The segmental tibial fracture.

A series of 31 segmental tibial fractures is reported. Eighty-four percent of the patients sustained multiple trauma and 80% of these were open fractures. The series includes treatment with 20 external fixators, seven unreamed intramedullary nails, two casts, and two amputations. Complications included a 48% incidence of elevated compartment pressures necessitating fasciotomy. Despite rapid early decompression, there was a 19% incidence of residual motor and sensory deficit. Other complications observed were a 35% incidence of wound infection, nonunion, and malunion. Eighty-one percent of dela

Elastic intramedullary nail treatment of adolescent perihardware radius and ulna refracture.

A 13-year-old girl presented to the clinic with a midshaft refracture of both forearm bones adjacent to the site of a prior forearm fracture that had been treated with plating. She was treated with hardware removal and placement of elastic intramedullary nails. Flexible intramedullary nailing can be successful for a skeletally mature adolescent in treatment of refracture surrounding plate fixation of a midshaft forearm fracture. This technique allows for additional protection of the entire length of the affected bones, while avoiding the extensive dissection needed for extended plating.

Experimental and numerical analysis of the influence of intramedullary nail position on the cut-out phenomenon.

BACKGROUND AND OBJECTIVE: Proximal femur fractures, colloquially known as hip fractures, are a common pathology with increasing incidence in the last years due to the enhanced ageing population. Regarding the extracapsular fracture, the treatment for this pathology consists of a fixation of the fragments using an osteosynthesis device, mainly the intramedullary nail. This repairing method implies several complications, which may include the failure of the fixation device, frequently occurring due to the "cut-out" mechanism. The present work focuses on the study of how the position of the cepha

Bullet Fragment Retrieval During Intramedullary Nailing: A Report of 2 Cases.

CASE: Two young adult patients, 22 and 27 years, who sustained ballistic long bone fractures, presented for incarcerated, retained metallic fragments in the medullary canal, which blocked fixation. Owing to the presence and location of the fragments, each case required a bullectomy (removal of the metallic fragment) before intramedullary nail fixation because the projectile impeded the appropriate insertion of the intramedullary rod and, in one case, induced fracture malalignment. Attempts at removal using conventional tools did not provide adequate length or grasp of the retained projectile,

Outcome of Titanium Elastic Intramedullary Nail in the Treatment of Shaft of Femur Fracture in Children.

Background Femoral-shaft fractures are among the most common fractures of the lower extremity in children. There are several different options for treating femoral-shaft fractures in children. Elastic stable intramedullary nailing (ESIN) has become the standard treatment for fractures of shaft of femur in children for reasons including mini-invasive surgery, no need for casting, early mobilization and discharge as well as growing concerns toward cost-effectiveness. Objective To demonstrate the effectiveness of intramedullary fixation of fracture shaft of femur in skeletally immature children u

Humeral shaft fractures.

Fractures of the humeral shaft are common injuries with multiple management strategies. Many still regard nonoperative management as the standard of care; however, as the understanding of these injuries increases, treatment recommendations are also evolving. Fracture pattern, fracture location, and identifiable patient risk factors may predict poor outcome with nonoperative management, and earlier operative intervention may be recommended. Operative management includes open reduction and internal fixation through a variety of exposures, intramedullary nail fixation, and external fixation. With

Intramedullary nailing of pediatric femoral shaft fracture.

Intramedullary nail fixation of pediatric long bone fracture, particularly femoral shaft fracture, has revolutionized the care and outcome of these complex injuries. Nailing is associated with a high rate of union and a low rate of complications. Improved understanding of proximal femoral vascularity has led to changes in nail insertion methodology. Multiple fixation devices are available; selection is based on fracture type, patient age, skeletal maturity, and body mass index. A thorough knowledge of anatomy and biomechanics is required to achieve optimal results without negatively affecting

Screw elastic intramedullary nail for the management of adult forearm fractures.

BACKGROUND: The failure of the conventional nailing of both forearm bones or isolated fractures of radius and ulna pose a potential problem of nail migration and rotational instability, despite the best reduction. The purpose of this paper is to evaluate the results of screw elastic intramedullary nail for the treatment of adult diaphyseal fractures of both forearm bones, which effectively addresses the problems associated with the conventional nailing systems for the forearm fractures. MATERIALS AND METHODS: Seventy-six adults with forearm fractures (radius and ulna or isolated fracture of t

Evolution of minimally invasive plate osteosynthesis (MIPO) in the femur.

Problems with conventional open reduction and internal plate fixation of distal femoral fractures are well established. These problems have been associated with extensile exposures of the fracture site. "Biological plating", like intramedullary nail fixation, of distal femoral fractures preserves the soft tissues about the fracture, and is associated with early fracture consolidation and low rates of infection. Anatomical restoration of the articular surface continues to be the main goal in the treatment of these fractures regardless of the stabilization technique. Submuscular plating techniqu

Radioulnar Synostosis after Endomedullary Fixation of the Radius and Ulna in a Patient with Floating Elbow: A Case Report.

Floating elbow is a complex and rare entity caused by high-energy trauma. In this paper, we present the case of a patient who suffered a traffic accident with severe head trauma, floating elbow (humeral diaphyseal fracture, radial proximal diaphyseal fracture, and ulnar segmental fracture) and radial nerve injury. Fixations were made with a humeral plate and intramedullary rods in the forearm. Although the outcome was satisfactory, radioulnar synostosis was identified in postoperative controls. Due to the neurological compromise, type of fractures, and stabilization selected, we believe that t

Cannulated screw fixation of fracture neck of femur in children with osteogenesis imperfecta.

Osteogenesis imperfecta is a disorder of abnormality in collagen metabolism due to genetic defects, which causes fragility fracture in children. Fragility fracture of the neck of femur can be difficult to treat in adults. The difficulties increase exponentially in children. The challenge becomes more severe when there is an intramedullary rod in situ in the femoral shaft. It is a technically demanding work to fix fracture caused by osteogenesis imperfecta. There is hardly any published study on the difficult fixation of fracture neck of femur in osteogensis imperfecta. Therefore, we present tw

Proximal Fifth Metatarsal Fractures in Athletes: Management of Acute and Chronic Conditions.

Proximal fifth metatarsal fractures are common in the athlete and can be a source of significant, temporary disability and missed playing time. The pattern of fracture can vary, and the type of fracture leads to a significantly different prognosis and treatment. Jones fractures of the fifth metatarsal are particularly common and difficult to treat in the athlete, can have recurrence and refracture, and require expertise to heal. Intramedullary screw fixation is currently the preferred method of fixation. Most other (non-Jones fractures and os vesalianum) proximal fifth metatarsal fractures can

Subtrochanteric fractures of the femur.

A consecutive series of 103 patients with a subtrochanteric fracture were prospectively studied. Ten patients were treated non-operatively, whilst the other 93 had operative treatment. The overall fixation failure rate was 12 per cent with a re-operation rate of 6 per cent by 1 year. There were six (8 per cent) failures of fixation for the 74 fractures treated with the sliding hip screw. No method of fracture classification was demonstrated to be of value in predicting either the choice of treatment or the risk of fracture healing complications. Either intramedullary nailing or extramedullary

Cyclic loading of olecranon fracture fixation constructs.

BACKGROUND: Despite the good results that are usually reported after fixation at the sites of olecranon fractures and osteotomies, problems such as loss of fixation, nonunion, and the need for revision surgery are still encountered. Various types of fixation have been recommended, but few have been evaluated with use of clinically relevant cyclic load testing at appropriate levels of stress. The purpose of the present study was to test multiple olecranon fixation techniques under physiologic cyclic loads. METHODS: We studied ten cadaveric elbows with use of cyclic loading that simulated (1) a

A case report: Distal humeral diaphyseal fracture in a child.

RATIONALE: Distal humeral diaphyseal fractures are much less common than supracondylar humeral fractures. The triangular shape and thinner periosteum in the diaphyseal region than in the supracondylar region of the distal diaphysis makes the treatment of diaphyseal fractures difficult. Nonoperative treatment for this fracture is challenging and troublesome; thus, open reduction and internal fixation (OR/IF) is generally recommended. PATIENT CONCERNS: We herein report a distal humeral diaphyseal fracture in a child. A 6-year-old boy fell from a chair, injuring his left elbow. Radiographs were

Short segment coronal plane stability screws in retrograde nailing of proximal femoral shaft fractures: a technical trick and case series.

Retrograde intramedullary nailing is commonly used for fixation of femoral shaft fractures due to high healing rates and ease of placement. However, for proximal femur fractures, there is concern that the limited distance between the fracture and proximal interlock fixation may predispose these fractures to varus deformity and malunion. Coronal plane stability screws placed just adjacent to the outer diameter of the intramedullary nail can be used to counteract this coronal plane deformity but can be difficult to place proximally due to limitations from body habitus and difficulty with fluoros

Flexible intramedullary nailing in the treatment of forearm fractures in children and adolescents, a systematic review.

BACKGROUND: Forearm fractures are common injuries among children and adolescents. Traditionally, they were managed conservatively with closed reduction and cast immobilization. The last decade there is an increasing trend towards operative treatment. The treatment modalities available include plate and screws, flexible intramedullary nailing and external fixation devices - rarely used nowadays. The aim of this systematic review is to investigate the indications of flexible intramedullary nailing in the childhood population, to compare its results with plating and to provide detailed informatio
