Arrhythmias and conduction disorders in patients with viral heart disease.

Viral heart disease comprises of two cardiovascular entities being evoked by viral infection: acute viral myocarditis and viral cardiomyopathy. Viral myocarditis may completely resolve leaving no traceable sign or cause ongoing inflammation with subsequent development of hypokinetic dilated/non-dilated cardiomyopathy. The exact epidemiology of viral myocarditis remains unknown due to its sometimes asymptomatic course, but according to the Global Burden of Disease Study 2019, the prevalence of myocarditis in young adults is estimated to range between 6.1 per 100,000 in men and 4.4 per 100,000 i

Fetal cardiac arrhythmia detection and in utero therapy.

The human fetal heart develops arrhythmias and conduction disturbances in response to ischemia, inflammation, electrolyte disturbances, altered load states, structural defects, inherited genetic conditions, and many other causes. Yet sinus rhythm is present without altered rate or rhythm in some of the most serious electrophysiological diseases, which makes detection of diseases of the fetal conduction system challenging in the absence of magnetocardiographic or electrocardiographic recording techniques. Life-threatening changes in QRS or QT intervals can be completely unrecognized if heart ra

[Holter monitoring of patients treated with cytostatic agents].

After cytostatic treatment severe arrhythmias, the development of angina pectoris and even the development of acute myocardial infarction and sudden death were observed. Therefore we made in 42 patients with malignant haematological disease treated with cytostatics 96 Holter monitorings of the electrocardiographic signal. The monitoring was made during the administration of cytostatics as well as during the time interval between the administration of combinations of cytostatics. In both instances (during the administration and during the interval between administration) we recorded a surprisin

[Extrasystole in the neonatal period].

We investigated the clinical course in 35 healthy newborn with premature beats and studied the literature with regard to their frequency, pathogenesis, and therapy with the following results: Investigations with continuous monitoring or long-term ECG registration show a frequency of 7-25%. In literature, almost one-fourth, in our patients one-half of the newborn have ventricular premature beats. These will probably disappear later than atrial premature beats, but their prognosis is not worse. The premature beats of newborn are often complex (blocked or aberrant conduction, bigeminus, tachycard

[Indications and risks of anti-arrhythmia treatment with propafenone (author's transl)].

Propafenone, a new anti-arrhythmia drug, was given at an average dose of 70-140 mg (1-2 mg/kg body-weight) to 124 patients with various types of cardiac arrhythmias. It proved successful in patients with ectopic beats and tachycardias of atrial or ventricular origin. Ectopic beats were suppressed in 40 of 81 patients, while sinus rhythm was restored in 15 of 19 patients with paroxysmal tachycardias. On the other hand, sinus rhythm was restored in only 5 of 22 patients with atrial tachy-arrhythmias, while in most of the others in this group the heart rate decreased markedly during propafenone i

Cardiac arrhythmias: diagnosis and management. The bradycardias.

OBJECTIVE: To review the diagnosis and management of cardiac arrhythmias in a two-part presentation. DATA SOURCES: Articles and published peer-review abstracts on tachycardias and bradycardias. SUMMARY OF REVIEW: Bradycardias are caused by a failure of the sinus node to generate normal impulses or due to a defect in cardiac conduction that in turn causes a delay or failure of impulse propagation. During sleep, the heart rate may decrease to 30 beats per minute (bpm) with episodes of sinoatrial block, junctional rhythms and first and second degree atrioventricular block that occur often enoug

Toward cardiac electrophysiology digital twins with an efficient open source scalable solver on GPU clusters.

Modelling and simulation are essential in biomedicine, and specifically in computational cardiology. Reliable, efficient and accurate solvers are critical. This study presents an open-source, GPU-based cardiac electrophysiology solver for scalable multiscale simulations (MONOALG3D), incorporating conduction system calibration and performance optimization. The solver employs the monodomain equation coupled with the Purkinje network, solved via the finite volume method, featuring a GPU-based linear solver and concurrent simulation dispatch with MPI. We demonstrate a [Formula: see text] speedup o

Single-center initial experience with a new pulsed-field ablation system: pulmonary vein isolation lesions and beyond.

Pulsed-field ablation (PFA) is a newer energy delivery method, an alternative to thermal lesion, allowing for safe and durable pulmonary vein isolation in the context of atrial fibrillation ablation. Graded tissue sensitivity allows for specifically targeting atrial cardiomyocytes, rendering collateral damage to nearby nerves and vessels unlikely. In the present rapid communication, we report on our center's initial experience with a novel PFA platform and describe the ways in which it can be used targeting sites beyond PV ostia and antra, in the context of atrial fibrillation/flutter/tachycar

Electrocardiographic spiked-helmet sign in critical non-cardiac illness.

The spiked-helmet sign (SHS) is an electrocardiographic finding characterised by a distinctive pattern associated with critical illness, resolving once the clinical condition improves. While the underlying mechanism remains uncertain, different mechanisms have been proposed to account for these findings including changes in intrathoracic or intra-abdominal pressures, transient compression of the coronary arteries, repolarisation abnormalities and electromechanical artefacts. We describe the development of the SHS in a patient with underlying left bundle branch pacing following the development

Implications for neutrophils in cardiac arrhythmias.

Cardiac arrhythmias commonly occur as a result of aberrant electrical impulse formation or conduction in the myocardium. Frequently discussed triggers include underlying heart diseases such as myocardial ischemia, electrolyte imbalances, or genetic anomalies of ion channels involved in the tightly regulated cardiac action potential. Recently, the role of innate immune cells in the onset of arrhythmic events has been highlighted in numerous studies, correlating leukocyte expansion in the myocardium to increased arrhythmic burden. Here, we aim to call attention to the role of neutrophils in the

Computational Analysis of the Action of Chloroquine on Short QT Syndrome Variant 1 and Variant 3 in Human Ventricles.

AIMS: The short QT syndrome (SQTS) is a rare genetic disorder associated with arrhythmias and sudden cardiac death (SCD). The SQTI and SQT3, SQTS variants, result from gain-of-function mutations (N588K and D172N, respectively) in the KCNH2-encoded and KCNJ2-encoded potassium channels, in which treatment with potassium channel blocking agents has demonstrated some efficacy. This study used in silico modelling to gain mechanistic insights into the actions of anti-malarial drug chloroquine (CQ) in the setting of SQTI and SQT3. METHODS AND RESULTS: The ten Tusscher et al. human ventricle model wa
