Although abnormalities of vascular anatomy in the forearm are common, variations of the radial and ulnar arteries are rare. Nevertheless, arterial variants in the forearm may present clinically with neurovascular signs or symptoms. Even when anomalous arteries are not apparent, they may complicate surgery of the forearm and hand, as well as reconstructive surgery that involves the harvest of radial or ulnar artery-based forearm free flaps. PURPOSE: Longitudinal radioulnar dissociation occurs when traumatic axial loading through the wrist disrupts the interosseous membrane (IOM) of the forearm and fractures the radial head (Essex-Lopresti injury). Six cases that required soft-tissue replacement in the central midface are presented. The greatest number of flaps were used for large defects in patients with cleft palates who had undergone multiple previous operations. Several were for palatal defects attributable to cocaine abuse, and one was used for lining in a nasal reconstruction. There were no flap losses and, on the basis of these experiences, it is concluded that this is an excellent method for providing soft tissue in these difficult situations.

An amputated fingertip was banked temporarily under the skin of the volar wrist. This prefabricated fingertip was transferred back to the finger 2 months later, together with a free flap from the volar wrist based on the superficial palmar branch of the radial artery. The fingertip pulp showed a little pale pigmentation, but maintained its length well. The dorsal tip of the finger looked normal. This two-stage procedure made it possible to salvage the amputated bone and nail.

Twenty fascial flaps were used in the reconstruction of defects in the distal forearm, wrist and hand in 18 patients over a 2-year period. In 16 patients the fascial flaps were based on a single fascial feeding vessel or 'perforator' arising from the anterior interosseous artery and/or ulnar artery when the radial artery had been used as the donor vessel in free flap reconstruction elsewhere in the body. We carried out a retrospective study of fracture morbidity of radial forearm osteocutaneous donor sites. During a period of three-and-a-half years, 12 patients had had composite flaps taken, and 5 of them had subsequently fractured the radius. We then elected to plate the radius prophylactically and found that over a period of just over two years, none of the 8 patients who had had prophylactic plating at the original harvesting operation had subsequently fractured the radius.

We studied the local changes of blood gas indices of arterialised capillary blood taken simultaneously from the fractured and the uninjured arm of 30 patients with diaphyseal fractures of the forearm. Blood gases were measured with a Stat Profile 5 analyser (Nova Biomedical). The results indicate a slight but statistically significant decrease of the acid-base indices without any changes in the oxygen indices. These slight changes do not affect considerably the diagnosis of the profile.
