Bowel perforation is an unusual complication of ventriculoperitoneal shunting. This article describes a case of bowel perforation associated with a ventriculoperitoneal shunt inserted in an 8-month-old male infant for meningocoele and hydrocephalus. Ten months after insertion of the shunt the infant presented with the shunting tube protruding through the anus. There were no signs of meningitis or peritonitis. Rectal perforation is a serious complication of manipulation of the anus and rectum, with catheters and thermometers. Such procedures should be performed with caution. We report a case of a 19-year-old male with retention of an intrapelvic foreign body, a mercury thermometer, which after perforating the rectum migrated into the pelvis and remained there without any symptoms for the next 7 years. A 42-year-old woman became pregnant for the second and third times following insertion of an IUD in the puerperium of her first pregnancy. The IUD was thought to have been expelled. Nineteen years after insertion, the woman developed abdominal pain and diarrhoea. A sigmoid stricture was diagnosed by endoscopy. Laparotomy showed that the IUD had perforated the uterus and penetrated the sigmoid colon. Resection was performed. This case demonstrates the need to investigate every case of a missing IUD.

Presented here is a rare case in which Kirschner wires migrated from the right sternoclavicular joint to the heart. A 29-year-old man suffering from sternoclavicular instability due to a motorcycle accident received surgical fixation with Kirschner wires. Six months after the surgery, the chest x-ray showed migration of the 3 broken wires to the anterior mediastinum and to the right hemithorax. The authors report a case of intra-abdominal migration of a Kirschner wire from the left hip to the right lobe of the liver in a 5-year-old child. The wire was used for stabilization of the left hip after open reduction for neglected unreduced congenital dislocation of the left hip. The migrated wire was removed by laparotomy. Surprisingly, no injury was noted to any intervening abdominal structure intra-operatively. This unusual migration of a Kirschner wire into a child's liver has not been reported previously.

A case of a lost GyneFix intrauterine contraceptive device (IUD) is described, in which laparoscopy failed to identify the device and laparotomy had to be carried out to remove the IUD, which was embedded in the small bowel necessitating bowel resection. Awareness of this complication is necessary, and advanced training is required in order to minimize risks. A description of the GyneFix device, the possible adverse effects and incidence of complications, the importance of post-insertion follow-up, and the need for awareness of the possibility of migration through the bowel are discussed.

Migration of left in foreign bodies like gauze sponge into intestine, stomach, urinary bladder and their extrusion through umbilicus or rectum is well known. We report a unique case wherein a gauze sponge was left in abdominal cavity 25 years back at the time of initial surgery. This patient who had not undergone any thoracic surgery in the past was recently subjected to posterolateral thoracotomy for a right sided lung abscess and a large sponge gauze was unbelievably recovered from the area of lung abscess.

Fish-bone foreign bodies represent a common condition readily identified in the pharynx. We encountered a case of fish bone in the thyroid gland. Only three other such cases have been reported in the English-language literature. This foreign body was identified by computed tomography and ultrasonography 16 days after onset, and removed through a cervical skin incision. The diagnosis and treatment of extrapharyngeal foreign bodies are discussed.

BACKGROUND: Intrauterine device (IUD) perforation of the bowel is uncommon. Although IUD perforation may be asymptomatic, the most common complaint is unexplained abdominal pain.
CASE: A case of IUD perforation of the large bowel was diagnosed 7 years after insertion. The patient presented with unexplained lower abdominal pain diagnosed initially as pelvic inflammatory disease. Laparoscopy revealed that the IUD was embedded deeply in the rectum. Bowel preparation and intravenous antibiotics followed by colonoscopy using a grasping snare resulted in successful IUD removal.
CONCLUSION: Patient

Vaginal pessaries still have a role in the management of uterine prolapse, particularly in elderly patients. However, they are known to cause serious complications if proper care is not taken. We present a case of a rectovaginal fistula, developing secondary to a forgotten vaginal pessary. The shelf pessary was found to have eroded through into the rectum. A review of the relevant literature was undertaken and complications associated with vaginal pessaries are discussed.

PURPOSE: To describe the unusual complication of migration and apparent disappearance of silicone tubes following intubation of the lacrimal drainage system.
DESIGN: Interventional case reports.
METHOD: Report of two patients with an unusual tube migration process.
RESULTS: Silicone stents migrated nasally, with healing of the erosion tract in the wake of the migrating tube. One tube became completely internalized.
CONCLUSIONS: Meticulous inspection of the medial canthal area in patients with suspected spontaneous extrusion of silicone stents from the lacrimal drainage system is warranted.

We report a case of tracheal stenosis in which a migrated Polyflex (Rusch AG, Germany) stent entered the right main bronchus but could not be identified on the chest radiograph. The stent was identified at bronchoscopy and removed under general anaesthesia. Polyflex stents are radiopaque but may not always show up on radiograph. We recommend reliance on clinical symptoms rather than imaging to diagnose migration. If stent migration is suspected then imaging should include thoracic inlet films that incorporate a lateral view.

The authors report a rare consequence of an anteriorly migrated encircling silicone band. The extruded encircling band, which "cheesewired" forward through the superior and medical rectus insertions straddled the cornea firmly and formed a corneal groove. Corneal groove is an extremely rare complication of scleral buckling surgery.
