A prospective study was undertaken to determine the incidence of endotracheal tube contamination by malignant cells in patients with head and neck cancer undergoing bronchoscopy. In 67 consecutive patients, bronchoscopy was performed with a flexible instrument, through or beside the endotracheal tube. Following the procedure, the inside and outside of the tube were washed and the specimens examined cytologically. Fifteen positive (class III to V) cytologic specimens were obtained, all from the outside of the tube. All washings from inside the tube were negative (class I and II). Twelve of the

A flexible fibreoptic laryngoscope was used to facilitate the passage of a long polyvinyl chloride tracheal tube into the left main bronchus during repair of a tracheo-oesophageal fistula situated near the carina in a patient in whom intubation with a double-lumen endobronchial tube was difficult. Close monitoring of neuromuscular transmission using a peripheral nerve stimulator also contributed to the successful anaesthetic management of this patient.

Fiberoptic endoscopy is an important diagnostic modality for evaluation of the patient with upper gastrointestinal (GI) tract symptoms following gastric bypass and gastroplasty. During a 3-year period, 182 patients underwent gastric partitioning procedures and 22 patients (12%) developed upper GI symptoms requiring endoscopic evaluation. Eight patients had undergone Mason vertical banded gastroplasty, 12 patients had undergone Gomez gastroplasty, and two patients had undergone Roux-en-Y gastric bypass. In four of five patients with abdominal pain, gastritis of the proximal pouch was observed.

Tracheo-bronchial ruptures are serious complications of thoracic trauma. The authors report their experience of 18 cases: 3 tracheal ruptures, 3 tears in the tracheal or bronchial membrane, 11 unilateral bronchial ruptures and 1 bilateral bronchial rupture. Acute epiglottitis may be more common in adults than is generally believed, but the diagnosis is often missed. Three cases of adults with epiglottitis are reported. The presenting signs and symptoms included sore throat, dysphagia, symptoms disproportionate to pharyngeal findings, and tenderness over the neck anteriorly. The results of 2500 upper gastrointestinal fibreoptic endoscopies carried out at Soba University Hospital, Khartoum, between January, 1980, and June, 1982, were reviewed. 42% of the patients had no disorder visible on endoscopy, 9% had oesophageal varices, 0.7% gastric ulcer, 17% duodenal ulcer, and 2% pyloric obstruction. Duodenal ulcer was seen more commonly in men (male/female ratio 4.5/1) and in young people. Endoscopy is a feasible procedure in developing countries. It is more informative and cheaper than barium meal radiology.

Upper gastrointestinal fiberendoscopy in pediatric patients is done safely and under local anesthesia in most instances. This study of 47 children confirmed the value of fiberendoscopy in establishing the etiology of upper gastrointestinal hemorrhage and the presence of esophageal varices. It also contributed significantly to the management of patients with disphagia, pyrosis, epigastric pain, and ingestion of foreign bodies. No significant morbidity was caused.

The authors have analyzed 25 observations of patients with the Mallory-Weiss syndrome which made up 3,04% of all the patients with gastroduodenal bleedings. The authors emphasize the role of the endoscopic method for examining the patients to diagnose this pathology. The most rational method is believed to be the conservative treatment. When it fails the surgery should be organ-preserving. Twenty four patients were treated conservatively, one patient was subjected to gastrotomy and suturing of the mucosa fissure. All the patients recovered.

Endoscopic gastroelectromyograms were obtained from 8 patients with gastric cancer, which was located at the gastric antrum. Nineteen patients with suspected coeliac disease were examined by duodenoscopy and biopsy. In all patients a histopathology consistent with coeliac disease was demonstrated in endoscopic biopsies of the duodenal mucosa. On close-up view villous atrophy was seen in vivo. Fourteen patients were re-examined on a gluten-free diet. In 13 patients a restitution of the duodenal mucosa was shown. Duodenoscopy combined with biopsy seems to be a reliable method for the assessment of the mucosal change in coeliac disease.

Cancer of the ampula of Vater occurs in 10% of cases of jaundice caused by malignant neoplasms.
