Midfacial necrotizing lesions.

Because of obvious therapeutic and prognostic implications, as definitive a pathologic diagnosis as possible is necessary for necrotizing lesions of the midfacial mucous membranes. In the beginning of the diagnostic process, the lesions are placed under the noncommittal rubric of midfacial necrotizing lesions, thus forever avoiding confusing nomenclature such as lethal midline granuloma. Following this, the lesions are approached with the full appreciation they may be (1) localized or part of a systemic illness; (2) neoplastic or non-neoplastic; (3) caused by a specific agent or organism or be

Staging of squamous cell carcinoma of the tongue: extrinsic lingual muscles and the 8th editions of the American Joint Committee on Cancer/Union for International Cancer Control staging manuals.

Our aim was to find out first whether the extrinsic muscles of the tongue are histologically identifiable, and secondly to what degree the use of the new criteria in the 8th editions of the American Joint Committee on Cancer(AJCC)/Union for International Cancer Control (UICC) manuals (which have recognised the importance of depth of invasion of tumour, rather than invasion of the extrinsic muscles of the tongue and extranodal extension), will alter staging of lingual squamous cell carcinoma (SCC). The histological sections from 165 patients who had had primary resection of lingual SCC were rev

[Treatment of 48 cases of benign lower posterior tumor of the parotid gland with C-shaped small incision].

PURPOSE: To summarize the feasibility and advantages of C-shaped small incision for the treatment of benign lower posterior tumor of the parotid gland. METHODS: Forty-eight cases of benign lower posterior tumor of the parotid gland were treated with C-shaped small incision. The treatment outcomes were recorded and analyzed. RESULTS: The wound healed primarily in all 48 cases. The follow-up period was 1-5 years, and there was no recurrence of tumor and the shape of parotid region recovered well. The residual parotid tissue had certain secretory functions postoperatively. CONCLUSIONS: C-shape

Brooke-Spiegler syndrome.

A 50-year-old woman presented with a long history of multiple, flesh-colored papules and nodules on her central face, neck, and upper shoulders, as well as pink papules on her scalp. They were neither pruritic nor tender, but the patient desired treatment for cosmesis. Histopathologic examination of a representative facial lesion was consistent with a trichoepithelioma and an upper forehead papule was consistent with cylindroma. She reported a positive family history of similar lesions.

Osseous reconstruction of the midface and orbits.

The midface or orbitonasomaxillary complex is an intricate three-dimensional entity with important functional and aesthetic purpose. Bony reconstruction of defects caused by resection of malignancy, congenital abnormalities, or trauma requires close attention to both function and the complex anatomy. Replacement of bone should be done in the functional anatomic position. Free bone grafts require a well-vascularized bed and may require adjuvant soft-tissue reconstruction prior to their placement. Vascularized bone through the temporoparietal fascial flap and free tissue transfer provides an exc

Metastatic melanoma to the facial nerve.

Isolated metastatic malignant melanoma to the facial nerve has never been reported. This presentation illustrates a primary melanoma of the helix of the ear that was treated by excisional biopsy and then wedge resection in 1983. The primary melanoma was Clark's level IV and 1.3 mm in thickness. In 1985, a facial paresis slowly developed. There was no gross evidence of recurrent melanoma in the ear or neck, but CT scan showed a mass in the region of the stylo mastoid foramen. A reoperation of the primary site revealed metastatic melanoma in the facial nerve, expanding it to approximately 10 tim

[The special problems of lymphangioma perioculare and hemifaciale (author's transl)].

The authors describe two cases of the rare disease lymphangioma hemifaciale. Every ophthalmologist should know this disease in view of the complications such as hemorrhage or erysipelas. Treatment methods are discussed. Furthermore, lymphangioma must be taken into consideration in the differential diagnosis of ptosis, protrusio bulbi, protrusio conjunctivae and congenital hemifacial hypertrophy.

Two cases of localized B-cell lymphoma in the acquired immunodeficiency syndrome.

We report 2 cases of localized B-cell non-Hodgkin lymphoma occurring on the face in AIDS patients. The tumors were limited to the skin and bone, without other involvement. They were characterized by an aggressive local behavior that caused infectious and compressive complications and resulted in fatal outcome. Knowledge of the possibility that lymphoma may remain localized in AIDS patients should prompt clinicians to perform a biopsy as soon as possible, regardless of the CD4 count. Indeed, early diagnosis might increase the efficacity of treatment and might avoid disastrous local complication

Perineural Invasion in Cutaneous Squamous Cell Carcinoma.

Cutaneous squamous cell carcinoma is sometimes characterized by an increased risk of locoregional recurrence and occasionally distant metastasis. Several clinical and pathological factors, including perineural invasion, have been shown to have prognostic value in this setting. Perineural invasion, that is, the spread of tumor cells into the space surrounding a nerve, is usually an incidental finding. In the presence of symptoms or radiographic evidence of perineural spread, the diagnosis is clinical perineural invasion, which is associated with an increased risk of local recurrence and mortali

[Precancerous and early invasive carcinomas: non-surgical treatment of head and facial skin].

Chronic exposure to sunlight with its high proportion of high energy ultraviolet light is the main cause of the common cutaneous precancerous lesions and carcinomas of the head and neck. This causes a field cancerization effect frequently with multiple actinic keratoses (AKs), basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs). Although demonstrating the best cure rates and lowest recurrence rates, surgical excision rapidly progresses to its limits. Field cancerization requires field therapy. Non-specific caustic agents may remove superficial lesions, however, the modern therapeu
