Optimizing outcomes in pediatric microvascular mandibular reconstruction: A 23-year institutional experience.

BACKGROUND: The vascularized free fibula flap (VFFF) is a well-established technique for addressing critical-sized mandibular defects, inclu … h; two flap failures occurred with single venous anastomosis, and none with two venous anastomoses. Hardware removal was planned on an average 15.7 months after nine operations (29%) to maintain growth potential; subsequent corrective orthognathic surgery was performed following four flaps (13%). CONCLUSION: Pediatric mandibular reconstruction using VFFF is safe …

A systematic review and meta-analysis of tension band plating vs. intramedullary nailing for anterior tibial diaphyseal stress fractures in athletes.

BACKGROUND: Return to sport (RTS) and optimal operative management for anterior tibial diaphyseal stress fractures (aTDSF) in athletes remai … ronic anterior knee pain (2/14, 14%), while tension band plating complications included symptomatic hardware removal (6/20, 30%) and one athlete unable to compete (1/20, 5%). The included studies were small case series or case reports and thus prone to bias. CONCLUSION: Both IM nailing and tension band plating yield high RTS rates, but tension band plating may of …

Percutaneous Transphyseal Screw Hemiepiphysiodesis (PETS) for Tibia Vara in Blount's Disease.

BACKGROUND: Blount's disease is a progressive developmental disorder causing pathologic genu varum due to growth disturbances in the medial proximal tibial physis. Guided growth is a primary treatment, but established methods like staples and 8-plates have limitations, including hardware failure and unpredictable correction. This study evaluates an alternative technique of hemiepiphysiodesis using percutaneous transphyseal screws (PETS) for treating juvenile and adolescent Blount's disease. METHODS: This prospective case series included 61 patients (80 knees) aged 4 to 12 years treated for Bl

Fix it or risk it? Revisiting contralateral hip prophylactic fixation in unilateral slipped capital femoral epiphysis: an updated review of the past decade's literature.

Slipped capital femoral epiphysis (SCFE) is a common hip pathology occurring in pediatrics and adolescents, and its management is well docum … sk of altering proximal femoral growth, peri-implant fractures, and requiring secondary surgery for hardware removal. The current review was formulated based on the literature published in the last decade and aimed to evaluate the incidence of SCS and indications for CHPF. Apart from institutions' protocol or individual surgeons' preferences, various variables wer …

Conversion of First Metatarsal-Phalangeal Joint Arthrodesis to Arthroplasty: A Case Report.

INTRODUCTION: Arthrodesis remains the gold standard for most first metatarsal-phalangeal joint (1MTPJ) pathologic conditions due to its high patient satisfaction, low complication rates, and consistent data. 1MTPJ arthroplasty remains a pursued procedure given the advantages described above, but the literature remains complicated and controversial as a primary surgical treatment. To the authors' knowledge, there is no prior report describing utilization of arthroplasty as an approach to managing clinical failure of a successful fusion in the setting of a technically successful procedure withou

Tibial derotational osteotomies in two neuromuscular populations: comparing cerebral palsy with myelomeningocele.

PURPOSE: To review the outcomes of tibial derotational osteotomies (TDOs) as a function of complication and revision surgery rates comparing a cohort of children with myelodysplasia to a cohort with cerebral palsy (CP). METHODS: A chart review was completed on TDOs performed in a tertiary referral centre on patients with myelodysplasia or CP between 1985 and 2013 in patients aged > 5 years with > 2 years follow-up. Charts were reviewed for demographics, direction/degree of derotation, complications and need for re-derotation. Two-sample T-tests were used to compare the characteristics of the

Complications of the surgical treatment of fractures of the tibial plateau: prevalence, causes, and management.

Open reduction and internal fixation is the gold standard treatment for tibial plateau fractures. However, the procedure is not free of complications such as knee stiffness, acute infection, chronic infection (osteomyelitis), malunion, non-union, and post-traumatic osteoarthritis. The treatment options for knee stiffness are mobilisation under anaesthesia (MUA) when the duration is less than 3 months, arthroscopic release when the duration is between 3 and 6 months, and open release for refractory cases or cases lasting more than 6 months. Early arthroscopic release can be associated with MUA.

A theoretical exploration of protocols for treating prosthetic joint infections with combinations of antibiotics and bacteriophage.

With the increase in the placement of prosthetic joints and other hardware in the body, associated infections have risen. These infections are complicated to treat due to the underlying bacteria generating matrices that resist clearance by immune system effectors or antibiotics. These matrices, biofilms, have two primary ways of being eradicated: either by physical removal during surgery or by killing the underlying bacteria, usually with antibiotics. The viruses that kill bacteria, bacteriophages, are readily capable of entering biofilms and eradicating the bacteria therein. Therefore, bacter

Retained Permanent Epicardial Pacemaker Leads and Lung Abscess: A Diagnostic and Surgical Challenge.

BACKGROUND: Epicardial pacemaker leads are used when transvenous access is contraindicated, but retained leads can rarely result in serious long-term complications. CASE SUMMARY: A 40-year-old male with intravenous drug use, tricuspid valve replacement, and prior epicardial pacemaker implantation presented with hemoptysis. Imaging revealed a left lower lobe lung abscess containing retained epicardial leads, left in situ after incomplete extraction years earlier. He underwent lobectomy and resection of infected leads. Postoperatively, he developed respiratory failure and arrhythmias requiring

A "Rash" Decision in Anesthetic Management: Benzyl Alcohol Allergy in the Perioperative Period.

Here, we present the case of a 54-year-old female presenting for outpatient ankle hardware removal who experienced severe total body pruritus along with a maculopapular rash persisting four days after the procedure. Patch testing demonstrated a sensitivity to benzyl alcohol, a preservative in propofol and several other anesthetics. The patient returned for left ankle arthroscopy a year later, and during that procedure, the anesthetic team avoided medications containing benzyl alcohol. This resulted in no pruritus or rash. Hypersensitivity reactions, ranging from contact dermatitis to anaphylax…

Nontuberculous Mycobacterial Musculoskeletal Infection Cases from a Tertiary Referral Center, Colorado, USA.

Nontuberculous mycobacteria represent an uncommon but important cause of infection of the musculoskeletal system. Such infections require ag … ey sought care at the referral center. Six cases involved joint prostheses; all ultimately required hardware removal and placement of an antimicrobial spacer for eradication of infection. Our findings highlight the importance of vigilance for nontuberculous mycobacterial musculoskeletal infection, particularly in patients who are immunosuppressed or have a history …

Pharyngoesophageal perforation 3 years after anterior cervical spine surgery: a rare case report and literature review.

Pharyngoesophageal perforation after anterior cervical spine surgery is rare and the delayed cases were more rarely reported but potentially life-threatening. We report a case of pharyngoesophageal perforation 3 years after anterior cervical spine surgery. The patient presented with dysphagia, fever, left cervical mass and developing dyspnea 3 years after cervical spine surgery for trauma. After careful examinations, he underwent an emergency tracheostomy, neck exploration, hardware removal, abscess drainage and infected tissue debridement. 14 days after surgery, CT of the neck with oral contr…

Distal radius fracture fixation with an intramedullary nail.

The purpose of this article is to describe a novel technique and implant system for fixation of unstable (Arbeitsgemeinschat fur Osteosynthesesfragen Types A and B) distal radius fractures. Currently the most common complications directly related to distal radius plate fixation includes tenosynovitis, tendon attrition, and rupture, often necessitating hardware removal. With the advent of this new device, a decrease in soft tissue complication is expected. The implant utilizes the principles of load sharing, subchondral screw divergence, and locked fixed-angle fixation. It is inserted through a…

Shortening of a unit rod protruding into the hip joint: case report and description of a surgical technique.

A unit rod is a well-accepted method of posterior spinal instrumentation in patients with cerebral palsy (CP). Several types of complication … h a lower blood loss than was previously reported for revisions of unit rods. There was no need for hardware removal and therefore no loss of correction. The unit rod may be shortened before surgery to prevent this problem. The described procedure is a simple way of correcting penetration of a unit-rod limb into the hip joint.

Irritation from metalwork after ankle arthrodesis fixed using screws: a proportional meta-analysis and systematic review.

OBJECTIVE: Ankle arthrodesis (AA) is often fixed using cannulated screws. The irritation from metalwork is a relatively common complication, but there is no consensus regarding the need to remove the screws on a systematic basis. The aim of this study was to determine (1) the proportion of screws removed after AA and (2) whether predictors of screw removal could be identified. METHODS: This PRISMA-compliant systematic review was part of a larger previous protocol registered on the PROSPERO platform. Multiple databases were searched including studies in which patients undergone AA using screws

Proximal Femoral Bone Cyst After Single Screw Fixation for Slipped Capital Femoral Epiphysis: A Case Report.

CASE: An 11-year-old female patient underwent bilateral in situ fixation for slipped capital femoral epiphyses using single, cannulated, stainless steel screws. She presented 12 years later with a large osteolytic lesion of the proximal femur, which only involved 1 side. Histological evaluation showed a foreign-body reaction and synovial lining. Infection was ruled out and dynamic hip screw stabilization, and bone grafting were performed. The bone graft healed, and the hardware was removed to prevent a similar reaction. In retrospect, postoperative radiographs at 7 weeks showed subtle osteolys

Kyphectomy in children with myelomeningocele: a long-term outcome study.

Twenty-four children with myelomeningocele and a kyphotic deformity treated by surgical correction between 1980 and 1994 were reviewed. Different techniques of instrumentation and spinal fusion were used. The mean age at surgery was 9.5 years. The mean kyphotic angle was 121 degrees preoperatively, 50 degrees postoperatively, and 57 degrees at final follow-up. The postoperative complication rate was high. Delayed wound healing and late skin breakdown with exposure of instrumentation were common problems. Further surgery to remove protruding hardware was necessary in 18 patients. Long posterior

Osteotomy and distraction of the anterior segment of the pelvic ring in epispadias repair: case report.

We report a new therapeutic approach for bladder exstrophy and epispadias in one case of failed epispadias repair. The width of the pelvis was measured by what we defined as the anteroposterior diameter (APD) on combined transverse computed tomography (CT) scan cuts of the pelvis. The APD was half the normal value in an incontinent patient with failed epispadias repair. He underwent a supraacetabular osteotomy of the pelvis with progressive anterior distraction of the anterior segment of the pelvic ring. Four months later, hardware was removed, and the APD was near normal value. Within 9 month
