A 56-year-old female patient suffered a bimalleolar ankle fracture with an additional proximal fibular fracture. This is an unusual fracture type, seldom reported in literature. It was operatively treated by open reduction and internal fixation of the lateral malleolar fracture. The proximal fibular fracture was initially not found. Pathogenesis of the fracture seems most likely to be a pronation-eversion trauma, with an additional direct proximal fibular fracture. By reviewing this unusual case, it becomes clear that thorough examination of a trauma patient is very important. Secondary survey

Massive segmental bony defects in open tibial fractures are generally treated with conventional bone grafting, a free vascularised fibular graft or the ring fixator technique. Over a ten-year period, 485 patients suffering from malleous fracture were operated at the Department for Orthopaedic Surgery and Traumatology in Novi Sad. Postoperative infection appeared in ten cases (2.06%). Eight of these patients primarily had open fractures, and two of them had closed ones. BACKGROUND: The irreducible trimalleolar fracture case we describe is different from the Bosworth fracture, defined as a fixed posterior fracture-dislocation of the distal part of the fibula, in which the proximal fibular shaft fragment locks behind posterior tibial tubercle.
