Open Reduction Internal Fixation With Transverse Volar Plating for Unstable Proximal Interphalangeal Fracture-Dislocation: The Seatbelt Procedure.

Background: Unstable intra-articular proximal interphalangeal (PIP) joint fracture-dislocations present a difficult problem that requires congruous joint reduction and stable internal fixation or distraction. Though fractures with limited articular involvement may be treated successfully with less invasive procedures, fracture-dislocations with a volar shear component may benefit from joint reduction with subchondral support for maintenance of stability. The purpose of this article is to describe a volar transverse plate and screw technique and report the short-term postoperative results. Meth

[Palmar dislocation of the proximal interphalangeal joint and traumatic boutonnière deformity].

Injury to the central slip of the extensor tendon may occur with open and also with closed injuries, such as volar dislocation of the proximal interphalangeal (PIP) joint. For adequate treatment, it is necessary to identify all injured structures. Without appropriate primary management, the patient is likely to develop a subacute to chronic posttraumatic boutonnière deformity. A fixed boutonnière deformity requires recovery of joint mobility. Once joint mobility is achieved, secondary surgical reconstruction of the central slip can be performed with a tendon transfer or a tendon transplant.

[Proximal interphalangeal joint dorsal dislocations without fractures--four cases report].

Four patients with proximal interphalangeal joint (PIP) dorsal dislocation without fracture had been treated at Chang Gung Memorial Branch (Kaohsiung and Keelung) Hospital from June 1987 to Jan. 1989. All four cases were open dislocations, only one patient received immediate diagnosis and treatment, while the other 3 cases were delayed diagnosis and treatment. Open reduction, repair of the ruptured ligaments and joint capsule, reinsertion of the disrupted volar plate were given in these 4 patients. The patient with immediate surgical treatment received rehabilitation program, the other three p

Irreducible volar dislocation of the proximointerphalangeal joint.

Volar dislocations of the proximointerphalangeal joint are not injuries to be treated lightly. If the dislocation reduces easily, a tear of the central tendon would be indicated and immediate open repair of this structure then becomes mandatory. If the dislocation is irreducible, then a tear of the collateral ligament with button-holing of the head of the proximal phalanx through the interval between the central tendon and the lateral band should be suspected, and open reduction and replacement of these structures in their normal position is indicated. The results after open repair of irreduci

Brachymesophalangy and loose bodies in the metacarpophalangeal joints.

A family with autosomal inherited brachymesophalangy is presented. Some of the family members also had loose bodies in the metacarpophalangeal joints. This condition is similar to osteochondritis in other joints. New loose bodies may be formed after operative removal and arthrotic changes may occur. The patients were not able to perform hard physical work with their hands.

A case of occupational gout.

Gout can occur in any joint but most commonly afflicts the first metatarsophalangeal joint. Many theories have been suggested as explanations for this preferential joint involvement. We describe a case of gout occurring in the fingers of a man with occupational cold exposure, a case of "occupational" gout, and support for the involvement of temperature in acute attacks of gout.

Isolated Dupuytren's Disease in Proximal Phalanx of the Little Finger Mimicking Giant-Cell Tumor: A Rare Case Presentation.

Background and Clinical Significance: Dupuytren's disease (DD) is characterized by abnormal myofibroblast proliferation and excessive collagen deposition, leading to the formation of pathological fibrous cords. It typically affects the palmar surface of the hand, where these contractile cords cause progressive flexion contractures of the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints. Lesions involving the proximal interphalangeal (PIP) joint without significant flexion contracture may be misdiagnosed as soft-tissue tumors or inflammatory lesions based on imaging findings,

Outcome of flexor digitorum superficialis opponensplasty for Type II and IIIA thumb hypoplasia.

UNLABELLED: This study reports on outcomes of the flexor digitorum superficialis tendon transfer from the ring finger in Type II and IIIA hypoplastic thumbs. We included 22 patients with 27 involved hands: 16 Type II thumbs and five Type IIIA treated by transfer and six non-operated Type II thumbs. The outcomes were assessed by range of motion, strength, sensibility, joint stability and patient/parent satisfaction. Compared with normative data, the range of motion was diminished in all patient groups. Opposition strength in operated Type II thumbs was significantly better than in non-operated

Analysis of Tsuge's procedure for the treatment of radial nerve paralysis.

Radial deviation and limited flexion of the wrist joint and a lack of abduction of the thumb have been noticed after the Riordan's procedure. Therefore, Tsuge et al. modified the Riordan's procedure, and their procedure includes transfer of the pronator teres to the extensor carpi radialis brevis, the flexor carpi radialis (FCR) to the extensor digitorum communis (EDC), and the palmaris longus to the extensor pollicis longus, along with tenodesis of the abductor pollicis longus. We reviewed the charts of 21 patients with isolated radial nerve paralysis who were treated with the Tsuge's procedu

[A woman with a stepladder deformity of her little finger].

A 41-year-old woman presented to our emergency department after she tripped over a doorstep. Physical examination showed swelling and a stepladder deformity of the left little finger. Radiographs confirmed dorsal dislocation in both interphalangeal joints. Closed manual reduction under local anaesthesia restored position and function of the finger.

Percutaneous Fixation of Unstable Proximal Phalanx Fractures: A Biomechanical Study.

BACKGROUND: Unstable extra-articular proximal phalanx fractures are common injuries to the hand that are often treated by closed reduction and percutaneous pinning. Fracture-induced shortening of the proximal phalanx leads to an extensor lag at the proximal interphalangeal joint. We describe a biomechanical study in cadaver hands to compare the ability of each of three different pin configurations to resist shortening in unstable fractures. METHODS: Seventeen fresh frozen hands were disarticulated at the proximal ends of the metacarpals. The second, third, and fourth proximal phalanges were t

An overview of mallet finger injuries.

Mallet finger describes a fingertip deformity where the distal interphalangeal joint (DIPJ) of the affected digit is held in flexion, unable to extend the distal phalanx actively. The deformity is typically a consequence of traumatic disruption to the terminal extensor tendon at its insertion at the proximal portion of the distal phalanx or slightly proximally at the level of the DIPJ. Patients typically present with a history describing the event of injury with a typical mallet deformity. Common mechanisms include sport activities causing a direct blow to the finger, low energy trauma while p

[Surgical correction of contractural arachnodactyly deformity of a child with Marfan syndrome].

A Marfan syndrome child with contractural arachnodactyly deformity of both hands was admitted to our unit in June 2015. The left and right middle fingers and ring fingers of the child received the continuous " Z" skin flap and " V-Y" plasty to reduce the palmar tension on the skin and release the contracture. Due to the improper therapeutic strategy and the underestimation of effect of underlying lesions of Marfan syndrome on the therapeutic strategy, the right middle finger of child was with ischemic necrosis and amputated, and the finger tip was accompanied with distension and paroxysmal neu

Double Dislocation of Interphalangeal Joints Accompanied with Contralateral Shoulder Dislocation: A Case Report.

Dislocation of any joint is an orthopaedic emergency and needs immediate attention by the attending physician. A delay in reducing a dislocated joint can lead to disastrous complications both immediately as well as in the long run. Although anterior dislocation of a shoulder joint is by far the commonest dislocation encountered by any emergency care physician, other joints may also get dislocated. In certain cases two joints may get dislocated simultaneously. Such dislocation is known as a double dislocation. Double dislocation of the proximal interphalangeal joint and the distal interphalange

[Ultrasound examination in rheumatoid arthritis].

Musculoskeletal ultrasound in the clinical routine praxis increases the hit rate for the diagnosis of inflammatory joint diseases. Power Doppler ultrasound is helpful in the detection of subclinical synovitis in the small finger joints in patients with rheumatoid arthritis; subclinical synovitis can lead to severe joint destruction. Rigorous clinical remission criteria result in lower synovitis detected by power Doppler ultrasound.

Polymyositis associated apatite arthritis.

A patient with polymyositis of 12 years' duration developed inflammatory polyarthritis confined to the hands and wrists, with erosions and periarticular calcifications. Her joint disease exemplified that of a previously described subset of patients with polymyositis associated arthritis. In this case, calcific material was aspirated from the right long finger proximal interphalangeal joint and the mineral was analyzed and identified as apatite.

Illusory deformation of the finger is more extensive in the distal than the lateral direction.

Previous studies have examined the rubber hand illusion with finger lengthening, but there is limited research on finger widening. This suggests a strong cognitive bias toward the illusory expansion of the finger in a distal direction rather than lateral. To test this, we compared the illusory deformability of the finger in the distal and lateral directions through the generation of illusory finger deformation using a double-touch operation, referred to as the numbness illusion. Our results showed that perceived distal distortion was wholly superior to perceived lateral distortion in terms of
