A pilot study of standardized treatment in geriatric bipolar disorder.

OBJECTIVE: The authors sought to determine the feasibility of treating elderly adults with bipolar disorder under standardized-treatment conditions. METHODS: Thirty-one patients age 60 and older with bipolar disorder were treated in standardized pathways. Mood state was checked at each study visit with the Hamilton Rating Scale for Depression-17 item (Ham-D-17) and the Young Mania Rating Scale (YMRS). RESULTS: Defining "well days" as both Ham-D and YMRS scores of <or=10, the mean percentage of well days was 72.5 (range: 0%-100%) over study participation. CONCLUSIONS: Treating older adults w

[Validation of geriatric depression scale in a general outpatient clinic].

OBJECTIVE: The Geriatric Depression Scale for screening depressive symptoms in the elderly has not been assessed in elderly outpatients who seek primary health care in Brazil. The objective was to determine the validity of the Short Scale for Major Depressive Episode or Dysthymia (GDS-15) in elderly outpatients. METHODS: The scale was applied in 302 subjects with 65 years and older and then examined by an independent geriatrician, blinded to the results. Major depression and dysthymia were diagnosed using the Diagnostic and Statistical Manual of Mental Disorders-IV criteria. Sensitivity and s

End-of-life care: issues relevant to the geriatric psychiatrist.

Most deaths in the United States occur in the context of chronic diseases in later life and are too often accompanied by potentially remediable emotional or physical suffering. Geriatric psychiatrists and other mental health professionals can contribute meaningfully to the provision of optimal care during the final phases of life. This review provides an overview of end-of-life care, focusing on issues most relevant to the geriatric psychiatrist. The author examined palliative care textbooks and review papers to determine the topics to be included in this article, and searched computerized lit

Drugs and falls in older people in geriatric care settings.

BACKGROUND AND AIMS: Falls and their consequences constitute serious health problems in the older population. The aim was to study predisposing factors for falls among older people in geriatric care settings, focusing on drugs. METHODS: This population-based study, with a cross-sectional design, analysed all geriatric care settings, comprising 68 residential care facilities, 31 nursing homes, 66 group dwellings for people with dementia, seven rehabilitation/short-stay units, two somatic geriatric and two psychogeriatric clinics, in the county of Västerbotten; 3604 residents with a mean age of

[Unit dose systems prepared for geriatric patients. I].

The project was carried out to facilitate and improve the quality of the use of drugs by geriatric patients aiming at the observation of the therapeutic regimen and the improvement of their compliance. The group under study comprised 15 clients living in a selected old people's home in the city of Brno, for whom the pharmacist prepared drugs into one-week blister dispensers on the basis of their health dossiers for a period of 6 months (February - July 2002). Polypharmacy occured in all patients included into a unit-dose preparation for drugs; they took on average 16 drugs in a week. The most

Potential applications of matrix metalloproteinase inhibitors in geriatric practice.

Matrix metalloproteinases are a family of enzymes that degrade different components of extracellular matrix. They play an important role in normal physiologic processes of maintaining tissue integrity and remodeling, as in wound healing, processes of development, and regeneration. However, excessive expression of MMP has been observed in many disease states, including rheumatoid arthritis and osteoarthritis, vascular remodeling in atherosclerosis and aortic aneurysm formation, neoplastic processes, macular degeneration and many others.

Pica in the geriatric mentally ill: unrelenting and potentially fatal.

Pica has rarely been reported in patients with geriatric mental illness. The authors describe 3 male patients with pica in the geriatric unit of a state mental hospital. Two of these patients had a diagnosis of developmental delay with concomitant diagnoses of schizophrenia and schizoaffective disorder, respectively. The third patient was diagnosed with paranoid schizophrenia. In all 3 cases, pica started late in life and was unrelenting and unresponsive to psychotropic medications, with limited to no response to behavioral interventions. In the 2 patients who died, pica was implicated as the

Orthostatic hypotension in acute geriatric ward: is it a consistent finding?

BACKGROUND: Orthostatic hypotension (OH) is a common finding among older patients. We designed a study to examine the prevalence and consistency of OH during the day. METHODS: A total of 502 inpatients (241 men and 261 women) with a mean age of 81.6 years were included in the study. Orthostatic tests were performed 3 times during the day, 30 minutes after meals. In 13 patients only 2 sets of measurements were obtained, and they were omitted from some of the calculations. Orthostatic hypotension was defined as a fall of at least 20 mm Hg in systolic blood pressure and/or 10 mm Hg in diastolic

New glaucoma medications in the geriatric population: efficacy and safety.

Glaucoma can be considered a disease of the aging eye. Most medications used to treat glaucoma are in topical eyedrop form and may cause numerous untoward systemic effects in older persons. In recent years, several new ocular hypotensive medications have become available. These medications are being used more commonly because there is a growing trend by ophthalmologists to aggressively lower intraocular pressure. Therefore, geriatricians require a comprehensive knowledge of medications used to treat glaucoma, in addition to an understanding of their mechanism of action profiles of untoward eff

Dizziness among older adults: a possible geriatric syndrome.

BACKGROUND: In previous studies of dizziness, the prevalence of specific causes has varied widely and either no or multiple causes have been identified. Dizziness might be better considered a geriatric syndrome that results from impairment or disease in multiple systems. OBJECTIVE: To determine the predisposing characteristics and situational factors associated with dizziness. DESIGN: Population-based, cross-sectional study. SETTING: Community. PARTICIPANTS: Probability sample of 1087 community-living persons in New Haven, Connecticut, who were at least 72 years of age. MEASUREMENTS: Epis

[Dehydration in geriatric patients. Fluid substitution--also subcutaneous!].

Signs of exsiccation are to be found in almost every fourth acutely ill patient admitted to the geriatric department. The major clinical signs are those associated with reduced general condition, together with somnolence, possibly agitation, oliguria, dry skin, orthostatic hypotension, and a tendency to be bedridden. In such cases, subcutaneous infusion is a simple-to-apply and for the patient stress-free, largely pain-free possibility for fluid replacement. Properly applied and with account being taken of the contraindications--in particular emergency situations, decompensated cardiac insuffi

[The treatment of epilepsy in medical conditions, geriatric and pregnant patients].

INTRODUCTION: Treatment with antiepileptic drugs in patients with medical conditions, old age or pregnancy is associated with concomitant pathology and the specific physiopathological changes of age or pregnancy. DEVELOPMENT: The difficulties of treatment in special conditions are related to changes in the pharmacokinetics of antiepileptic drugs, interaction with other medication and with concomitant treatment with drugs which may potentially cause convulsions. Also, the occurrence of a particular disorder or of pregnancy may lead to changes in the frequency of crises. Besides this, the side-

The efficacy and safety of clozapine versus chlorpromazine in geriatric schizophrenia.

BACKGROUND: There has been an absence of controlled studies focusing specifically on neuroleptic treatment in the elderly schizophrenic population. Therefore, we conducted a 12-week double-blind comparison study to assess the efficacy and tolerability of clozapine and chlorpromazine in a group of elderly inpatients with chronic schizophrenia. METHOD: Forty-two elderly DSM-IV schizophrenic veterans were randomly assigned to clozapine or chlorpromazine and assessed for efficacy at baseline and at termination with the Positive and Negative Syndrome Scale (PANSS) and the Clinical Global Impressio

[Aggressive behavior of psychiatric patients among geriatric population].

Medical documentation of 674 elderly patients of gerontopsychiatric department was studied in order to estimate the frequency of physical aggression in relation to sex, diagnosis of dementia and other psychiatric disorders. This form of aggression was found significantly more often in men (52%) than in women (32%). The prevalence of aggression in men was even more evident in the dementia group (67% men and 39% women). Aggressive acts, with the use of objects as weapons, were aimed mainly against other patients when carried out by women, while male patients more often used the objects to attack
