Pathological gambling in Parkinson's disease--a review of the literature.

The prevalence of pathological gambling is 3.4% to 6% in treated Parkinson's disease, which is higher than the background population rate. In this review we discuss current evidence to indicate that dopamine agonists are much more likely to trigger this behavior than either L-dopa or selective monoamine oxidase B inhibitor monotherapy. New insights from recent behavioral and functional imaging studies and possible treatment approaches are also covered. A PubMed literature search using the terms "gambling" and "Parkinson's disease," "impulse control disorder," "impulsive compulsive behaviour," …

Methods of assessing craving to gamble: a narrative review.

Although the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV) does not explicitly list craving as a diagnostic criterion for pathological gambling, theories of disordered gambling and research on relapse precipitants suggest that cravings--among other factors--provoke and maintain episodes of gambling. Assessment of craving to gamble is complicated by questions regarding (a) the emotional, cognitive, behavioral, and physiological components of such craving; (b) the degree to which craving is viewed as an acute and fluctuating experience or as a relatively stable preoccup…

Pathological gambling and the structure of common mental disorders.

This study investigated a proposed three-factor model of common mental disorders in the National Epidemiologic Survey of Alcohol and Related Conditions (NESARC; N = 43,093). It also evaluated the inclusion of pathological gambling (PG) in the three-factor model to determine whether PG loaded more strongly on a higher-order externalizing factor comprised of alcohol and drug dependence and antisocial personality disorder. The results indicated the three-factor model showed good fit to the NESARC data. PG loaded onto the externalizing factor for men and women, but in women, PG showed the best fit…

Adolescent gambling: a review of an emerging field of research.

PURPOSE: The purpose of this review was to summarize the research on adolescent gambling with implications for research and prevention or intervention. METHOD: The methodology involved a comprehensive and systematic search of "adolescent or youth gambling" in three diverse electronic databases (MedlineAdvanced, PsycINFO, and Sociological Abstracts) and three peer-reviewed journals (International Journal of Gambling Studies, International Journal of Mental Health and Addiction, and Journal of Gambling Issues). RESULTS: The search resulted in 137 articles (1985-2010) focusing on gambling among…

Outcomes of patients with Parkinson disease and pathological gambling.

OBJECTIVE: To determine the outcomes of patients with Parkinson disease (PD) with pathological gambling (PG) from one Canadian Movement Disorders Clinic. METHODS: Assessments were performed in-person during routine clinic visits of all patients currently followed by one neurologist (OS). Pathological gambling was defined according to DSM-IV-TR criteria. Chart review was performed to obtain details on medication use, dosages, and patient demographics. Follow-up of patients with PG collected information on gambling behavior, PG management interventions, medications, treatment, and psychosocial …

The development of a multi-dimensional gambling accessibility scale.

The aim of the current study was to develop a scale of gambling accessibility that would have theoretical significance to exposure theory and also serve to highlight the accessibility risk factors for problem gambling. Scale items were generated from the Productivity Commission's (Australia's Gambling Industries: Report No. 10. AusInfo, Canberra, 1999) recommendations and tested on a group with high exposure to the gambling environment. In total, 533 gaming venue employees (aged 18-70 years; 67% women) completed a questionnaire that included six 13-item scales measuring accessibility across a …

The comorbidity of tobacco smoking and gambling: a review of the literature.

ISSUES: Evidence suggests that tobacco smoking and gambling frequently co-occur. Although high rates of comorbid smoking and gambling have been documented in studies with clinical populations of pathological gamblers in treatment, in studies using samples drawn from the community, and in large-epidemiological surveys, little empirical attention has been directed towards investigating the exact nature of this relationship. APPROACH: In this review, we stress the literature that has examined the epidemiology, aetiology and environmental factors implicated in comorbid smoking and gambling. Publi…

A pilot study of impulsivity and compulsivity in pathological gambling.

We examined the relationship between gambling severity, impulsivity and obsessionality/compulsivity in 38 pathological gamblers, representing the complete Minnesota sample of a randomized, placebo-controlled clinical trial of paroxetine for the treatment of pathological gambling (PG), using Pearson correlations and linear regression models at baseline and treatment endpoint. At baseline, Pathological Gambling Modification of the Yale-Brown Obsessive-Compulsive Scale (PG-YBOCS) scores correlated significantly with those of the Eysenck Impulsiveness Questionnaire (EIQ) Impulsiveness subscale and…

Prevalence of problem and pathological gambling in Parkinson's disease.

Pathological gambling (PG) has been identified in patients with Parkinson's disease (PD) treated with dopamine agonists suggesting that dysregulation of brain dopaminergic activity may contribute to the development of gambling problems. The current study was undertaken to further establish the prevalence of problem and PG in patients with PD, identify any clinical correlates, and determine if psychiatric or substance use co-morbidity contributes to the increased prevalence of problem and PG. A cross-sectional survey of 140 serially recruited moderate to severe PD patients was undertaken utiliz…

Is control a viable goal in the treatment of pathological gambling?

According to a report of National Gambling Impact Study Commission (National Gambling Impact Study Commission (1999). Final report. Washington, DC: Government Printing Office.), 97% of problem gamblers in the United States do not seek treatment. Within the small proportion of problem gamblers who enter into treatment, a high percentage drops out. Despite the fact that some researchers argue against abstinence as the only acceptable treatment goal and that regaining control over gambling behaviour appears to be possible for some pathological gamblers (PG), abstinence has been the only gambling …

Dispositional Mindfulness as a Predictor of the Severity of Gambling Outcomes.

Two studies were conducted to test and explain the relation of mindfulness to the severity of gambling outcomes among frequent gamblers. In both studies, dispositional mindfulness related to less severe gambling outcomes as measured by a DSM-IV-based screen for pathological gambling, even after controlling for gambling frequency and dispositional self-control. Study 2 extended this finding in showing that the association between mindfulness and lower pathological gambling was partially mediated by better performance on two risk-taking tasks that capture overconfidence, risky bet acceptance, an…

The effect of pathological gambling on families, marriages, and children.

Pathological gambling (PG) is widely reported to have negative consequences on marriages, families, and children. Empirical evidence is only now accumulating but when put together with anecdotal information, the extent of these problems is clear. PG contributes to chaos and dysfunction within the family unit, disrupts marriages, leading to high rates of separation and divorce, and is associated with child abuse and neglect. Divorce rates are high, not surprising in light of reports that these marriages are often abusive. Research shows that the families of pathological gamblers are filled with…

A comment on the utility of prevalence estimates of pathological gambling.

The case is presented that researchers interested in policy aimed at treating the pathological gambler need to shift focus to improving the … these dimensions to ascertain the proportion of gamblers most likely to need or seek treatment for gambling-related disorders. The recommendations provided are subject to a number of possible objections and are presented in the interest of stimulating further discussion such as the distinction between symptom assessment and the measurement of severity.

Psychological experience of gambling and subtypes of pathological gamblers.

Clinical experience and some empirical data suggest that different subtypes of gamblers may exist. This study examined psychometric properties of the Gambling Experience Measure (GEM) to establish support for differing subtypes of gamblers. In total, 149 treatment-seeking pathological gamblers completed the GEM and other instruments. A principal component analysis produced a three-factor solution that included gambling as a way to escape negative emotions (Escape), dissociating (Dissociation), and narcissism/seeking attention (Egotism). Multiple regression analyses revealed that being female a…

Electronic gaming machines: are they the 'crack-cocaine' of gambling?

BACKGROUND: There is a general view that electronic gaming is the most 'addictive' form of gambling, in that it contributes more to causing problem gambling than any other gambling activity. As such, electronic gaming machines have been referred to as the 'crack-cocaine' of gambling. While this analogy has popular appeal, it is only recently that the scientific community has begun to investigate its validity. In line with the belief that electronic gambling has a higher 'addictive' potential than other forms of gambling, research has also begun to focus on identifying the characteristics of ga…

Methodological problems in research on treatments for pathological gambling.

These common methodological problems affect the usefulness of pathological gambling (PG) treatment research: (1) Most PG etiological models derive from substance abuse; (2) Most PGs recover on their own and few seek treatment, so PGs in treatment studies may not be representative; (3) Data on treatment-seeking PG women, older adults, and minority group members, or on group, marital, and psychodynamic approaches to PG, are sparse; (4) Little research has examined the impact of comorbidity on PG treatment response; (5) Design deficiencies burden many PG treatment studies.

The "big win" and resistance to extinction when gambling.

One hypothesis for the reason a person might become a pathological gambler is that the individual initially experiences a big win, which creates a fallacious expectation of winning, which may then lead to persistent gambling despite suffering large losses. Although this hypothesis has been around for several decades, only one controlled empirical study has addressed it, and that study reported null results. In the present experiment, the authors tested the "big win" hypothesis by having 4 groups of participants with little to no experience gambling play a computer-simulated slot machine for cr…
