Anorectal infection: abscess-fistula.

Anorectal abscess and fistula are among the most common diseases encountered in adults. Abscess and fistula should be considered the acute and chronic phase of the same anorectal infection. Abscesses are thought to begin as an infection in the anal glands spreading into adjacent spaces and resulting in fistulas in ~40% of cases. The treatment of an anorectal abscess is early, adequate, dependent drainage. The treatment of a fistula, although surgical in all cases, is more complex due to the possibility of fecal incontinence as a result of sphincterotomy. Primary fistulotomy and cutting setons …

Advancing the predictive accuracy of PNTML in rectal prolapse: An ongoing quest.

Fecal incontinence is a common symptom among patients with rectal prolapse. Pudendal nerve terminal motor latency (PNTML) testing can serve as a reference indicator for predicting the outcomes of rectal prolapse surgery, thereby assisting surgeons in formulating more appropriate surgical plans. The direct correlation between preoperative PNTML testing results and postoperative fecal incontinence in patients with rectal prolapse remains a contentious issue, necessitating further clarification. Thus, we analyze the existing publications from both clinical and statistical perspectives to comprehe…

The role of the Malone antegrade continence enema (MACE) in the management of myelodeysplatic patients.

Myelodysplasia is a congenital neural tube defect commonly affecting bladder and bowel function. Management of fecal incontinence is paramount to achieve patient independence and self-confidence. When conservative measures fail alternative invasive methods can be successfully applied. The Malone antegrade continence enema (MACE) was introduced in the late 1980's to treat spina bifida patients with fecal incontinence refractory to conservative management. Since its introduction, multiple successful variations have been described and its role has expanded. Indeed, the MACE has revolutionized the…

Anal sphincter function as assessed by 3D high definition anorectal manometry.

PURPOSE: High resolution anorectal manometry has been developed over the past years, as well as 3D high definition manometry (HDARM). However, the clinical impact of the results obtained with these new technologies remains to be determined. We thus analyzed various HDARM parameters of anal sphincter function and tested their capacity to discriminate between patients with constipation and those with fecal incontinence. METHODS: One hundred and fourteen consecutive patients underwent the same HDARM protocol (Medtronic), including 2 short duration voluntary anal contractions (5seconds) and 1 sus…

[Artificial bowel sphincter ten years later].

INTRODUCTION: Congenital anorectal malformation is the most common cause of fecal incontinence in children and young adults. Surgical treatment options are limited. One of the treatment methods is the implantation of an artificial bowel sphincter. The goal of this study was to investigate the efficacy of the artificial bowel sphincter in reaching long-term fecal continence in patients with anorectal malformation. METHODS: Young adults with fecal incontinence due to anorectal malformation were included in the study. Those who underwent an artificial bowel sphincter implantation were followed e…

Intermittent parasympathetic symptoms in lumbar spinal stenosis.

Symptoms related to autonomic nervous dysfunction are uncommon in spinal stenosis. Involvement of nerve roots S2-S5 has previously been reported only in a few cases. Of great interest is the occurrence of phenomena such as intermittent penile erections and fecal incontinence on walking in patients with compromise of the lower lumbar spinal canal. We report on two patients, one of whom presented because of fecal incontinence and the other with penile erections on walking. In both cases, these manifestations resolved after decompressive laminectomy. These patients are compared to six other patie…

The Typical Triad of Idiopathic Normal Pressure Hydrocephalus in a 62-Year-Old Male.

Normal pressure hydrocephalus (NPH) is a rare pathological condition of the brain in which the ventricles are enlarged due to cerebrospinal fluid accumulation and is associated with normal opening pressure on lumbar puncture with a large-volume cerebrospinal fluid (CSF) tap. This results in three classical symptoms: mental impairment, gait disturbance, and urinary or fecal incontinence. We present a case of idiopathic NPH in which a 64-year-old retired man with diabetes was brought to the emergency department after recurrent previous falls. The patient complained of an unsteady gait and presen…

A randomized, controlled trial of transanal irrigation versus conservative bowel management in spinal cord-injured patients.

BACKGROUND & AIMS: Bowel dysfunction in patients with spinal cord injury often causes constipation, fecal incontinence, or a combination of both with a significant impact on quality of life. Transanal irrigation improves bowel function in selected patients. However, controlled trials of different bowel management regimens are lacking. The aim of the present study was to compare transanal irrigation with conservative bowel management (best supportive bowel care without irrigation). METHODS: In a prospective, randomized, controlled, multicenter trial involving 5 specialized European spinal cord…

Artificial sphincters: An overview from existing devices to novel technologies.

Artificial sphincters (ASs) are used to replace the function of the biological sphincters in case of severe urinary and fecal incontinence (UI and FI), and gastroesophageal reflux disease (GERD). The design of ASs is established on different mechanisms, e.g., magnetic forces or hydraulic pressure, with the final goal to achieve a implantable and durable AS. In clinical practice, the implantation of in-commerce AS is considered a reasonable solution, despite the sub-optimal clinical outcomes. The failure of these surgeries is due to the malfunction of the devices (between 46 and 51%) or the sid…

Prevention and management of moisture-associated skin damage.

Disruption to the integrity of the skin can reduce patient wellbeing and quality of life. A major cause of skin breakdown is prolonged exposure to moisture, but this is often overlooked. When skin is wet, it becomes more susceptible to damage from friction and shearing forces, and skin flora can penetrate the disrupted barrier, causing further irritation and inflammation. If untreated, moisture-associated skin damage (MASD) can rapidly lead to excoriation and skin breakdown. MASD includes incontinence-associated dermatitis (IAD), which is caused by prolonged skin exposure to urine and stool, p

Incontinence-Associated Dermatitis: Pathogenesis, Contributing Factors, Prevention and Management Options.

Incontinence-associated dermatitis (IAD) is a common type of irritant contact dermatitis, seen in patients with urinary or faecal incontinence. Mechanical factors such as traumata and friction may aggravate the lesions. The fragile skin in elderly patients is more prone to developing IAD. The clinical picture of IAD consists of persistent redness on the one hand and skin loss on the other. Both categories may be associated with clinical signs of major colonisation or infection. It is important to distinguish IAD from pressure ulcers and other dermatoses in the genital region. Due to the lack o

Prevention and management of incontinence-associated dermatitis.

Maintaining skin integrity in people with incontinence is challenging. Incontinence is common in older people and those compromised by medical or surgical comorbidities. Urinary and faecal incontinence can result in skin breakdown, which is characterised by erosion of the epidermis, creating a moist, macerated appearance to the skin. Incontinence and associated skin breakdown can have a considerable effect on an individual's physical and psychological wellbeing. This article discusses the aetiology of incontinence-associated dermatitis and considers the best options for the prevention, managem
