Modifications to the Hip Arthroscopy Technique When Performing Combined Hip Arthroscopy and Periacetabular Osteotomy.

In the realm of hip preservation, hip arthroscopy is often used to address intra-articular impingement pathology, whereas periacetabular osteotomy (PAO) is used to address dysplasia and instability. Indications to combine these 2 procedures include hip dysplasia and symptomatic instability with a concomitant symptomatic labral tear or the other symptomatic intra-articular pathology (i.e., loose body, chondral flap). The arthroscopic portion of the procedure allows repair of the injured labrum and close inspection of the hip joint, and the PAO addresses undercoverage and/or inappropriate versio…

Return to sport after hip arthroscopy: aggregate recommendations from high-volume hip arthroscopy centers.

Hip arthroscopy is a minimally invasive surgical technique often performed in athletes who want an expeditious return to sport. To the authors' knowledge, no studies in the literature provide a time frame or criteria for return to sport after hip arthroscopy. The purpose of this study was to develop an aggregate recommendation for return to sport after hip arthroscopy based on data assimilated from high-volume hip arthroscopy centers. Twenty-seven orthopedic surgeons from high-volume hip arthroscopy centers completed a survey regarding return to sport after hip arthroscopy. The questionnaire a…

Calcium pyrophosphate dihydrate deposition in the trochanteric hip bursa presenting as acute hip pain.

Acute hip pain is a common reason for attendance to hospital. Immediate diagnoses include occult fractures, infectious and non-infectious inflammatory processes such as gout and pseudogout. Calcium pyrophosphate deposition disease (CPDD) is a rheumatological syndrome affecting articular cartilage and/or synovial fluid. It can occur as a clinical presentation (pseudogout) where calcium pyrophosphate dihydrate (CPPD) crystals are deposited within the joint space or radiographically, as chondrocalcinosis (CC), where CPPD crystals are deposited onto articular cartilages. Extraarticular manifestati…

Clinical outcome of Zweymüller total hip arthroplasty for patients with high congenital hip dislocation.

Treatment of high congenital dislocation of the hip (CDH) remains controversial. We report the outcome of hip arthroplasty using a cementless threaded cup and a cementless straight stem in patients with high congenital hip dislocation. Between January 2001 and August 2004, 17 patients with high congenital hip dislocation were treated. During surgery, at least 25% of the cup was anchored in bone. By monitoring somatosensory-evoked potentials (SEPs) of the common peroneal nerve we were able to reduce the femoral head into position in the true acetabulum by releasing soft tissues. A bulk autogeno…

Long-term outcome of shelf grafts in total hip arthroplasty for developmental hip dysplasia.

Primary total hip arthroplasty (THA) in patients with osteoarthrosis secondary to developmental hip dysplasia is often complex due to anterolateral acetabular bone deficiency. The use of femoral head (shelf) autograft during the index arthroplasty provides nonimmunogenic, osteoconductive support with the potential for enhanced bone stock should revision surgery be required. Few long-term studies document the outcome of the use of shelf grafts in primary THA or quantify the need for further bone graft at revision surgery. A retrospective analysis was conducted of a single surgeon's series of 31…

Tight fit technique in primary hybrid total hip arthroplasty for patients with hip dysplasia.

This article presents the midterm results of hybrid total hip arthroplasty for patients with hip dysplasia by use of a tight fit technique for the femoral component. We followed up 113 hips in 99 patients for a mean of 11 years. All final femoral rasps used in this study overrasped by 0.5 to 1.0 mm for stem insertion, resulting in relatively thin cement mantles. Both components of one hip were removed because of infection. The other 5 acetabular components were revised for osteolysis, recurrent dislocation, or dislodgement of the polyethylene liner. No femoral component was revised for aseptic…

Hip disability and osteoarthritis outcome score (HOOS)--validity and responsiveness in total hip replacement.

BACKGROUND: The aim of the study was to evaluate if physical functions usually associated with a younger population were of importance for an older population, and to construct an outcome measure for hip osteoarthritis with improved responsiveness compared to the Western Ontario McMaster osteoarthritis score (WOMAC LK 3.0). METHODS: A 40 item questionnaire (hip disability and osteoarthritis outcome score, HOOS) was constructed to assess patient-relevant outcomes in five separate subscales (pain, symptoms, activity of daily living, sport and recreation function and hip related quality of life)…

[Hip dysplasia and congenital hip dislocation - a roentgenometric study in 110 families (author's transl)].

In human genetics and orthopedics quite different answers have been given to the question of hereditary transmission and frequency of hip dysplasia in families of children with congenital hip dislocation. We therefore have made roentgenometric measurements of 110 parents of children with congenital hip dislocation. In 25% we found abnormal flat acetabulae, whereas 12% had pathologic deep hips. This may propose a new concept of morphology of congenital hip dysplasia.

Developmental dysplasia of the hip--good results of later total hip arthroplasty: 7135 primary total hip arthroplasties after developmental dysplasia of the hip compared with 59774 total hip arthroplasties in idiopathic coxarthrosis followed for 0 to 15 years in the Norwegian Arthroplasty Register.

The purpose of the present article was to compare the results of primary total hip arthroplasty (THA) done because of developmental dysplasia of the hip (DDH) with the results of THA done because of idiopathic coxarthrosis (osteoarthritis) using data from the nationwide Norwegian Arthroplasty Register (NAR). In the period from 1987 to 2003, 84871 primary THAs were reported to the NAR. Of these, 6347 (7.5%) were performed because of sequelae after DDH, 788 (0.9%) because of sequelae after DDH with complete dislocation of the femoral head, and 59774 (71.0%) because of idiopathic coxarthrosis. Th…

Select large language models outperform hip preservation experts on consensus-based hip preservation questionnaire.

PURPOSE: Artificial intelligence (AI) is increasingly utilized in medical education and clinical contexts, yet few studies compare the performance of large language models (LLMs) to subspecialized experts in providing guideline-based medical information on hip preservation. The purpose of this study was to evaluate the performance of three LLMs compared to a panel of international hip preservation experts in answering guideline-based questions related to femoroacetabular impingement syndrome, hip dysplasia and microinstability of the hip. METHODS: A 21-item questionnaire was developed based o…

Outcomes in patients undergoing primary total hip arthroplasty with history of prior hip arthroscopy.

INTRODUCTION: As the number of total hip arthroscopies performed rises, further research is needed on the impact hip arthroscopy (HA) may have on total hip arthroplasty (THA). This study aimed to compare clinical and patient-reported outcomes of THA in patients with and without a history of HA. METHODS: We retrospectively reviewed 167 patients who underwent unilateral, primary THA following ipsilateral HA from 2012 to 2024 (HA, n = 167). Patients were propensity matched 3:1 to THA patients without history of ipsilateral HA (NHA, n = 448) based on age, sex, race, smoking status, American Socie…

Hip abduction with hip extension produces the largest obturator nerve displacement: a cadaveric investigation.

BACKGROUND: The obturator nerve may be injured during surgery or trauma. Neurodynamic testing (NDT) is theorized to assist in obturator nerve injury identification, but the optimum hip position for NDT has not been biomechanically supported. OBJECTIVES: This study evaluated the displacement and strain of the obturator nerve within the pelvis using three NDT positions of the hip (neutral, flexion, and extension) combined with hip abduction. DESIGN: One-way repeated measures. METHODS: Nine cadavers were selected by consecutive sampling, and metal markers were glued to the exposed intrapelvic …
