Modeling Hepatitis C Elimination Among People Who Inject Drugs in New Hampshire.

IMPORTANCE: The success of direct-acting antiviral therapies for chronic hepatitis C virus (HCV) infection led the World Health Organization to set elimination targets by 2030. For the United States to achieve these benchmarks, public health responses must target high-risk populations, such as people who inject drugs (PWID), a group with high rates of HCV incidence and low rates of treatment uptake. OBJECTIVE: To evaluate potential improvements in the HCV care cascade among PWID, focusing on improved testing, treatment uptake, and access to harm reduction. DESIGN, SETTING, AND PARTICIPANTS: …

Bridging ethics and practice: Nurses' attitudes, norms, and self-efficacy delivering wound care for people who use drugs.

BACKGROUND: There is a high demand for wound care among people who use drugs. PURPOSE: The purpose of our study was to identify difference-making factors that consistently distinguish wound care nurses who practice harm reduction from nurses who do not. METHODS: Data came from a national, cross-sectional survey of 173 wound care nurses conducted from September 2024 to June 2025. The Theory of Planned Behavior and coincidence analysis were applied to pinpoint difference makers for three different outcomes: 1) intention to carry out harm reduction practices; 2) interest in volunteering in-pers…

Youth Transitions and the Politics of Risk: Schooling amid Drug-related Violence in Mexico City.

Recent scholarship in Mexico has examined the intersection between the War on Drugs, drug-related violence, and schooling, revealing significant impacts on the educational experiences of young people. My study deepens this focus to the peripheries of Mexico City, where the expansion of the drug market and marginalization shape neighborhoods' everyday life. Situated in a community school that serves primarily working-class families, this study analyzes how teachers and school administrators perceive the risks of student drug use and distribution, as well as the strategies the school enacts to r

Nicotine pouches: an aid in smoking cessation, or a new public health hazard?

Tobacco harm reduction strategies aim to reduce the devastating health burden of smoking by providing lower-risk alternatives to those unable or unwilling to quit combustible cigarettes. Nicotine pouches have recently emerged as a novel, tobacco-free oral product category, evolving from the concept of Swedish snus but eliminating the tobacco leaf entirely. This narrative review assesses the current scientific evidence regarding the chemical composition, toxicological profile, nicotine delivery and smoking cessation efficacy, and potential public health impact of nicotine pouches. Chemical anal…

Illegal markets, broadly defined to include markets where only specific transactions are illegal, are the consequence of policy, either a pr … e large adverse collateral consequences. The paper concludes by suggesting that an extension of the harm reduction framework developed specifically for drug policy can be applied usefully to making policy choices in controlling other illegal markets.

Projected Outcomes of Reducing Federal Funding for Syringe Service Programs via Executive Order.

IMPORTANCE: Syringe service programs (SSPs) deliver evidence-based harm reduction interventions to persons who inject drugs to reduce morbidity and mortality in this population, including syringe exchange, naloxone distribution, linkage to medications for opioid use disorder, and other services. In July 2025, a federal executive order threatened federal support for SSPs in the US. OBJECTIVE: To estimate the potential long-term outcomes of halting federal funding for SSPs. DESIGN, SETTING, AND PARTICIPANTS: This decision analytical model study used a closed cohort microsimulation model of the…

Willingness to use a drug checking service for cannabis and cannabis-based products: results from an online survey of people who regularly use cannabis in France.

INTRODUCTION: As cannabis is illegal in France, there is no regulation of the quality or composition of cannabis products on the illicit market. Although access to drug checking services for people who use drugs (PUD) is possible in several harm reduction (HR) structures for a large number of illicit substances, these structures seldom have the tools needed to adequately analyze organic substances such as cannabis. METHODS: We conducted an online survey among people who use cannabis. The survey questionnaire collected data on respondents' socio-demographic characteristics, use of cannabis and…

Behavioral reactivity to ecological momentary assessment of alcohol and sexual assault protective behavioral strategies.

Ecological momentary assessment (EMA) is increasingly used to examine risk and protective factors associated with alcohol-involved sexual assault (SA). Whereas extant literature demonstrates limited reactivity (i.e., systematic behavior change) to repeated assessment of risk behaviors (e.g., drinking), we know little about the impact of EMA on protective behaviors (i.e., harm reduction strategies). The present study examined between- and within-person reactivity to EMA participation using data from a larger study of alcohol and SA protective behavioral strategy (PBS) use. First-year college wo…

Extending public health framings of drug use to the health impacts of drug production: The case of coca growers in Northeastern Colombia.

This article critically examines the absence of a public health approach to the well-being of coca leaf producers in Colombia, a group disproportionately affected by the War on Drugs. While harm reduction frameworks address drug consumption as a public health issue, no comparable strategy has been developed to tackle the health vulnerabilities afflicting those involved in drug production. Drawing on ethnographic research and interviews in Catatumbo - a coca-producing region in northeastern Colombia - this study highlights the multiple health risks faced by coca producers and leaf pickers, incl…

In Support of the Decriminalization of Personal Drug and Paraphernalia Use and Possession: Position Statement of AMERSA, Inc (Association for Multidisciplinary Education, Research, Substance Use and Addiction).

More than 25% of all arrests made nationwide are related to drug offenses, affecting almost 1.2 million people and their social networks. Furthermore, roughly 20% of people in jails and prisons across the United States are incarcerated for a drug offense and millions more are under community supervision for these charges. This criminalization of drug use has negatively affected the health and well-being of people who use drugs (PWUD) and their communities. Decriminalization-a process of removing criminal sanctions for a previously criminalized behavior-of drug use is central to harm reduction …

Rapid Response to the Legalization of Fentanyl Test Strips in Alabama: An Academic-Community Partnership.

In June 2022, Alabama legalized fentanyl test strips (FTS). In response to this new opportunity to prevent overdoses, Project Linkage, Education, and Prevention (LEAP)-an academic-community partnership providing substance use prevention services-quickly purchased FTS and started distributing them in the Birmingham area. We describe how the Addiction Prevention Coalition, a substance use education and harm reduction provider, distributed 7300 FTS in the first year of legalization via Project LEAP and discuss its efforts to decrease substance use among young people. Nonfatal Overdoses Among Pregnant Individuals With Opioid Use Disorder.

Little is understood about overdose history among pregnant individuals with opioid use disorder (OUD). We performed a cross-sectional secondary analysis of data from the OPTI-Mom 2.0 (Optimizing Pregnancy and Treatment Interventions for Moms 2.0) study (NCT03833245), a multi-site randomized controlled trial of patient navigation and usual care. We summarized participant demographics, overdose history, and substances involved in most recent overdose. Of the 102 participants with severe OUD included, 64.7% (95% CI 54.8-73.4%) had a reported a history of an overdose event and 41.2% (95% CI 31-52%

Perceptions and Experiences With Evidence-based Treatments Among People Who Use Opioids.

OBJECTIVES: Even where treatment is available, people who use drugs (PWUD) may not seek help. Few published studies examine beliefs, experiences, and perceptions of evidence-based treatment among PWUD who are not actively engaged in care. This study aimed to explore the experiences of PWUD in considering or accessing treatment and gauge receptiveness to low-threshold treatment models. METHODS: A purposeful sample of participants actively using opioids and with previous interest in or experience with treatment was recruited from a harm reduction program in Chicago. Semistructured interviews we…

Syringe Services Programs' Role in Ending the HIV Epidemic in the U.S.: Why We Cannot Do It Without Them.

Diagnoses of HIV among people who inject drugs have increased in the U.S. during 2014-2018 for the first time in 2 decades, and multiple HIV outbreaks have been detected among people who inject drugs since 2015. These epidemiologic trends pose a significant concern for achieving goals of the federal initiative for Ending the HIV Epidemic in the U.S. Syringe services programs are cost effective, safe, and highly effective in reducing HIV transmission and are an essential component of a comprehensive, integrated approach to addressing these concerns. Yet, geographic coverage of these programs re

Injecting substance use in prisons in the United States: a case for needle exchange programs.

Despite efforts to prevent illicit substance use in prisons, inmates in many countries still have access to them, along with associated paraphernalia. In the United States, a large proportion of people in prison are there due to drug-related charges. The secretive nature of illicit substance use in prison and the opposition to implementing harm reduction interventions such as needle exchange programs have put prisoners who engage in such practices at high risk for infectious bloodborne diseases. While states have their reasons for opposing the implementation of needle exchange programs in pris…

Analytical findings in used syringes from a syringe exchange program.

BACKGROUND: Since it's first implementatation in 1984, Syringe Exchange Programs (SEP) are a critical component of harm reduction interventions among people who inject drugs.. The aim of this work was to use a scientific analytical approach to obtain drug use information through the analysis of the content of used syringes. METHODS: 357 syringes were collected in New York City and submitted to qualitative analysis. Screening analysis was performed by gas chromatography mass spectrometry (GC-MS) and confirmatory analysis by liquid chromatography quadrupole time of flight mass spectrometry (LC-…
