Analysis of fatigue development during elite male handball matches.

This study aimed at describing the physiological demands and fatigue development during elite male handball matches. Our hypothesis was that players perform multiple high-intensity activities during periods of the game and develop temporary and end-match neuromuscular fatigue. Time-motion analyses and heart rate (HR) recordings were performed in 40 players during 12 competitive matches. Blood samples were collected, and sprint, jump, and intermittent exercise performance (Yo-Yo Intermittent Endurance level 2 test [YYIE2]) was assessed for 18 players at baseline conditions and after 2 competiti…

Protracted exercise without overt neuromuscular fatigue influences cortical excitability.

The authors' aim was to determine the cortical mechanisms that underlie the transition from effective performance to its disruption. They thus used transcranial magnetic stimulation (TMS) to study changes of corticospinal excitability after a motor exercise that did not produce overt or perceived neuromuscular fatigue. Forty-four subjects performed either 5 or 10 min of repetitive finger movements paced by tones at 2 Hz, a frequency below the spontaneous movement rate. Changes of corticospinal excitability were assessed with TMS at rest and during motor response preparation (premovement facili…

The Copenhagen Soccer Test: physiological response and fatigue development.

INTRODUCTION: The aims of the study were 1) to evaluate whether a multifaceted simulated soccer game protocol, entitled the Copenhagen Soccer Test (CST), elicited a similar physiological loading as a competitive game (CG) and 2) to determine muscle metabolites, blood variables, and sprint performance in various phases of CST. METHODS: Twelve Danish Second- and Third-Division soccer players participated in the study. On separate days, HR measurements, frequent blood sampling, and physical/technical tests were performed during 60- and 90-min versions of the CST during which repeated musculus va

Supraspinal fatigue after normoxic and hypoxic exercise in humans.

Inadequate cerebral O₂ availability has been proposed to be an important contributing factor to the development of central fatigue during strenuous exercise. Here we tested the hypothesis that supraspinal processes of fatigue would be increased after locomotor exercise in acute hypoxia compared to normoxia, and that such change would be related to reductions in cerebral O₂ delivery and tissue oxygenation. Nine endurance-trained cyclists completed three constant-load cycling exercise trials at ∼80% of maximal work rate: (1) to the limit of tolerance in acute hypoxia; (2) for the same duration b…

Fatigue in ankylosing spondylitis: causes, consequences and self-management.

OBJECTIVES: To explore the perceived causes, consequences and management of fatigue in ankylosing spondylitis (AS). METHODS: In this single-cohort qualitative study, ten participants completed a seven-day diary and participated in a semi-structured interview exploring possible causes, consequences and self-management of fatigue. Interviews were recorded, transcribed and analysed using 'framework' analysis. Ethical approval was obtained. RESULTS: Three themes emerged: patterns and causes, consequences and management of fatigue. While patients' descriptions of fatigue were similar, the pattern…

Metal fatigue causing cystoscope rupture during bladder neck incision.

The modern cystoscope is the result of the advancement in technology in numerous areas and is an invaluable tool that allows the urologist to perform a number of diagnostic and therapeutic procedures. Although various degrees of endoscope failure have been widely reported, instrument breakage that leads to a foreign body has not. While performing a bladder neck stricture incision for a 72-year-old male patient with a previous radical prostatectomy for prostate cancer and a resulting bladder neck stricture, we documented a major 17-French cystoscope malfunction and a resulting foreign body that

Diaphragm fatigue after submaximal exercise with chest wall restriction.

PURPOSE: We asked whether submaximal exercise with chest wall restriction (CWR), as a model of restrictive pulmonary disease, would result in fatigue of the diaphragm in healthy humans. METHODS: To address this question, we used cervical magnetic stimulation of the phrenic nerves along with measures of transdiaphragmatic pressure. Seven healthy young men (30 ± 7 yr) completed pulmonary function tests and a maximal cycle exercise test. On a separate day, baseline measures of diaphragm contractility were obtained followed by cycle exercise at 45% of maximum intensity for 10 min with no restrict…

Quadriceps and hamstrings fatigue alters hip and knee mechanics.

Neuromuscular fatigue exacerbates abnormal landing strategies, which may increase noncontact anterior cruciate ligament (ACL) injury risk. The synergistic actions of quadriceps and hamstrings (QH) muscles are central to an upright landing posture, though the precise effect of simultaneous fatigue of these muscles on landing and ACL injury risk is unclear. Elucidating neuromechanical responses to QH fatigue thus appears important in developing more targeted fatigue-resistance intervention strategies. The current study thus aimed to examine the effects of QH fatigue on lower extremity neuromecha…

Mechanisms by which pharmacologic agents may contribute to fatigue.

Fatigue is a common side effect of medications. This review summarizes some of the mechanisms by which drugs may cause fatigue. One major mechanism by which medications can cause fatigue is by central nervous system (CNS) depression. CNS depression can result from decreased excitatory activity, as is commonly seen with some anticholinergic agents, centrally acting alpha-agonists, and anticonvulsants. CNS depression can also result from increased inhibitory activity within the CNS, as is seen with benzodiazepines and barbiturates, which augment the effects of the inhibitory neurotransmitter, ga…

Fatigue factors affecting metropolitan bus drivers: a qualitative investigation.

Metropolitan bus drivers daily face work in a stressful and draining work environment, exposing them to the serious risk of driver fatigue. However, there has been a dearth of information exploring the unique antecedents and effects of such fatigue. To date, much of the research into metropolitan bus drivers has been under the umbrella of large heavy vehicle driving studies, which include a disproportionally large population of long-haul drivers, who are likely to face a significantly different set of fatigue factors [1]. The present study aimed to investigate which work and environmental fact…

Fatigue in a simulated cross-country skiing sprint competition.

The aim of this study was to assess fatigue during a simulated cross-country skiing sprint competition based on skating technique. Sixteen male skiers performed a 30-m maximal skiing speed test and four 850-m heats with roller skies on a tartan track, separated by 20 min recovery between heats. Physiological variables (heart rate, blood lactate concentration, oxygen consumption), skiing velocity, and electromyography (EMG) were recorded at the beginning of the heats and at the end of each 200-m lap during the heats. Maximal skiing velocity and EMG were measured in the speed test before the sim…

Small heart syndrome in patients with chronic fatigue syndrome.

BACKGROUND: Small heart syndrome has previously been reported as neurocirculatory asthenia, associated with a small heart shadow on a chest roentgenogram. This is characterized as weakness or fatigue even after ordinary exertion, palpitation, dyspnea, and fainting, resembling patients with chronic fatigue syndrome (CFS). HYPOTHESIS: Small heart syndrome may be prevalent in patients with CFS. METHODS: The study population consisted of 56 patients (<50 y of age) with CFS, and 38 control subjects. Chest roentgenographic, echocardiographic, and physical examinations were performed. RESULTS: Sma…

Effects of fatigue on intermuscular coordination during repetitive hammering.

Fatigue affects the capacity of muscles to generate forces and is associated with characteristic changes in EMG signals. It may also influence interjoint and intermuscular coordination. To understand better the global effects of fatigue on multijoint movement, we studied movement kinematics and EMG changes in healthy volunteers asked to hammer repetitively. Movement kinematics and the activity of 20 muscles of the arm, trunk, and leg were recorded before and after subjects became fatigued (as measured using a Borg scale). When fatigue was reached, maximal grip strength and elbow range of motio…

Why does chronic heart failure cause breathlessness and fatigue?

Traditional explanations for the symptoms of fatigue and breathlessness experienced by patients with chronic heart failure (CHF) focus on how reduced cardiac output on exercise leads to impaired skeletal muscle blood supply, thus causing fatigue, and on how the requirement for a raised left ventricular filling pressure to maintain cardiac output results in reduced pulmonary diffusion owing to interstitial edema, thus causing breathlessness. However, indices of left ventricular function relate poorly to exercise capacity and symptoms, suggesting that the origin of symptoms may lie elsewhere. Th…

Independence of exercise-induced diaphragmatic fatigue from ventilatory demands.

Exercise-induced diaphragmatic fatigue (DF) manifests after - rather than during - exercise. This suggests that DF reflects post-exercise diaphragm-shielding. This study tested the physiological hypothesis that diaphragmatic force-generation undergoes similar regulations during either whole-body-exercise or controlled hyperventilation, but differs during recovery. Ten trained subjects (VO2(max) 60.3+/-6.4 ml/kg/min) performed: I, cycling exercise (maximal workload: 85% VO2(max)); II, controlled hyperventilation (exercise breathing pattern) followed by recovery. Ergospirometric data and twitch …

Chronic fatigue syndrome: inflammation, immune function, and neuroendocrine interactions.

Investigations into the underlying cause of chronic fatigue syndrome have advanced the field considerably in the past year. Gene microarray data have led to a better understanding of pathogenesis. Recent research has evaluated genetic signatures, described biologic subgroups, and suggested potential targeted treatments. Acute viral infection studies found that initial infection severity was the single best predictor of persistent fatigue. Genomic studies showed that persistent cases express Epstein Barr virus-specific genes and demonstrate abnormalities of mitochondrial function. Studies of im…
