Blunt Vascular Injuries of the Face and Scalp: Systematic Review and Introduction of a Grading Scheme and Treatment Algorithm.

INTRODUCTION: Branches of the internal and external carotid arteries supplying the scalp and face are vulnerable to injury from blunt trauma and penetrating injuries, especially over bony structures. Blunt trauma can lead to vessel dissection, pseudoaneurysm, or transection with extravasation. These injuries often present days to months post-trauma unless accompanied by avulsion with hemorrhage. Compounding the challenge, additional injuries are rare. Unlike blunt cerebrovascular injuries, there is no standardized grading system for diagnosing blunt cephalovascular injuries. METHODS: A system

Outcome analysis of sports-related multiple facial fractures.

In this paper, we report a retrospective study of 236 patients with facial bone fractures from various sports who were treated at the Department of Plastic and Reconstructive Surgery, Inha University Hospital, Incheon, South Korea, between February 1996 and April 2007. The medical records of these patients were reviewed and analyzed to determine the clinical characteristics and treatment of the sports-related facial bone fractures. The highest frequency of sports-related facial bone fractures was in the age group 11 to 20 years (40.3%); there was a significant male predominance in all age grou

Accidental and Abusive Mandible Fractures in Infants and Toddlers.

OBJECTIVE: Mandible fractures are uncommon injuries in infants and young children and may raise concern for nonaccidental trauma. Our study describes several children with mandible fractures to identify features that might differentiate abuse from accident. METHODS: Records and imaging were reviewed for children aged 24 months and younger who were diagnosed with mandible fractures at 2 tertiary pediatric care centers. Twenty-one cases were included, 8 of whom had formal child abuse consultations. Cases were reviewed for mechanisms of injury, physical examination findings, and occult injuries

The human bite injury: a clinical audit and discussion regarding the management of this alcohol fuelled phenomenon.

BACKGROUND: Human bite injuries are both deceptive and challenging in their presentation and management. They remain a frequent presentation … as documented in 86% of cases. The majority (70%) occurred over the weekend or on a public holiday. Facial injuries made up 70% of injuries with the remainder being to the upper limb. The ear was the most common target of all facial injuries (65%). Infection was documented in 18 cases (20%), with bite injuries to the upper limb and those presenting late (>12 h) h …

Experimental model of facial paralysis by nerve compression in primates (Callithrix sp.): a new model of facial paralysis in small nonhuman primates.

CONCLUSION: We conclude that facial nerve injury induced by compression is associated with a reasonable time window (4 weeks) that allows possible neurotrophic effects to be analyzed. Additionally, there are no hints of cross-innervation by the contralateral facial nerve or parallel innervation by other nerves in the hemiface ipsilateral to the injured nerve. OBJECTIVE: The aim of this work was to develop an experimental model of facial nerve injury in nonhuman primates (Callithrix sp.). METHODS: In this study, individuals of the non-human primate species Callithrix sp. were subjected to thr

Photobiomodulation Therapy for Managing Motor and Sensory Dysfunctions Following Temporomandibular Joint Surgery: A Case Report.

Introduction: Surgical procedures involving the temporomandibular joint (TMJ) are frequently associated with nerve injuries and subsequent dysfunctions. Considering that traumatic peripheral nerve injuries may resolve slowly and their prognosis is generally unpredictable, the current study aimed to report a clinical case in which both motor (affecting the temporal and zygomatic branches of the facial nerve) and sensory dysfunctions (affecting the auriculotemporal nerve of the trigeminal nerve) following TMJ surgery were effectively treated by using photobiomodulation therapy (PBMT). Case Repor

Maxillofacial fractures and craniocerebral injuries - stress propagation from face to neurocranium in a finite element analysis.

BACKGROUND: Severe facial trauma is often associated with intracerebral injuries. So it seemed to be of interest to study stress propagation from face to neurocranium after a fistlike impact on the facial skull in a finite element analysis. METHODS: A finite element model of the human skull without mandible consisting of nearly 740,000 tetrahedrons was built. Fistlike impacts on the infraorbital rim, the nasoorbitoethmoid region, and the supraorbital arch were simulated and stress propagations were depicted in a time-dependent display. RESULTS: Finite element simulation revealed von Mises st

Deficient functional recovery after facial nerve crush in rats is associated with restricted rearrangements of synaptic terminals in the facial nucleus.

Crush injuries of peripheral nerves typically lead to axonotmesis, axonal damage without disruption of connective tissue sheaths. Generally, human patients and experimental animals recover well after axonotmesis and the favorable outcome has been attributed to precise axonal reinnervation of the original peripheral targets. Here we assessed functionally and morphologically the long-term consequences of facial nerve axonotmesis in rats. Expectedly, we found that 5 months after crush or cryogenic nerve lesion, the numbers of motoneurons with regenerated axons and their projection pattern into th

Maxillofacial trauma in children.

Pediatric trauma involving the bones of the face is associated with severe injury and disability. Although much is known about the epidemiology of facial fractures in adults, little is known about injury patterns and outcomes in children. The most common facial fractures were mandible, nasal and maxillary/zygoma. The most common mechanisms of injury are motor vehicle collisions, violence and falls. These fracture patterns and mechanisms of injury varies with age. Cranial and central facial injuries are more common among toddlers and infants, and mandible injuries are more common among adolesce…

Diagnosis and management of sports-related injuries to the face.

The face is often the most exposed part of the body during athletic competition. This article concentrates on sports-related injuries to the zygoma and periorbital area, the maxilla, the nose, and the external ear. Discussions of the management of soft-tissue injuries and the diagnosis and treatment of underlying disruption of bone and cartilage are presented. A new piece of protective athletic equipment for the prevention of facial injuries to baseball players is introduced.

Pediatric Pedestrian Facial Fracture Patterns and Management Following Motor Vehicle Collisions.

PURPOSE: Pedestrian trauma due to motor vehicle crashes can be especially destructive to the pediatric population as the facial skeleton is immature and developing. Almost half of crashes resulting in pedestrian death involved alcohol consumption, and children are often victims of irresponsible driving. The objective of this study was to examine the prevalence of facial fractures in this patient population in order to analyze management strategies that optimize functional recovery. METHODS: A retrospective chart review was performed for all facial fractures resulting from motor vehicle collis

Post-traumatic Unilateral Avulsion of the Abducens Nerve with Damage to Cranial Nerves VII and VIII: Case Report.

Traumatic injuries of the abducens nerve as a consequence of facial and/or head trauma occur with or without associated cervical or skull base fracture. This is the first report on unilateral avulsion of the abducens nerve in a 29-year-old man with severe right facial trauma. In addition, he exhibited mild left facial palsy, and moderate left hearing disturbance. Magnetic resonance imaging (MRI) using fast imaging employing steady-state acquisition (FIESTA) revealed avulsion of left sixth cranial nerve. We recommend thin-slice MR examination in patients with abducens palsy after severe facial

Anxiety and self-consciousness in patients with minor facial lacerations.

BACKGROUND: Although minor facial injuries are relatively common, their psychological impact is an area neglected in the literature. For physiologically major injuries (such as facial cancers, burns and fractures), the face has been suggested to be a psychologically significant area of the body and disfigurement has been found to have numerous potential social consequences for patients. AIMS: This paper reports the findings of an inquiry that explored the psychological impact of minor facial injuries and the influence of patient and scar characteristics in relation to self-consciousness and a…

Blindness after facial trauma.

Blindness after facial fractures has been reported to occur with an incidence that ranges between 0.67% and 3% depending on the reporting institution. To verify this finding we undertook a retrospective chart review of 5936 patients with facial fractures that occurred over a 12 1/2-year period. We found that vision in 19 eyes were lost in 18 patients. Vision loss was more frequently encountered in Le Fort III level fractures (2.2%) followed distantly by Le Fort II level fractures (0.64%), and zygomatic fractures (0.45%). The cause of blindness was most frequently associated with motor vehicle

Parturitional injury of the head and neck.

Parturitional injuries refer to injuries sustained during and secondary to fetal delivery. The skull, brain, and head and neck regions are frequently involved. Accurate differentiation and classification of the various injuries is essential for treatment, prognosis, and parental counseling. In this review, we discuss the various "bumps and lumps" that maybe encountered along the neonatal skull as well as the most frequent calvarial and intracranial parturitional injuries. In addition, a short discussion of the most common head and neck, facial, and spinal lesions is included. Various mimickers

[Epidemiology of the mild and moderate brain injuries].

Unlike severe head injuries upon which much attention is focused, mild or moderate brain injuries are not or not well known and often neglected in maxillofacial surgery. Nevertheless, they are frequent in our routine facial traumatology activity. Studying the epidemiological data shows that mild brain injuries account for 64 to 94% of all head injured patients whereas moderate injuries occur in 3 to 23%. Furthermore, owing to their frequent occurrence, their high potential for sequelae and because they especially affect young people, they are a real public-health problem.

Traumatic asphyxia compounding craniofacial trauma.

Traumatic asphyxia is a condition characterized by cervicofacial petechiae, subconjunctival ecchymosis and other possible accompanying problems particularly ophthalmic, thoracic and cardiovascular injuries. While the facial petechiae per se are of little consequence, the attendant injuries to other organ systems can be serious and even fatal. Craniofacial injuries also present with fatal ecchymosis and haematoma formation due to the underlying skeletal injuries which may require active management. Clinicians managing trauma patients should be well aware of these two different entities which ma…
