PURPOSE: To understand the pathological basis of focal hypoperfusion seen on interictal 99Tc(m) hexamethylpropyleneamine oxime (HMPAO) single-photon-emission computed tomography (SPECT) in intractable temporal lobe epilepsy, and to determine why the technique may be misleading in the localization and lateralization of the seizure focus in some cases. METHODS: Interictal 99Tc(m) HMPAO SPECT and proton magnetic resonance spectroscopy (1H MRS) of the mesial temporal regions were performed in 14 children with intractable temporal lobe epilepsy not caused by a foreign tissue lesion. RESULTS: Hypo

Pure amnestic seizures in temporal lobe epilepsy. Definition, clinical symptomatology and functional anatomical considerations.

Pure amnestic seizures (PAS) sometimes occur in patients with temporal lobe epilepsy. They never represent the only type of seizures in these patients. Pure amnestic seizures are defined as seizures during which the only clinical manifestation is the patients' inability to retain in memory what occurs during the seizure coupled with the preservation of other cognitive functions and the ability to interact normally with their physical and social environment. It is postulated that PAS result from selective ictal inactivation of mesial temporal (MT) structures without isocortical involvement. Thi

Seizure Freedom After Epilepsy Surgery and Higher Baseline Cognition May Be Associated With a Negatively Correlated Epilepsy Network in Temporal Lobe Epilepsy.

Background: Brain regions positively correlated with the epileptogenic zone in patients with temporal lobe epilepsy vary in spread across the brain and in the degree of correlation to the temporal lobes, thalamus, and limbic structures, and these parameters have been associated with pre-operative cognitive impairment and seizure freedom after epilepsy surgery, but negatively correlated regions have not been as well studied. We hypothesize that connectivity within a negatively correlated epilepsy network may predict which patients with temporal lobe epilepsy will respond best to surgery. Method

Astrocytic Igfbp2 Promotes Spontaneous Seizures in a Mouse Model of Mesial Temporal Lobe Epilepsy.

Mesial temporal lobe epilepsy (MTLE) is a common, frequently drug-resistant epilepsy characterized by seizures arising from the hippocampus. Its hallmark pathology is hippocampal sclerosis with neuronal loss and reactive astrogliosis. Although astrocytes have emerged as potential targets for antiepileptic therapies, their role in epilepsy development remains poorly defined. Here, we combined adeno-associated virus (AAV)-mediated labeling with translating ribosomal affinity purification (TRAP) to generate astrocyte-enriched transcriptome profiles from sclerotic hippocampal regions in a mouse mo

An rs-fMRI based neural marker for MRI-negative temporal lobe epilepsy with depression.

OBJECTIVE: Depression is the most common comorbidity in epilepsy. Currently, the diagnosis of comorbid depression in epilepsy primarily relies on medical history and scales. However, this approach is highly subjective and heavily dependent on the physician's experience, and prone to missed or misdiagnosis. The primary objective of this study was to evaluate the effectiveness of network homogeneity (NH) measurements analyzed via support vector machine (SVM) in diagnosing MRI-negative temporal lobe epilepsy with depression (MRI-negative TLED). METHODS: The study included a total of 217 particip

Temporal lobe epilepsy is associated with neuroinflammation, extracellular matrix remodeling, and synaptic protein alterations.

INTRODUCTION: Temporal lobe epilepsy is the most prevalent form of drug-resistant focal epilepsy and is frequently associated with neuronal cell loss and astrogliosis in the hippocampus, i.e. hippocampal sclerosis (HS). METHODS: In this study, we performed mass spectrometry-based proteomic profiling of microdissected hippocampal, neocortical, and white matter tissue obtained from TLE patients and respective control samples. RESULTS: In hippocampal TLE tissue, we observed significant upregulation of proteins involved in complement system activation, extracellular matrix (ECM) organization, an

Tweak regulates glial cell activation in temporal lobe epilepsy through a positive feedback circuit.

Gliosis is a hallmark of temporal lobe epilepsy (TLE) and contributes to disease progression and cognitive deficits, yet its regulatory mechanisms remain poorly understood. Tweak (tumor necrosis factor-related weak inducer of apoptosis) has been implicated in glial activation and inflammation, but its role in TLE remains unclear. In this study, a TLE mouse model was established by intraperitoneal injection of pilocarpine. Knockdown of either Tweak or long non-coding RNA Snhg3 (small nucleolar RNA host gene 3), a lncRNA co-expressed with Tweak, alleviated glial activation, neuroinflammatory, an

Thalamocortical connectivity and nucleus-specific thalamic dysfunction in temporal lobe epilepsy: an integrative review.

Temporal lobe epilepsy (TLE) is increasingly recognized as a distributed network disorder extending beyond mesial temporal structures. Among the brain regions implicated in epileptic network organization, the thalamus has emerged as a key hub due to its extensive structural and functional connections with cortical, limbic, and subcortical systems. This integrative review aimed to synthesize current evidence regarding nucleus-specific thalamic abnormalities and thalamocortical connectivity alterations in TLE.A literature search was conducted following PRISMA guidelines using PubMed and Google Sc

How Focal Seizures Impair Consciousness: Clinical Features and Network Mechanisms in Temporal Lobe Epilepsy.

Focal impaired consciousness seizures (FICS) are among the most common seizure types in epilepsy and are a major cause of reduced QOL, occurring predominantly in temporal lobe epilepsy. Although seizure activity often remains confined to a focal temporal lobe network, patients develop widespread impairment of consciousness, indicating dysfunction of distributed brain systems rather than local seizure activity alone. Over the past two decades, converging evidence from human electrophysiological recordings, neuroimaging, and experimental animal models has substantially advanced our understanding

Explainable Deep Learning for MRI-Negative Temporal Lobe Epilepsy: Classification and Brain Region Analysis.

Background/Objectives: Deep learning has achieved remarkable success in medical image analysis; however, limited model interpretability remains a major barrier to its clinical translation. MRI-negative temporal lobe epilepsy (TLE) is characterized by the absence of readily identifiable structural abnormalities on conventional MRI. Methods: We propose a hierarchical, multiscale 3D residual network (H-MSResNet) combined with layer-wise relevance propagation (LRP). The study included structural T1-weighted MRIs from 101 patients with MRI-negative TLE and 101 healthy controls. Model classification

Theta oscillations, epileptogenesis, and focal seizures in rodent models of mesial temporal lobe epilepsy.

Theta oscillations (4-12 Hz) are a prominent network rhythm within hippocampal and parahippocampal structures. Driven by cholinergic and γ-aminobutyric acidergic inputs originating from the medial septum, physiological theta gates network excitability to control spatial navigation, memory encoding, and sensorimotor integration during specific behavioral states. Theta rhythm also organizes the precise temporal binding of interneuron and principal cell firing via phase-locking, a mechanism critical for synaptic plasticity and long-term potentiation. Emerging evidence indicates that theta activit

Voxel-based morphometry in mesial temporal lobe epilepsy surgery: an exploratory Moroccan pilot study.

INTRODUCTION: Mesial temporal lobe epilepsy (MTLE) is a common focal epilepsy syndrome that may require surgery when seizures persist despite appropriate antiseizure medications. Although widespread gray matter (GM) abnormalities have been reported in MTLE, quantitative neuroimaging has not yet been implemented in the newly established epilepsy surgery programme in Fez, Morocco. This prospective observational exploratory pilot study evaluated the feasibility of implementing a whole-brain voxel-based morphometry (VBM) workflow using the Computational Anatomy Toolbox (CAT12), Statistical Paramet

Changes in anxiety after temporal lobe epilepsy surgery: A systematic review and meta-analysis.

BACKGROUND: Temporal lobe epilepsy (TLE) is often accompanied by psychiatric manifestations, of which, anxiety is among the most common. While the effects of surgery or ablation on TLE and seizure outcomes, as well as neuropsychological outcomes, have been well studied, anxiety is an exception and has not been systematically examined. Our study aims to systematically review the changes in anxiety before and after TLE surgery. METHODS: We systematically reviewed the existing literature on anxiety in the setting of TLE. Different electronic databases were queried for studies published until 3rd
